About this file
This is a federal contracting representation form used for acquisitions of leasehold interests in real property. The form implements FAR clause 52.204-24 and requires offerors to represent whether they will provide covered telecommunications equipment or services in performance of government contracts resulting from a solicitation.
The form mandates that offerors complete a representation indicating whether they will or will not provide covered telecommunications equipment or services as defined in FAR clause 52.204-25. If an offeror responds affirmatively, they must provide detailed disclosures including: all covered telecommunications equipment and services offered with brand, model number, and item description; explanation of the proposed use and any factors relevant to determining permissibility under the prohibition established by Section 889(a)(1)(A) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019; for services, the entity name, unique entity identifier, and CAGE code of the service provider; and for equipment, the entity name, unique entity identifier, CAGE code, and whether the entity was the original equipment manufacturer or distributor. The form includes exceptions for services connecting to third-party facilities and telecommunications equipment that cannot route, redirect, or provide visibility into user data. The offeror or legally authorized representative must sign the form with name, address, zip code, telephone number, and date before submission to the Lease Contracting Officer.
