STATEMENT OF WORK

FOR

PURCHASE AND INSTALLATION OF FURNITURE

367th Cyberspace Operations Squadron

LACKLAND AFB, TEXAS

1. GENERAL DESCRIPTION: The Contractor shall provide management, labor, equipment, tools, materials, delivery, transportation, disposal and incidentals necessary to accomplish the work required within this Statement of Work (SOW). The work required includes but is not limited to the design, purchase of new products, and the installation in building 2058, 367COS spaces (Rooms 134 & 382) located at 250 Hall Blvd, San Antonio TX, 78243. If the contractor is unable to provide any of the items listed in this document, these deficiencies must be identified clearly in contractor’s offer. Specific, detailed installation drawings and specifications are the responsibility of the contractor. All work shall be in conformance with the specifications for installation of the carpet. All materials and accessories shall be installed in accordance with industry standards and manufacturer’s written recommendations.

a) The contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental risks. These policies shall be in compliance with Federal and Installation policies and procedures.

b) The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate or alternates who shall act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer (CO) within 10 days after contract award, if required. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to the daily operations of this contract on the installation.

c) The contractor shall provide a Project Manager (PM) post award with experience installing the contractor’s product. PM will be responsible for project site management, scheduling, coordination and technical assistance to the Air Force representative as required throughout the project.

d) Installation Access. The Contractor shall enter JBSA-Lackland through the appropriate visitor control center with a current delivery invoice or bill of lading and must be able to show a current vehicle registration, current proof of insurance, a valid and current driver’s license for the driver, and a valid and current driver’s license or other state identification for any passenger in the vehicle. The Contractor is required to be a United States citizen and shall be escorted by an authorized escort 100% of the time while in the facility. The Contractor shall not bring any Electromagnetic Emission Devices (EED's) or other devices that connects

8/21/2023 2 to Bluetooth and/or Wi-Fi into the facility. EEDs include radio/radar transmitters, navigation aids/instrumentation, signaling, intrusion detection, identification devices, mobile and fixed business/radio communications equipment, MARS, CDs, and amateur radios. These are to include but not limited to Fitbit, Apple watches, cell phones, two-way pagers, MP3 players, IPODS, flash drives, tablets, laptops, or any other electronic media. All electronic devices will be validated that they meet security requirements prior to entry into the facility. Any/all boxes/items being brought into the SCIF will require 100% inspection by a properly cleared member of the 67 CW to ensure no unauthorized items are introduced into the facility. Contractor will need to remove items from the boxes outside of the facility and ensure all items are inspected by a properly cleared member of the 67 CW to ensure no unauthorized items are introduced into the facility.

e) The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the government as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees (i.e., name tags, badges, uniforms).

f) The vendor shall manage ordering, shipment, delivery and installation of all furniture and supplies from the manufacturer to final acceptance by the designated Government Acceptor. In no case is the Government responsible to store product above and beyond the product required during installation, or to accept direct delivery of freight for the contractor.

g) Vehicle registration, proof of insurance and a valid driver’s license shall be presented to gain base access.

h) The contractor shall be responsible for coordination of an escort through any controlled areas through the assigned Government Acceptor.

i) Lost vehicle and entry passes shall be reported immediately to the Security Police as well as the Government Acceptor in accordance with the local security forces policy.

j) The contractor shall be responsible for storage of all furniture, components, and anything else needed to install furniture prior to and during installation.

k) The contractor shall provide the necessary resources to manage and administer the contract. The contractor shall, as a minimum, maintain a single point of contact for this effort.

l) Contractor shall notify the Contracting Officer (502d CONS) in writing of any conditions which shall prevent satisfactory start and completion of furniture installation.

m) Contractor is responsible for daily cleanup of the project area.

