About this file

This is a Cost Submittal template (Appendix B) for the Pennsylvania Insurance Department, Office of Liquidations, Rehabilitations and Special Funds, for legal services related to the Underground Storage Tank Indemnification Fund (USTIF). The Commonwealth is seeking a limited multiple award to the highest evaluated law firms to provide coverage opinions, legal support, and representation of USTIF to defend claims brought directly against USTIF, the Tank Installers Indemnification Program (TIIP), or the USTIF Board. Services include representation of USTIF and TIIP interests in claims resolution, including participant coverage claims, third-party property damage and bodily injury claims, and subrogation matters. The engagement may include additional general work designated by the Chief Counsel of the Pennsylvania Insurance Department or the Governor's Office of General Counsel. The contract term includes potential renewal options for Year 2 and Year 3 at the Commonwealth's discretion under the same terms and conditions.

Cost proposals must be submitted on the provided Cost Matrix using fixed hourly rates for each position, with evaluation based on 1,000 estimated hours per year distributed among proposed staffing positions. Offerors must complete all required tabs: the Cost Overview with firm contact information matching vendor registration, the Cost Matrix with position rates for Years 1-3, Optional Services—Frequently Used Billable Items for informational purposes only, and an Alternative Pricing Proposal tab for non-evaluated alternate pricing. All costs must be inclusive of overhead, travel, subsistence, supplies, and other expenses. The cost submittal must be bound and sealed separately from the technical proposal and clearly labeled "Cost Submittal." Actual hours required may vary and are not guaranteed, with payment based on labor hours provided at specified fixed hourly rates. Offerors must verify all calculations and subtotals for accuracy.
