Attachment 4 - DQ Affirmation Page

This is a Disqualification (DQ) Affirmation Page document related to Colorado's Talent Innovation Qualitative Evaluation contract opportunity. The document serves as a certification form for bidders to affirm their eligibility and qualification status for participation in the procurement process. The affirmation page requires bidders to confirm compliance with specific disqualification criteria and any relevant statutory or regulatory requirements established by the State of Colorado. The document establishes the procedural framework through which prospective vendors must verify their standing and eligibility prior to bid submission, ensuring that only qualified entities meeting the state's predetermined standards advance in the evaluation process.

The DQ Affirmation Page functions as a gatekeeping mechanism within the competitive bidding process, requiring vendors to make binding representations regarding their qualifications and absence of disqualifying factors. By executing this affirmation, bidders certify their understanding of and compliance with all applicable disqualification provisions, thereby streamlining the evaluation process and reducing the risk of award to ineligible vendors. This document is typically completed and submitted alongside a bid proposal to formally establish each vendor's eligibility status at the point of bid submission.
