About this file
This is an Additional Terms and Conditions document for a State of Alabama Invitation to Bid (ITB) solicitation for preventative maintenance and repair services of one Formax FD2096 high-capacity pressure sealer system and associated components, including a V-Stack36i Vertical Stacker and V-Stack36-10 Adjustable Stand. The solicitation requires bidders to submit technical documentation supporting equipment specifications and pricing on a per-month basis through the Alabamabuys supplier portal. Questions regarding the ITB must be submitted by October 9, 2026, at 12:00 PM Central Time. The resulting contract will have an initial 24-month term with options to extend for three additional 12-month terms. The award will be made on an all-or-none basis to the lowest responsible bidder meeting all specifications.
Key requirements include that the bidder must be an authorized Formax maintenance service provider with a letter of confirmation from Formax, provide qualified and trained technicians, assume full responsibility for all equipment and services without outsourcing, furnish original equipment manufacturer warranties, provide references with years of service experience and average response times, and maintain all work and materials free from defects. Bidders must identify any non-OEM items and provide OEM authorization documentation as applicable. The awarded supplier must submit E-Verify documentation and the State of Alabama Vendor Disclosure Statement. Price escalation may be considered only if justified by documented increases in material and labor costs, limited to industry-specific or CPI increases, and subject to written approval by both parties. The State of Alabama pays thirty days in arrears following invoice receipt. Any de-escalation in pricing must be immediately passed to the state while maintaining all contract specifications and terms.
