About this file

This file is a Juvenile Clinical Interview form (Form C-2.3A, dated 03-2018) developed by the South Carolina Department of Juvenile Justice (DJJ), under the direction of Executive Director Freddie B. Pough and Governor Henry McMaster. The form serves as a comprehensive assessment tool designed to document clinical interviews with juveniles in the state's juvenile justice system. The instrument systematically collects information across multiple domains including criminal history, family functioning, adolescent development, substance use patterns, mental health symptomatology, behavioral and personality functioning, school performance, and peer relationships. The form is structured to gather detailed background information from the juvenile's perspective, including family composition, living arrangements, disciplinary structure, previous counseling and treatment history, current medications, and trauma exposure. The assessment includes extensive screening questions for various mental health conditions such as ADHD, Oppositional Defiant Disorder, Conduct Disorder, depression, anxiety disorders, PTSD, eating disorders, and psychotic symptoms, with guidance for clinicians to consider severity levels, onset timing, frequency, and contextual factors before making diagnostic determinations.

The form incorporates standardized screening criteria aligned with diagnostic frameworks while accounting for factors unique to the juvenile justice population, including substance use disorders, behavioral health comorbidities, and trauma-related conditions. The interview protocol specifically notes considerations for less common disorders such as Autism Spectrum Disorder and neurocognitive disorders, as well as the need to distinguish between substance-induced symptoms and primary mental health conditions. The assessment captures information on current and historical substance use across multiple drug categories (tobacco, alcohol, marijuana, cocaine, methamphetamine, heroin, prescription pills, and hallucinogens), documenting patterns and consequences. Additionally, the form documents school attendance and performance, peer relationships including gang involvement, sexual health history, and identified strengths and future goals. This comprehensive clinical interview tool appears designed to support individualized case management, treatment planning, and risk assessment within South Carolina's juvenile justice system.

Mental Health Symptoms The following is a list of questions to assist in making a diagnosis. Before making a diagnosis, factors such as number of symptoms, frequency, age of onset, how recently the symptoms occurred, and context in which the symptoms occur, all must be taken into consideration. It is important to remember that information from the parent, available records, and the juvenile’s presentation should also be considered. If a juvenile seems to have symptoms of a particular disorder, but not enough for a full diagnosis, consider an other/unspecified disorder.

Also keep in mind whether the juvenile may have a disorder that is less common among the DJJ population (for example, Autism Spectrum Disorder or a neurocognitive disorder) or whether the juvenile’s symptoms may be substance-induced or related to withdrawal.
