About this file
This is a Diversity Practices Questionnaire from the New York State Office of Contractor and Supplier Diversity, applicable to the NYS Screen Academy Curriculum RFP. The questionnaire is a mandatory submission document that requires respondents to provide detailed information regarding their company's diversity initiatives and minority- and women-owned business enterprise (MWBE) engagement practices. The document does not specify project scope, quantities, response dates, or contract terms, as it functions as an evaluation instrument rather than a project specification document.
The questionnaire assesses eight primary areas of diversity practices: the presence of a Chief Diversity Officer or equivalent diversity leadership, percentage of gross revenues directed to New York State certified MWBEs, percentage of overhead and non-contract expenses allocated to certified MWBEs, technical training provided to MWBEs, participation in government-approved MWBE mentor-protégé programs, quantitative utilization goals for MWBEs in non-government procurements, formal supplier diversity programming, and planned subcontracting agreements with certified MWBEs. All responses are subject to audit and potential criminal prosecution for fraudulent statements. The related NYS Screen Academy Curriculum RFP indicates MWBE contracting goals of 0.00% and SDVOB goals of 0.00%, suggesting this particular procurement contains no specific diversity set-asides or goals, though the questionnaire remains a required submission component.
