DESCRIPTION/ SPECIFICATIONS/ STATEMENT OF WORK EAST BRANCH
WEISER RIVER AOP 2016
C.1 SCOPE OF CONTRACT
Work consists of removal of existing culverts and replacement with AOP structures at two locations, NFSR#557 at MP 0.01 and NFSR#074 at MP 6.5. Replacement at NFSR#557 will be a 16’W x 5’3”R x 50’L open bottom structural plate arch. Replacement at NFSR#074 will be 18’W x 8’H x 24’L precast concrete box culvert with a 15 degree skew.
Work includes surveying and staking, dewatering, removal of existing metal culverts, excavation and embankment, placing riprap, installing structural plate arch with footings, developing streambed, placing crushed aggregate, and reinstalling salvaged gate. Project will also include the design, fabrication, transport and installation of precast box culvert.
GENERAL ITEMS COMMON TO ALL AOP’s:
1) Submit an erosion control plan that will show the stream diversion plans.
2) Remove and dispose of existing pipes.
3) Set and maintain survey control staking by referencing Government supplied control points.
4) Place stream simulation rock salvaged from excavation to construct a natural stream channel.
5) Backfill new structures.
6) Salvage rock for riprap on the fill slopes.
7) Haul any excess material to the disposal sites shown on the plans.
8) Haul and place government supplied crushed aggregate.
9) Scatter slash, mulch and spread government supplied seed on disturbed areas.
10) Supply and place Type III traffic barriers and ROAD CLOSED AHEAD signs.
C.2 PROJECT LOCATION
The project sites are located in the New Meadows Ranger District near the Price Valley Guard Station. Head west from New Meadows on Highway 95 to Price Valley Road/NFSR#074. Turn north onto NFSR#074, through Price Valley, for approximately 6.5 miles to reach site 074-MP 6.5. To reach the second site head north on NFSR#101 from the junction with NFSR#074 for approximately two miles to the junction of NFSR#557 and NFSR#101. The site 557-MP 0.01 is located near the intersection of the NFSR#101 and NFSR#557.
See map in project plans.
C.3 DESCRIPTION OF WORK
Installation of the open bottom structures shall be installed in the dry. Creating dry conditions will require capturing the flow of the East Branch Weiser River upstream of the project and diverting the flow past the work area in a manner which protects the natural flow from any additional turbidity. Seepage flow will be captured in a sump and pumped away from any flowing water to allow for infiltration so no muddy water reaches a flowing channel. The streambed simulation material will be placed as directed by the COR to approximate the drawings.
After placement the simulation material will be “washed in” utilizing the onsite dewatering pumps. The inlet and outlet of the structures will be protected by installing rip rap. Borrow material and surface aggregate should be obtained from Yantis Ditch Pit. Any material not available on site or at Yantis Ditch Pit shall be obtained for a commercial source. The culvert structure must meet HL-93 loading.
The material consists of unclassified Borrow and approximately 90 cy crushed road surfacing. Contractor must verify quantity.

Follow the requirements of FAR Clause 52.246-12 Inspection of Construction.
References to standard test methods of AASHTO, ASTM, GSA, and other recognized standard authorities refer to the methods in effect on the date of solicitation for bids.
Perform all work to the lines, grades, cross-sections, dimensions, and processes or material requirements shown on the plans or specified in the contract.
Incorporate manufactured materials into the work according to the manufacturer’s recommendations or to these specifications, whichever is more stringent.
Plan dimensions and contract specification values are the values to be strived for and complied with as the design values from which any deviations are allowed. Perform work and provide material that is uniform in character and reasonably close to the prescribed value or within the specified tolerance range. The purpose of a tolerance range is to accommodate occasional minor variations from the median zone that are unavoidable for practical reasons.
When standard manufactured items are specified (such as fence, wire, plates, rolled shapes, pipe conduits, etc., that are identified by gauge, unit mass, section dimensions, etc.), the identification will be considered to be nominal masses or dimensions. Unless specific contract tolerances are noted, established manufacturing tolerances will be accepted.
The Government may inspect, sample, or test all work at any time before final acceptance of the project. When the Government tests work, copies of test reports are furnished to the Contractor upon request. Government tests may or may not be performed at the work site. If Contractor testing and inspection is verified by the Government, the Contractor’s results may be used by the Government to evaluate work for acceptance. Do not rely on the availability of Government test results for process control.
