ATTACHMENT TO STANDARD FORM 30, (BLOCK 14 CONTINUED)

This amendment publicizes the following questions and responses submitted by the five Contractors who attended the pre-bid meeting for the Annie Creek Snow Groomer Shed Construction project.

Pre-bid conference location:

Klamath Ranger District Office 2819 Dahlia St.

Klamath Falls, OR 97601

The pre-bid meeting began at 1:00 at the address listed above. The following five Contractor attended the meeting:

1. Shane Bratcher

2. David Walecli

3. Rob Ledgerwood

4. Steve Belden

5. Cory Spires

Introduction:

Waiyen "Yogi" Yee began the meeting by providing a general recap of the project. The following information was shared: The Forest Service has all the building components. The site build location is at Annie Creek Snow Park. It’s about 40 miles from the address listed above. The contract requires putting in concrete footings. Some earth work is needed. Footing will likely require some light digging. The borrow site is about 100 feet from the site build location. From the office location we headed to the storage site. We arrived at approximately 12:45. After this site we went to the site build location. We arrived there at 2:35.

During the pre-bid meeting, Yogi stated the following. “Put archer bolts into your bid, as they are not likely provided”.

The following questions were asked and the Government responses are shown below.

Question 1 How many trucks did it take to bring the material to the site?

Response: One, or two I think.

Question 2 Do we need a loading device?

Response: Yes, the components are separated at the storage site.

Question 3 Is this on Federal Land?

Response: Yes, this is on Federal land and no permits are required.

Question 4 Where are the clips for the roof?

Response: They should be in the box, Yogi said, as he showed the closed box, which was located at the storage site.

Question 5 Is there power?

Response: Power can be obtained by digging a trench to the site. Electricity is also available for use as needed.

Question 6 Are you doing the compaction?

Response: Yes, said Yogi. We will test compaction if we chose to. Finish grade will be about 4 inches above parking lot edge.

Question 7 Do we need traffic control?

Response: No

Question 8 Is there any endangered plants, or animals?

Response: No, during soil disturbance an archeologist will be there to monitor work.

At the site build location Waiyen "Yogi" Yee noted the following:

The approach to the shed is going to be gravel.

A concrete approach may be added later.

One big stump is located on the install location of the shed.

I will set the grade and elevation at the center of the building.

The snow should be gone in about three weeks.
