About this file

This document is an Affirmation and Disclosure Form related to a state contract, specifically involving the Ohio Department of Transportation (ODOT) and a contractor located at 5155 Financial Way, Suite 13 in Mason, OH 45040. The form addresses compliance with Executive Order 2022-02D, which prohibits state agencies from purchasing services from Russian companies or institutions, expanding upon a previous executive order that generally precludes offshore services.

The form indicates that the contractor affirms no services will be performed outside the United States, and no state data will be sent, accessed, or stored remotely outside the country without a signed waiver from the state. No specific goods or services are detailed in this document, which appears to be a standard compliance form ensuring the contractor's commitment to performing work domestically and avoiding engagement with Russian entities. The document is signed by Ryan Halbert, Territory Sales Manager, on 2.13.24, and serves as a mandatory disclosure and affirmation prior to contract execution.