Acceptable work conforming to the contract will be paid for at the contract unit bid price. Four methods of determining conformity and accepting work are described in Subsections 106.02 to 106.05 inclusive. The primary method of acceptance is specified in each Section of work. However, work may be rejected at any time it is found by any of the methods not to comply with the contract.
Remove and replace work that does not conform to the contract, or to prevailing industry standards where no specific contract requirements are noted, at no cost to the Government.
(a) Disputing Government test results. If the accuracy of Government test results is disputed, promptly inform the CO. If the dispute is unresolved after reasonable steps are taken to resolve the dispute, further evaluation may be obtained by written request.
Include a narrative describing the dispute and a proposed resolution protocol that addresses the following:
(1) Sampling method;
(2) Number of samples;
(3) Sample transport;
(4) Test procedures;
AG-0261-S-16-0044 Attach 1 Page 16 of 52
(5) Testing laboratories;
(6) Reporting;
(7) Estimated time and costs; and
(8) Validation process.
If the evaluation requires additional sampling or testing be performed, mutually agree with the Government on witnessing procedures and on sampling and testing by a third party laboratory. Use a third party laboratory accredited by the AASHTO accreditation program. Provide proof of the laboratory’s accreditation for the test procedures to be used. Do not use the same laboratory that produced the disputed Government test results or that produced the test results used as a basis for the dispute.
The CO will review the proposed resolution protocol and may modify it before final approval and execution.
The Government will use the approved resolution protocol test results to determine the validity of the disputed testing. If the Government test results are validated, the Contractor will be responsible for all costs associated with developing and performing the resolution protocol. If the Government test results are not validated, the Government will be responsible for all costs associated with developing and performing the resolution protocol. If the validity of the Government test results cannot be determined, the Contractor and Government will equally share all costs associated with developing and carrying out the resolution protocol.
(b) Alternatives to removing and replacing non-conforming work. As an alternative to removal and replacement, the Contractor may submit a written request to:
(1) Have the work accepted at a reduced price; or
(2) Be given permission to perform corrective measures to bring the work into conformity.
The request must contain supporting rationale and documentation. Include references or data justifying the proposal based on an evaluation of test results, effect on service life, value of material or work, quality, aesthetics, and other tangible engineering basis. The CO will determine disposition of the nonconforming work.

The Contractor is solely responsible for all spills or leaks that occur during the performance of this contract.
Clean up spills or leaks to the satisfaction of the CO and in a manner that complies with Federal, state, and local laws and regulations.
Design and locate equipment repair shops, stationary refueling sites, or other facilities to minimize the potential and impacts of hazardous material spills on Government land.
Before beginning any work, submit a Hazardous Spill Plan. List actions to be taken in the event of a spill.
Incorporate preventive measures to be taken, such as the location of mobile refueling facilities, storage and handling of hazardous materials, and similar information.
Describe how the contractor will comply with the following three methods to minimize fuel/oil leakage from construction equipment into the stream channel. Include the following:
i. Clean all equipment used for in-stream work so that it is free of external oil, grease, dirt and mud, and repair leaks, prior to arriving at the project site. All equipment will be inspected by the Contracting Officer’s Representative before unloading at site. Correct any leaks or accumulations of grease before entering streams or areas that drain directly to streams or wetlands.
ii. Fuel and service equipment used for in-stream or riparian work (including chainsaws and other hand power tools) in an established staging area (site-specifically designated by CO). When not in use, store vehicles in the designated staging area. The staging area should be in an area that will not deliver fuel, oil, etc., to streams.
iii. Oil-absorbing floating booms, and other equipment such as pads and absorbent “peanuts” appropriate for the size of the stream, shall be available on site during all phases of construction. For very small streams with few pools or slack water, booms may not be effective. More pads and straw bales to anchor booms may be necessary. Place booms in a location that facilitates an immediate response to potential petroleum leakage.
Immediately notify the CO of all hazardous material spills. Provide a written narrative report no later than 24 hours after the initial report, and include the following:

Section 151. - Mobilization
151.01_0114_R04F12_3_6_2012
Description
151.01 Add the following:
This work includes washing and treating construction equipment and vehicles necessary for equipment transport to remove seeds, plants, and plant fragments before the equipment is used on Forest Service lands.
Wash the sides, tops, and undercarriages of all construction equipment with a high-pressure washing system.
