About this file
This document is an Affirmation and Disclosure Form related to a state contract, specifically involving the Ohio Department of Transportation (ODOT). The form is executed by Kenston Henderson, the CEO of a company located at 3637 Quickwater Rd in Grove City, Ohio. The document affirms compliance with Executive Order 2022-02D, which prohibits state agencies from purchasing services from Russian companies or institutions, and extends previous restrictions on offshore service procurement.
The form does not provide specific details about the goods or services under contract, but it does outline strict requirements for service delivery and data management. The contractor affirms that no services will be performed outside the United States, and state data will not be sent, accessed, or stored remotely outside the United States without a signed waiver. The primary subcontractor listed is also Kenston Henderson, with a location in Columbus, Ohio. The document was signed on March 11, 2024, and requires immediate notification to the state of any changes in service location, with the potential for contract termination if services are moved outside the United States without approval.
