About this file

This is a Vendor Disclosure Statement form issued by the State of Colorado on July 19, 2024, pertaining to Contract Performance Outside the United States or Colorado under Colorado Revised Statute 24-102-206. The form applies to all state contracts and solicitations for services executed after August 3, 2007, and must be completed and returned to the contracting agency when vendors have not disclosed relevant information in the Statement of Work. The document requires vendors to disclose whether any services under the contract or subcontracts are anticipated to be performed outside the United States or Colorado, and if so, to identify the specific countries and states where work will occur.

The form mandates that vendors explain the necessity or advantage of performing contract work outside the United States or Colorado and requires submission of the contract routing number and vendor name. The disclosure requirement does not apply to contracts to which the State is a party under Medicare, the Colorado Medical Assistance Act, the Children's Basic Health Plan Act, or the Colorado Indigent Care Program. This disclosure requirement ensures transparency regarding offshore or out-of-state contract performance and allows the state to evaluate vendor proposals based on work location and the justification for performance outside Colorado.