Perform all work at a location designated on the plans or other locations approved in writing. Provide the CO with an opportunity to monitor the washing and inspection.
Remove all seeds, plants, plant fragments, dirt, and debris from the construction equipment. Only equipment cleaned and inspected will be allowed to operate within the project area. All subsequent move-ins of equipment to the project area will be treated in the same manner as the initial move-in. This requirement does not apply to cars, pickup trucks, dump trucks, and other vehicles that regularly travel between the construction site and areas off the National Forest.
Equipment will be considered free of soil, seed, and other such debris when a visual inspection does not disclose such material. Disassembly of equipment or components, or the need for specialized inspection tools, is not required.
Notify the CO in writing at least 7 days before moving any construction equipment onto the National Forest, and at least 24 hours before changing locations within the project area. Include an agreed-upon location where the equipment will be available for inspection. Inspection is required after every cleaning.
Use methods of cleaning and locations for cleaning approved by the C.O.
For work on National Forest System lands, use a washing system that traps all wash water and either stores it for removal from National Forest System lands or recycles the water for continued use. If the equipment recycles the water, provide adequate filters for seed removal. Dispose of the filter material and removed seeds offsite in a legal and approved manner. Do not mix soaps, detergents, or other chemicals with the wash water.
For work at a commercial washing facility, use an approved facility.
New infestations of noxious weeds of concern to Forest Service and identified by either Contractor or Forest Service, on the Project Area or on the haul route, shall be promptly reported to the other party. Contractor and Forest Service will agree on treatment methods to reduce or stop the spread of noxious weeds when new infestations are found. 157.03 General
Delete the first two paragraphs and replace with the following:
Submit an Erosion Control Plan for each site, detailing permanent and temporary erosion-control measures to minimize erosion and sedimentation during and after construction, in accordance with the plans and storm-water permits. Do not modify the type, size, or location of any control or practice without approval. Submit the erosion-control plan proposal at least 7 days before operations begin to the CO for approval.
Reflect in the Erosion Control Plan special concerns and measures necessary to protect resources and government improvements. Include:
1. The construction activities and sequence of implementation relating to specific erosion control measures.
2. The location and type of permanent controls to be implemented during construction.
3. The location and type of temporary controls to be implemented during construction.
157.03_R04F12_03_27_2012
4. For work in stream channels with running water, submit a detailed dewatering plan. Convey water around the construction area during the operating season and higher flows in the event of a storm. Notify the COR at least 10 days in advance of any stream work or dewatering. The COR will notify the District Fish biologist who will be available during dewatering to move fish from the dewatering area using a combination of dip-netting and electrofishing. Schedule the start of dewatering to occur in the mid-morning, so the fish crew will have the afternoon to remove the fish. After the COR approves the stream simulation placement, re-water the area slowly to prevent loss of surface water downstream as the construction-site streambed absorbs water and to minimize a sudden increase in turbidity.
157.03_0510_11_10_2004
In perennial channels, which includes the two sites on this contract, pre-wash the reconstructed stream channel into a reach equipped with sediment-capture devices such as Sedimat, or settling pond prior to reintroduction of flow to the stream. Remove the turbid water by pumping to an infiltration area, or letting the sediment settle in the settling pond.
Re-water the construction site slowly to prevent loss of surface water downstream as the construction-site streambed absorbs water and to minimize a sudden increase in turbidity.
Use in-channel sediment-abatement barriers such as Sedimat to capture sediment that is liberated during re-watering of dewatered channels. Clean out and remove barriers, and dispose of captured sediment so it is not reintroduced into stream channels. Maintain such barriers throughout the related construction and remove only when construction is complete and erosion control is assured.

5. The location of each temporary stockpile site for the excavated material, and location of erosion control measures to prevent sediment from washing off of stockpile.
6. Keep a supply of erosion-control materials (e.g. silt fence, straw bales, and/or straw wattles) on hand to respond to sediment emergencies. Use only sterile straw or certified “weed-free” straw to prevent introduction of noxious weeds.
157.03_R04F12_03_07_2012
Add to the third paragraph:
Upon completion of construction at the site, remove all temporary dewatering materials and equipment from Government property. This includes items such as pumps, pipes and hoses, and sandbags.
