About this file
Summary: Addendum Two for RFB-63-27
This is Addendum No. 2 to an Invitation to Bid issued by the Commonwealth of Kentucky Department for Facilities and Support Services, Division of Engineering and Contract Administration for the replacement and repair of the HVAC system in Cell House 4 at Kentucky State Penitentiary in Eddyville, Kentucky. The project involves installing new rooftop units (RTU-1), energy recovery ventilation (ERV-1), ductwork, structural steel, gas piping infrastructure, and associated controls systems. The project includes phased construction with floor space allocated for three weeks per side, background check processing with a two-week turnaround, and on-site training for two days. The substantial completion period has been extended to 365 days from the original contract issuance. The addendum incorporates 33 clarifications and responses to contractor questions addressing specification details, drawings, material storage, equipment installation procedures, testing and balancing coordination, structural scanning requirements, gas piping specifications (requiring 4" piping throughout with 800 feet total developed length), roof cutting and restoration procedures with an 18-month warranty, and special inspection coordination. Supporting documentation includes asbestos and lead paint inspection reports from May 2026 by Summit Environmental Services Inc. and structural reinforcement scanning reports from GPRS dated October 2, 2025.
Key technical clarifications specify that scanning is required at all fifteen penetration points to minimize reinforcing steel cuts, exhaust air outlet ERV-1 must be positioned at least 10 feet from the outside air intake, the building control system will operate as an independent system with remote access capability, and the contractor is responsible for all natural gas meter and tap fees. The contractor must coordinate with Commonwealth-provided testing and balancing contractors and special inspections contractors. Scaffolding may remain erected throughout associated project phases within the secured area, tools may be stored overnight with daily inventory checks, and hydro excavation is permitted as an alternative to boring. The roof assembly must be restored to match existing construction and achieve a weathertight, warrantable condition. No geotechnical report is available for the underground natural gas line route, and no list of pre-approved glazing contractors was provided. All bidders are required to acknowledge receipt of this addendum on their proposal forms.

Addendum Number Two
DOC – Kentucky State Penitentiary KSP Repair Replace HVAC System 4 Cell House
527-CC2W-SP50-00
BCCLT Project No. 25054
This addendum amends Drawings and/or Specifications and/or Addenda for the above title project, as indicated below, and is hereby incorporated into the Contract Documents as part thereof.
Bidders are required to acknowledge receipt of this Addendum in the space provided on the Proposal/Bid form.
Separate Attachments:
1. Summit Environmental Services Inc – Asbestos and Lead Paint Inspection
2. GPRS – Structural Reinforcement Scanning Report
Clarifications:
1. Refer to sheet M203.
a. Keynote #7 shall read: 32" x 30" exhaust duct up thru roof. Gooseneck termination above roof, refer to sheet M400 detail 4. Ensure that exhaust air outlet ERV-1 is at least 10’-0” away from the outside air intake.
Contractor Questions:
1. Sheet M012 states unsecured floor space will be granted for three (3) weeks per side.
At the 9/3 site visit, bidders were advised to carry two (2) weeks per side. Please confirm by Addendum which duration governs, and revise M012 accordingly.
Response: Contractor shall be allowed floor space for three (3) weeks per side.
Coordinate timing and phasing with facility.
2. What is the anticipated turnaround for background checks and badging of the construction workforce
Response: On site training will occur for two days. Background check turnaround is anticipated to be 2 weeks.
Addendum No. 2
3. Please issue the asbestos, lead-based paint, and hazardous materials survey report by Addendum. Absent a survey, bidders have no basis to quantify this work.
Response: See separately attached Asbestos and Lead Inspection Report from May 2026 by Summit Environmental Services, Inc.
4. Bidders were advised that the existing slab has been X-rayed and that rebar is marked on the floor. Why is there a schedule for S100 for the contractor to fill out.
Response: Not every location of the existing attic slab was scanned. Scanning was completed in selected representative areas of the attic slab and was completed prior to having an understanding of exactly where all of the penetrations would need to be.
As such, the documents require scanning to take place in order to adjust the opening locations a few inches in one direction or another in order to miss reinforcing steel and to cut as few bars as necessary (see sheet S203). For example, if the exact position shown cuts 6 bars in one direction and if that opening position were to shift 2” and only 5 bars would be cut, that is a good choice to make in our opinion.
It should also be noted that scanning is “approximate” and not entirely precise.
Varying conditions can be found once the concrete is cut. We are interested in knowing how many bars are cut in each direction in each location. Please provide the information requested during the course of the work.
5. Please issue the scan reports and marked-up plans by Addendum.
Response: Please see attached report by GPRS.
6. Given the existing scan data, is the Contractor still needed to perform independent concrete scanning under Specification 030502, paragraph 3.1.B, at all fifteen (15) penetrations (A–O)?
Response: Yes, please do scan each location in order to review the final position of the penetration to be cut. As discussed above and on sheet S203, it is helpful to cut the least amount of reinforcing bars as possible in each location.
7. Who is responsible for meter and tap fees on Natural Gas?
Response: Contractor shall be responsible for all fees.
