About this file
This is an Appendix C Statement of Work Response Template for a Colorado Request for Proposals (RFP) issued by the Colorado Medical Board (under the Department of Regulatory Agencies) to establish peer health assistance services for physicians, physician assistants, and anesthesiologist assistants. The RFP seeks a qualified vendor to provide comprehensive peer health assistance services including evaluations, treatment referrals, monitoring, counseling, and educational programs for licensed professionals and applicants. Services must be available statewide across all geographic areas of Colorado on a 24/7 basis. The successful offeror must conduct biopsychosocial evaluations, maintain case management systems, provide monitoring agreements, report on compliance and program outcomes, and facilitate transitions between providers. The contract period is anticipated to begin February 1, 2027, with a transition period from December 1, 2026 through January 31, 2027 for any new vendor. The contract will span approximately 5 years, covering fiscal periods from February 1, 2027 through January 31, 2032.
Funding for the peer health assistance program is generated through licensure and renewal fees; the projected peer funds collected are $773,040 for FY26, $2,754,720 for FY27, $803,188 for FY28, $2,817,050 for FY29, and $831,047 for FY30, based on a current peer fee of $80 per two-year renewal cycle. The contract is contingent on the continued availability of these moneys, and the state reserves the right to terminate immediately if authorized collections cease. The Administering Entity may recover actual administrative costs not exceeding 10% of total collections. All unspent funds must be returned to the Administering Entity at the end of the contract period. Offerors must submit detailed budgets broken down by fiscal year with estimated personnel costs, evaluator fees, fringe benefits, and cost-per-recipient projections for each service category. The offeror must also demonstrate financial strength through audited financial statements from the last three years and maintain appropriate insurance coverage including professional liability coverage as specified in the Model Contract.