Add the following:
Adhere to the following practices:
a) In order to protect aquatic organisms, insure that drafting hoses are fitted with screens with a 3/32 inch mesh and the appropriate surface area for the pump to achieve a mazimum water velocity of 0.4 ft/ sec at the screen surface, consistent with NOAA guidelines.
b) Centrifugal pumps used for drafting water from live streams or removing water from structure excavation (if needed) shall sit in a secondary spill container with sufficient capacity to contain the quantity of fuel in the pump fuel tank.
c) Construct the Dewatering and Sediment Control according to the Contractor’s approved plan.
d) Maintain the roads and stockpiles of material in a manner that prevents direct entry of sediment into the stream, by either preventing the erosion of sediments or collecting the sediments before it reaches the stream.
e) Construct permanent and temporary control features to intercept sediments. A settling basin will be constructed, or a portable one used, that is able to handle the volume of water anticipated, and that provides filtering capability. Dewatering of the excavation site, (of ground water from the excavation) will be done by pumping water to the settling basin, and the basin shall be cleaned out when full of sediments.
f) Any sanitation facilities established by the Contractor shall be located as designated by the CO.
g) Cease all project operations, except efforts to minimize storm or high-flow erosion, under precipitation and high-flow conditions that result in uncontrollable erosion in the construction area.
157.04 Controls and Limitations on Work.
Add the following to the second paragraph:
Place temporary erosion-control measures at the end of each day’s operation if greater than 30 percent chance of rainfall is predicted. Earthwork operations may be temporarily suspended by the CO if significant rainfall is forecast.
157.08 Water Crossings.
Add the following:
When there is water flowing through a channel crossing, dewater that site by rerouting the flow around the site before and during excavation and embankment operations that have the potential for sediment delivery to the live stream. If pumps are used, install screens on pump intakes with maximum openings of 3/32 inch and the appropriate surface area for the pump to achieve a maximum water velocity of 0.4 ft/sec at the screen surface, AG-0261-S-16-0044 Attach 1 Page 31 of 52 consistent with NOAA guidelines to protect aquatic organisms.

157.13 Maintenance and cleanup
Delete the third paragraph and insert the following:
Remove any Sedimat used from the stream channel after sediment-producing activities have ended. Place Sedimat on stream bank as an erosion-control measure. Erosion-control materials, such as hay bales, silt fences, and straw wattles, will be removed by the Forest Service when the sites have revegetated.

203.01_R04F12_06_11_2012
203.01 Description.
Add the following:
Dispose of the existing culverts by method (a) remove from project.
The existing corrugated metal pipe culvert and related appurtenances shall be removed and disposed of away from Forest land according to all applicable local, state and federal regulations and 203.05 (a). Temporary soil erosion control measures shall be in place prior to commencing.
203.01_nat_us_2_18_2005
203.05 Disposing of Material.
Add the following:
(e) Windrowing Construction Slash. Place construction slash outside the roadway in neat, compacted windrows approximately parallel to and along the toe line of embankment slopes. Do not permit the top of the windrows to extend above subgrade. Use construction equipment to matt down all material in a windrow to form a compact and uniform pile. Construct breaks in windrows of at least 15 feet at least every 200 feet and at every draw and ridge and for wildlife trails. Do not place windrows against trees. Obtain approval for pioneer roads. A pioneer road may be constructed to provide an area for placement of windrows, provided the excavated material is kept within the clearing limits and does not adversely affect the road construction.
(f) Scattering. Scatter construction slash outside the clearing limits without damaging trees. Limb all logs.
Place logs and stumps away from trees, positioned so they will not roll, and are not on top of one another. Limb and scatter other construction slash to reduce slash concentrations.
(g) Chipping or Grinding. Use an approved chipping machine to grind slash and stumps greater than 3 inches in diameter and longer than 3 feet. Deposit chips or ground woody material on embankment slopes or outside the roadway to a loose depth less than 6 inches. Minor amounts of chips or ground woody material may be permitted within the roadway if they are thoroughly mixed with soil and do not form a layer.
(h) Debris Mat. Use tree limbs, tops, cull logs, split stumps, wood chunks, and other debris to form a mat upon which construction equipment is operated. Place stumps upside down and blend stumps into the mat.
(i) Decking Firewood Material. Remove brush from decks. Limb and deck logs that do not meet Utilization Standards according to Subsection 201.04, as directed by the CO. Cut logs to lengths less than 30 feet. Ensure that logs stacks are stable and free of brush and soil.
