14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible.
Portions of the West Arm Cholmondeley LTF Floating Log Slide are underwater making a condition determination difficult. This amendment to remove the replacement of the High-density polyethylene (HDPE) floats from the Base items. The government may modify this contract to repair the floats once the condition known.
Change paragraph C.2(a)(1) to read: “West Arm Cholmondeley LTF: Raise, transport and place the Floating Log Slide system on the Thorne Bay LTF. Any repairs, including replacement of the High-density polyethylene (HDPE) floats, will be made after a government structural evaluation and through a contract modification to the successful Bidder. No HDPE floats are required in the Base Bid.”
Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

452.215-71 Instructions For The Preparation of Technical and Business Proposals (SEP 1999) (Amendment 3)
(a) General Instructions. Proposals submitted in response to this solicitation should include the following:
(1) 1 copy – Completed SF-1442 Solicitation, Offer and Award form, signed and dated by offeror.
(2) Response to any amendments that were issued.
(3) 1 copy – Schedule of Items (Section B)
(4) Information required from Section K Representations and Certifications and copy of Alaska
Native or Indian-Owned Business.
(5) Draft plan to accomplish the work. The plan must include:
(i) Estimated dates of performance to include:
(A) Start work
(B) Mobilization
(C) Start and completion of work items
(D) Demobilization
(E) Completion
(ii) Anticipated method to accomplish work
(iii) Anticipated subcontracts
(iv) Personnel and equipment to be used
(6) 1 copy – Experience and Capability Questionnaire (Section M) OR clearly written resume that provides the same information. Questionnaire or resume must address sources selection factors listed in Section M. Evaluation The Experience and Capability Questionnaire can be used as the format to answer some of the Past Performance requirement. Include the names of project managers and projects of similar scope they’ve managed.

SECTION M. EVALUATION FACTORS FOR AWARD (Amendment 3)
Evaluation for Award: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Ensure offers address the following evaluation factors (also see Section L, 452.215-71 Instructions for the Preparation of Technical and Business Proposals):
(b) Draft plan to accomplish work, sub-factors are:
(1) Proposed schedule of completion
(2) Method to accomplish work
(c) Sufficient capacity to perform contract work, sub-factors are:
(1) List of equipment available to complete work. Identify equipment contractor currently possess and equipment contractor plans to lease/rent to accomplish this work.
(2) Personnel by position to perform this work, and years of experience.
(d) Construction experience of project managers performing similar projects. Submit names of project managers and projects of similar scope they’ve managed.
(e) Past Performance: The offeror shall provide specific information on recent relevant contracts which support past and present performance. Offerors without past performance history will not be evaluated favorably or unfavorably on past performance. Past performance evaluation will include:
(1) History of favorable and cost effective business relationships.
(2) History of cooperative and non-adversarial project site relationships.
(f) All evaluation factors other than price, when combined, are more important than price.
Accordingly, award may be made to an offeror other than the lowest priced offeror, if that higher-priced offeror provides the best value.
