About this file

This is a Business Proposal Attachment E document issued by the State of Indiana (IDOA) as part of a Request for Proposals (RFP 26-85126) for unspecified products and/or services. The attachment serves as an instruction template for respondents to complete various sections of their business proposals, including company structure, financial information, references, and relevant experience. The document does not specify quantities, response dates, due dates, site visits, or bidder conferences, as it is a form template rather than a formal RFP announcement. Contract terms, award dates, and contract duration are referenced as existing within other sections of the RFP but are not detailed in this attachment.

The proposal requires respondents to demonstrate financial stability through Dunn & Bradstreet reports or audited financial statements for the two most recently completed fiscal years, provide three customer references with direct submission to idoareferences@idoa.in.gov, and confirm registration with the Indiana Secretary of State prior to contract negotiations. Respondents must address diversity and subcontracting opportunities with certified Minority & Women's Business Enterprises (MBE/WBE) and Indiana Veteran Owned Small Business (IVOSB) firms, disclose business continuity and disaster recovery plans, and outline data security processes for state information. The attachment also requires respondents to indicate acceptance of cloud-based system terms and conditions (IaaS, PaaS, or SaaS as applicable) and express willingness to extend pricing to other governmental bodies. No specific pricing terms, set-asides, incumbent information, or funding amounts are specified in this template document.