2. SCOPE: For this project, the following is required for purchase, delivery, and complete installation:

A. Workstations features (bldg. 2058 1st and 3rd floor). Workstation panels will be 42” minimum with lower panels being utilized in collaboration areas and higher panels in areas that require more privacy. Finishes utilized will include perforated metal panels at lower sur¬faces, fabric wrapped panels, plastic laminate work-surfaces, painted metal file cabinets and trim. A durable, stain resistant fabric will be used for the cushion on the mobile pedestal filing cabinets.

C. The Contractor will run fiber optic cabling from existing zone boxes through furniture walls to terminal jack for Workstations.

D. The Contractor will run CAT6 cabling from existing zone boxes through furniture walls to terminal jack for Printer Area.

E. Demo and dispose of existing furniture, and the install the above specified furniture as referenced.

F. Period of performance is 120 days after the contract has been awarded.

G. Once the contract has been awarded, it becomes the responsibility of the contractor to validate the actual site conditions as well as any specific needs of the customer. In no case shall prices exceed the Not-to-Exceed (NTE) amount established in the Tier I contract. It is understood that the actual site parameters or certain customer requirements may deviate from the standard typical layouts; therefore, any deviation that causes an increase in the price submitted during the offer process must be documented in writing and submitted to the Contracting Officer for review prior to implementing changes, along with a floor plan reference to show just cause for the increase in price in comparison to original offer.

3. ACCESSORIES AND INCIDENTALS: Contractor shall provide all incidentals necessary, special tools and other parts needed to properly install the furniture. All accessories shall be installed in accordance with the manufacturer’s recommendation.

4. WARRANTIES: Contractor shall provide all commercial warranties for product as provided by the manufacturer.

A. Contractor shall provide a manufacturer's warranty for all equipment items provided under this contract.

B. Contractor shall provide a 12‐month warranty on workmanship to begin after delivery, installation, and final acceptance

5. LOCATION: Project work is to be performed on Lackland AFB. Project area is in Building 2058, 1st floor and 3rd floor, 367 COS. The organization is the 367th Cyberspace Operations Squadron. The building is located at 250 Hall Blvd, JBSA-Kelly Annex, 78243. The project includes ample surrounding parking and accessibility areas. No cell phones or two-way communication devices will be allowed into the facility. The contractor shall coordinate with the 367th COS Area Manager, 977-6589 to gain base and facility access. Vehicle registration, proof of insurance as wells as names, date of birth and valid driver’s license number for all drivers shall be presented to gain base access.

6. HOURS OF OPERATION: Project work is required between 7:00 am. and 5:00 pm, Monday through Friday, excluding holidays.

Access to work sites will be restricted to these hours and days, unless approved otherwise by the 502d Contracting Officer.

a) Recognized US Federal Holidays are as follows: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas. If the holiday falls on a Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.

b) When the contractor requires to work other than the agreed upon times, a written request shall be submitted to the Government Acceptor for approval. Written approval from the Contracting Officer must be received before proceeding with the work. The request shall be submitted at least five (5) work-days prior to the proposed performance of work, and it shall indicate the day/days, time/amount of work, and reason why performance is necessary during nonstandard hours.

7. INSTALLATION: All related accessories shall be installed in accordance with industry standards, manufacturer’s recommendations, and call order requirements and per the approved design. Furniture will be installed once the old furniture is removal and disposed of by the contractor. Contractor shall coordinate installation start date no less than one calendar week prior to start of installation. Contractor shall repair any damages caused to government property as a result of installation at no additional cost to the government.

8. CLEAN UP: No government dumpsters are available for use. All debris, trash and refuse shall be removed from the work site on a daily basis. At completion of project, all areas shall be left clean and in a final professional appearance. All Contractor generated refuse and waste shall be disposed of in the proper manners outside the physical boundaries of the installation. The Contractor may use a waste container provided by the Contractor during the Period of Performance. If the contractor chooses to supply a dumpster or otherwise requires a laydown area, this must be requested from the government at least 14 days before work begins.

