USFS Project No. AG‐8371‐S‐15‐0003 City of Prescott Permit No. B1412‐119

April 8, 2015

TO ALL PROSPECTIVE OFFERORS RESPONDING TO INVITATION FOR BID FOR CONSTRUCTION OF PRC APRON RECONSTRUCTION (BASE BID – SCHEDULE I) AND PRESCOTT AIR TANKER BASE IMPROVEMENTS (ALTERNATE BID – SCHEDULE II) AT THE PRESCOTT MUNICIPAL AIRPORT, as outlined in the aforementioned Invitation of Bids published in Prescott Courier and Arizona Republic on March 15 and March 22, 2015; and available via the internet at www.a‐erepro.com; and available on FBO.gov.

I. General

1. The Bidders are made aware that they must acknowledge receipt of all Addendums (Amendments), at the time of bid, within Division I and Division II contract documents, (i.e. there are two locations to acknowledge receipt of all addendums). Amendments and Addendums are one in the same, and three have been issued to date. The federal Form SF30 for this Addendum (Amendment) No. 3 is attached.

II. Contractor’s Questions

The following questions were submitted in writing to Jeff Low (City of Prescott) before April 6th, as required in the contract documents:

1. Question: For the Past Performance element of Section M (Evaluation Factors for Award), the contract documents do not indicate how many Past Performance Questionnaires (PPQs) the Government would like sent out for completion. Please advise how many PPQs the Government would like in response to this factor. Can CCASS evaluations be provided as additional demonstration of experience and performance? Would you like a ‘fly sheet’ in the bound document indicating how PPQ requests for sent out to?

Response: Each contractor will be evaluated using the requirements listed in Section M. The following is taken from Past Performance portion of Section M:

“To help aid in the evaluation of past performance, the offeror must demonstrate performance of similar type, scope, and magnitude to that detailed in this RFP in the past three years. Documentation shall include contract numbers, current point of contact, telephone number, e‐mail address if available, fax number, description and value of the work. Offeror is responsible to make their best attempt to assure that past performance information is current so verification can occur. Information containing contact names no longer at the location provided, disconnected or changed phone numbers, or any other

PRC Apron Reconstruction (Base Bid – Schedule I) Addendum (Amendment) No. 3 4.8.15 Page 2 inaccurate information that hinders verifying past performance information runs the risk of the information not being considered.”

No specific numbers of required Past Performance Questionnaires are listed as it is up to the contractor to provide sufficient experience to demonstrate the criteria in Section M. Supplemental documentation is allowed to be included for the technical evaluation.

2. Question: Can previously completed Past Performance Questionnaires (PPQ) be submitted in lieu of generating new ones for this proposal provided that they show demonstration of experience? If so, in lieu of using the proposal PPQ form, how would you like those referenced?

Response: Past Performance Questionnaires are to be submitted directly to the CO listed on the form and are specific to the Prescott Air Tanker Base Improvements. Substitutions are not recommended and run the risk of incomplete information necessary to cover the intent of the questionnaire.

3. Question: What is the format for which the Government would like the bidder to demonstrate performance of similar type, scope and magnitude in the past three years? Is this a document generated uniquely by the bidder? How many projects would the Government like to demonstrate experience, as no quantity is indicated.

Response: No specific formats or quantity of projects are listed in Section M. It is up to the contractor to provide the following information as stated in Section M:

“To help aid in the evaluation of past performance, the offeror must demonstrate performance of similar type, scope, and magnitude to that detailed in this RFP in the past three years. Documentation shall include contract numbers, current point of contact, telephone number, e‐mail address if available, fax number, description and value of the work. Offeror is responsible to make their best attempt to assure that past performance information is current so verification can occur. Information containing contact names no longer at the location provided, disconnected or changed phone numbers, or any other inaccurate information that hinders verifying past performance information runs the risk of the information not being considered.”

It is up to the contractor to provide sufficient experience to demonstrate the criteria in Section M.

4. Question: Is the proposal to the Forest Service solicitation, AG‐8371‐S‐15‐0003 a separate, deliverable document, or is it to be incorporated into the bound Contract Documents for the City of Prescott? If so, where should the documents be placed in the bound document? If separate, what is the document / binder format and number copies required? Should there be separate dividers for the:

AGAR 452.209.70 Signed and dated (contained in Section K)

Bonds

PRC Apron Reconstruction (Base Bid – Schedule I) Addendum (Amendment) No. 3 4.8.15 Page 3

Response: The USFS solicitation is contained within the City of Prescott Document. Documents can be Captured within Division II of the bound document.

5. Question: In Block 10 of the Standard Form 1442, the directions indicated to ‘include the DUNs number on responses in block 17a.’ Block 17a on Page 2 of the SF1442 refers to the offeror agreeing to hold their price for a specified period of time. Is this the block that is intended to show the DUNs number, or would you like it indicated in Block 14 with the name and address of the offeror?

Response: Yes block 14 is acceptable to capture the DUNS number.

6. Question: One of the requirements of the United States Forest Service proposal is that the bidder must complete, sign, and submit a Buy American Certificate as part of the proposal submission. We are aware of FAR 52.225‐9 (Buy American – Construction Materials (MAY 2014)) as part of Page 23 of 49 of the solicitation, however there does not appear to be a specific form to complete and sign for the Certificate.

Can you please advise where the specific Buy American Certificate is that needs to be completed and signed?

Response: A Buy American Act certificate is not required to be completed. If Items are going to be procured from outside the US the process is listed in FAR 52.225‐9 Buy American‐ Construction Materials (page 23 of 49) and FAR 52.225‐10 Notice of Buy American Requirement (page 44 of 49).

7. Question: For the City of Prescott Prequalification Form, one of the requirements is to provide the major construction projects the organization has performed in the last 3 years. Please provide what the minimum dollar threshold for major construction projects is for this response. Is there a minimum and/or maximum number of projects to include, and what is the format that you would like for the response (i.e., a table?)

Response: The City is looking for projects of similar size to the job you are currently bidding on and would request references and contact information of up to 10 similar jobs during the last 5 years.
