PAST PERFORMANCE QUESTIONNAIRE

SOLICITATION NUMBER: 36C24521Q0407

The offeror shall provide the following questionnaire to up to three examples who can attest to the offeror’s performance covering contracts or jobs that are recent, that is within Five years, and relevant, that is of the same type and scope of work. The offeror shall provide a list of these references, including the company/agency name, Point of Contact (POC), and telephone number or email address of the POC as part of their quote in order to be considered for award. Information to substantial the relevancy and recency regarding the references must also be included. Offerors should instruct their chosen references that completed questionnaires must be submitted by the exact date and time specified for receipt of offers, that is, August 24, 2021, 1400 EST, to Crystal S. Hinton, Department of Veterans, VA Medical Center Annex, 209 West Fayette Street, Suite 611 Baltimore, Maryland 21201, email: crystal.hinton@va.gov

Acceptable: Satisfactory confidence based on the offeror’s recent/relevant performance record to successfully perform the required effort.

Marginal: Limited confidence based on the offeror’s recent/relevant performance record; the Government has a low expectation that the offeror will successfully perform the required effort.

Unacceptable: No confidence based on the offeror’s recent/relevant performance record. The Government has no expectation that the offeror will be able to successfully perform the required effort.

Neutral: Unknown confidence (neutral). No recent/relevant performance record is available, or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.
