About this file

This is a Parent Interview Form (Form C-2.3B) used by the South Carolina juvenile justice system to collect comprehensive background information on juveniles involved in the criminal justice system. The form is completed during initial intake assessments and serves as a standardized instrument for gathering data from parents or legal guardians. The interview covers multiple domains including the current charges and alleged offenses, family structure and composition, custody arrangements, family functioning dynamics, developmental history, mental health symptoms and treatment history, school and employment functioning, peer relationships and social connections, and living arrangements within the community. The form requires documentation of parental information, household composition, current behavioral concerns at home, disciplinary practices, and any history of out-of-home placements or Department of Social Services involvement.

The form systematically captures information regarding family risk factors including domestic violence history, parental mental health and substance abuse issues, family legal involvement, and significant life events that may have impacted the juvenile. Additionally, it documents the juvenile's developmental milestones, current health status, medication use, and any history of abuse or neglect. The assessment includes evaluation of the juvenile's educational progress, special education needs, school disciplinary history, peer relationships, online activity and social media use, sexual activity and reproductive history, and overall behavioral functioning in community settings. The form concludes by eliciting parental perspectives on the juvenile's strengths and areas for improvement, family strengths and needs, placement preferences, and any additional concerns or questions. This intake documentation directly supports the related state contract opportunity for psychological evaluation services for juveniles within the South Carolina juvenile justice system.
