About this file

This document is an Affirmation and Disclosure Form from ThorWorks Industries / SealMaster Inc., located in Sandusky, Ohio, for a contract with the State of Ohio, specifically the Ohio Department of Transportation (ODOT). The form addresses compliance with Executive Order 2022-02D, which prohibits the purchase of services from Russian companies or institutions, and affirms that all services will be performed within the United States. The contractor certifies that its business operations, service locations, and state data storage will be conducted exclusively at 2520 S. Campbell St. in Sandusky, Ohio, with no subcontractors involved.

The document does not specify the exact goods or services under contract, but it appears to be related to roadway sealants and equipment, given the signatory's title of "Director Roadway Sealants & Equipment." The form is signed by Robert Shutt on 2/17/2025, suggesting a contract effective date or renewal around that time. The primary focus is on ensuring compliance with state regulations regarding service locations and prohibiting business interactions with Russian entities, rather than detailing specific contract terms, pricing, or service scope.
