About this file

This is a Diversity Practices Questionnaire issued by the Office of Contractor and Supplier Diversity for New York State in connection with an Asset Valuation and Sponsorship Consultant RFP from the Erie Canal Harbor Development Corporation (ECHDC), a subsidiary of Empire State Development. The questionnaire is a mandatory submission requirement for respondents seeking to provide sponsorship consulting services to develop and secure long-term Naming Right Agreements for state-owned facilities, including Terminal B event space, the proposed Cruise Terminal at Buffalo Outer Harbor, and the Niagara Falls Convention Center. The questionnaire must be completed and notarized by an authorized company official and submitted as part of the proposal response.

The questionnaire evaluates a respondent's commitment to supplier diversity through eight key questions addressing the presence of diversity officers, percentage of revenues paid to New York State certified minority and women-owned business enterprises (MWBEs), participation in technical training programs, involvement in government mentor-protégé programs, quantitative goals for MWBE utilization, formal supplier diversity programs, and plans to subcontract with certified MWBEs. Respondents must provide detailed documentation supporting affirmative answers, including names of participating MWBEs, training program specifics, goal percentages achieved, and completion of a Utilization Plan if applicable. All responses are subject to audit and fraudulent statements may result in criminal prosecution and debarment.
