About this file

This document is a Quality Assurance Plan (QAP) for a Dust Abatement contract, specifically detailing performance requirements for distribution equipment and application processes. The QAP provides a comprehensive contractor performance checklist that outlines quality standards, acceptable quality levels, inspection methods, potential penalties, and performance incentives for dust palliative material application.

The plan specifies strict technical requirements for distribution equipment, including uniform material application, precise application rates (0.96 to 1.65 lbs/sq.yd), accurate weight measurement, and compliance with state and federal vehicle requirements. Application standards mandate precise material discharge rates, environmental protection (preventing material flow into streams), and protection of adjacent structures. Penalties for non-compliance include potential rework at contractor expense, compensation withholding, poor performance ratings, and potential contract termination, while exceptional performance could result in superior ratings and full compensation.

• Shall be equipped and operated so that material may be applied uniformly on variable widths of surface.

• Shall be able to determine and control rates from

0.96 to 1.65lbs/sq.yd with even distribution.

• Weight of flakes in the distribution truck shall be accurately determined prior to application.

• Spread products shall be uniform across the intended treatment area.

• Shall include an accurate measuring device or a calibrated tank.

• Variation from specified application rate shall not exceed 10% of the individual loads, and 2% rate for the entire project.

• For flake, the weight of the material placed shall be easily determined during application.

• All spreading and hauling equipment shall comply with State and Federal requirements including GVW.

• Visual verification • Contractor may be required to re-work, at their expense, the unacceptable section.

• Government may withhold compensation.

• Contractor may receive poor performance rating.

• Contractor would receive a superior performance rating.

• Accidental spillage or area of excess material that are traffic hazard will be covered with road surface material at the Contractor’s expense.

• If application rate used by the Contractor is greater than that specified, the additional material needed to complete coverage will be at the Contractor’s expense.

• Will be based on concentration shown on the manufacturer’s certificate and test results.

• Adjacent structures and trees shall be protected from spattering and marring.

• Material shall only be discharged in approved areas and not be allowed to flow into ditches and streams.

• If the Contractor exceeds the specified rate or total quantity per Subitem by more the 5%, no compensation will be made for the additional quantity.

• If the Contractor’s application is below the specified rate or total quantity per Subitem, the remaining amount will be applied at locations and rate directed by the COR.

• Visual verification.

• Will inspect completed application as the work progresses.

• Field samples taken from the distributor or hauling vehicles.

• Field sample prior to application to make sure the products meets minimum concentrations specified.

• Laboratory Quality Assurance Test done by the Forest Service.

• Contractor may be required to re-work, at their expense, the unacceptable portions of work.

• Contractor may receive poor performance rating.

• Contract may be terminated with repeated deficiencies.
