About this file
This document is an Appendix D - Required Disclosures for a state and local contract opportunity issued by the Pennsylvania Historic & Museum Commission (PHMC) to pre-qualify contractors, craftspeople, and artisans for preservation construction projects on historic buildings at PHMC's sites and museums throughout Pennsylvania. The opportunity is open for enrollment until June 30, 2019 and covers the jurisdictions of Dauphin County and Harrisburg City. The Appendix outlines 12 disclosure questions the applicant must answer related to their business history, including any debarments, suspensions, licenses revocations, liens, legal violations, and bankruptcies within the past 3 years.
The document does not provide details on pricing terms, set asides, incumbent contractors, funding requirements or sources, or contract duration and renewal options. It is focused solely on the required disclosures the applicant must submit as part of the pre-qualification process for this state and local preservation construction contract opportunity.