9. DELIVERY/STORAGE: Facility site does not include a loading dock. Product delivery from manufacturer to Contractor’s site is required. The contractor is responsible for transportation to the worksite or shall make arrangements for a laydown area two weeks in advance. Entrance to facility will be coordinated through the Government representative. The Contractor is responsible for transportation of all new products, loading, unloading, storage and all necessary movement from factory to final onsite installation.

A. The contractor Project Manager shall be responsible for on-site coordination of all staging and deliveries of their product, to include access to and operation of any equipment needed for unloading and movement of materials or product.

B. Contractor shall be responsible for determination of the appropriate delivery method and handling, based on site conditions to include the existence loading docks and equipment at any given location.

C. Work shall comply with applicable building standards and codes, specifically: National Electrical Code (National Fire Protection Association (NFPA 70) and National Life Safety Code (NFPA 101). Contractor is required to hire a licensed Electrician to connect power to all workstations.

D. The Contractor shall have the Electrician:

1. Disconnect all existing electrical whips from existing furniture.

2. Complete demolition of all unused electrical whips needed for new furniture.

3. Install all new whips as needed for new furniture.

E. The contractor shall be responsible for following all safety and security guidelines within the area in which work is being performed, to include securing any contractor owned tools or equipment, and any on site storage being utilized.

F. The contractor shall maintain accountability and control of any keys and/or passes provided and shall return them to the Government Acceptor upon completion of work.

G. The contractor shall prevent its personnel from entering any area other than the designated work area to complete the project herein.

H. The contractor shall maintain a means of egress within all designated work areas to comply with fire codes.

I. The contractor shall ensure its personnel eat, drink, or smoke only in designated areas.

J. The contractor shall provide the government with a full set of as-built drawing, to include any shop and technical drawings, in CAD and PDF format on compact disc (CD).

10. INSPECTION/ACCEPTANCE: The government shall inspect all work performed by the contractor. Work performance shall be deemed satisfactory and acceptable to the government when the 367 COS Area Manager approves and declares the project satisfactory. The government will document all inspections. Validation between the contractor and the Government Acceptor that all product has been installed according to manufacturer guidelines and customer specifications.

A. Substantial Completion Inspection.

1. When the Furniture Vendor considers the entire Work or designated part thereof substantially complete, the Furniture Vendor shall notify the General Contractor in writing that the Work will be ready for Substantial Completion Inspection on a date certain.

2. The Furniture Vendor shall include with this notice a list of items to be completed or corrected prior to final inspection.

3. On the date indicated by the Furniture Vendor or as soon thereafter as is practicable, the Owner, General Contractor, the Interior Designer, and the Furniture Vendor shall inspect the Work and if the Owner and the Interior Designer determine that the Work is substantially complete a notice of Substantial Completion shall be issued by the Owner fixing the date of Substantial Completion.

4. The Interior Designer will provide with this certificate a list of items to be completed prior to final inspection (Pre‐ Final Punchlist).

5. The Pre‐Final Punchlist may include additional items not included in the Furniture Vendor’s list, which are deemed necessary by the Interior Designer or by the Owner to correct or complete prior to Final Inspection.

B. Final Inspection

1. The Furniture Vendor shall complete the items listed on the Pre‐Final Punchlist prior to Final Inspection.

2. The Furniture Vendor shall complete this work within 30 days of the date of Substantial Completion.

3. Upon completion of the Pre‐Final Punch‐list, Furniture Vendor shall give written notice to the Owner and Interior Designer that the Work will be ready for Final Inspection on a date certain.

4. On this date, or as soon thereafter and is practicable, the Owner, Interior Designer and Furniture Vendor shall inspect the Work and the Interior Designer shall submit to the Furniture Vendor a list of items which the Owner and the Interior Designer have determined to require correction or completion before the Work will be accepted by the Owner (Final Punch‐list).

5. The Furniture Vendor shall correct or complete all items on the Final Punch‐list before Acceptance and Final Payment.