8. Please provide detail for Concrete pad on M010 associated with detail 2, Phase 1A RTU-1 Install. The detail is not sufficient as it will require a large turndown due to the slope of the existing grade.
Response: See concrete slab turndown detail below.
Addendum No. 2
9. Sheet M102 states scaffolding is to be built, disassembled, and removed at the end of each day. Please confirm this is literal and confirm whether scaffolds may remain erected within the fenced "Secured Area" shown on M012. The scaffold will need to stay erected for that entire phase.
Response: Omit Photo 1 note indicating the scaffolding to be built, disassembled, and removed at the end of each day. Scaffolding may remain erected during the entire associated phase of the project. Scaffolding may remain erected within the fenced “Secured Area.”
10. Where may materials and tools be stored inside the secure perimeter overnight? If none, please confirm all material must be transported in and out daily.
Response: Tools may be stored within the secure perimeter. These tools will still require daily inventory checks.
11. Bidders were advised the controls are standalone with the ability to tie in. Please define the scope demarcation on Sheet M501 between the Temperature Control Contractor (BMS/DDC, Status Command Center Panel, Firefighter Override Panel, motorized dampers) and the Fire Alarm Contractor (duct smoke detectors, addressable control modules, remote test switches, general alarm interface).
Response: There is no existing building control system for the entire facility currently. The control system for Cell House 4 will be its own independent control system for the entire building. Prime contractor shall define scope demarcation among subcontractors.
12. Controls point list shows graphics and trends. How is this to be accomplished without tying into a controls system?
Response: Building controller shall be its own independent control system with capability for computer/laptop workstation integration. The control panel shall be located in the mechanical room with capability for remotely access through a remote data connection or through the internet. All components required shall be provided by the controls Contractor.
Addendum No. 2
13. Sheet M501 Note 1 states duct smoke detectors are furnished, installed, and wired by the Mechanical Contractor, while Sheet E200 Keynote 1 assigns duct detector installation and remote test switches to the electrical scope. Which governs, so this is carried once?
Response: Prime Contractor shall determine which discipline and sub-contractors shall provide the associated scope.
14. Can Reliable Controls be added to the list of approved Building Management System manufacturers?
Response: Yes, equivalent manufacturers are acceptable. Final equipment approval will be determined during shop drawing review.
15. New gooseneck terminations penetrate the existing roof (M400 Detail 4). Is there an existing roof warranty in force on 4 Cell House?
Response: No.
16. Special Conditions Article '4 states Testing and Balancing will be procured separately by the Owner, yet 230593 appears in the specification index and Sheet M010 requires the Contractor to balance RTU-1 to 13,500 CFM and ERV-1 to 7,300 CFM before proceeding to Phase 2. Please clarify what balancing is Contractor scope.
Response: Commonwealth of Kentucky will provide all testing and balancing via a separate contract. It is the responsibility of the contractor to coordinate and schedule the testing and balancing with the commonwealth provided testing and balancing contractor.
17. With the complexity of this project and equipment lead times can the period of performance of substantial completion be extended to 280 days?
Response: Project substantial completion shall be extended to a total of 365 days.
18. Can work hours be changed to 6:00 am to 4:30 pm Monday through Thursday?
19. Who provides Special Inspections on Structural Steel?
Response: Commonwealth of Kentucky will provide all special inspections on structural steel via a separate contract. It is the responsibility of the contractor to coordinate and schedule the inspections the commonwealth provided special inspections contractor.
20. Please provide details of Daily Tool Check process?
Response: All tools will be required to be inventoried. This includes tools being brought in/out of the facility daily as well as tools stored in the secure area overnight.
21. Will hydro excavation be allowed instead of boring?
Response: Yes.
Addendum No. 2
22. Is the GEO Technical report available for the route of the underground Natural Gas line?
23. Has the number of elbows in the gas line to RTU-1 been calculated in the sizing of the pipe from the meter?
Response: Refer to Gas flow diagram on P001 – all piping shall be 4”, do not provide 3”. Total Developed length shall be 800 feet in lieu of 600 feet. Refer to P100 Keynotes 4, 5, 7, and 8 – Replace all references to 3” gas piping with 4” gas piping. Do not provide 3” gas piping.
24. Could you provide a detail for concrete bollards for underground Natural Gas piping?
Response: See concrete bollard detail below. Provide 4” galvanized steel pipe filled with concrete and capped. Paint bollard safety yellow. Provide 24” deep concrete footing to secure bollard in ground.
25. How does the engineer of record intent to place the exhaust fan and ERV in the attic?
The door entering the attic as well as the roof access hatch are not large enough for these to be installed through. Please provide details of how the roof is to be cut and repaired.
Response: Contractor shall cut and modify the existing roof opening as required to crane equipment to the attic. Contractor shall maintain the opening in a weathertight, secured condition at all times while open, and shall protect the building interior, attic, and equipment from water intrusion and debris until roof is permanently restored. Once the equipment has been set in its final attic location, contractor shall restore the roof assembly to match existing construction and to a warrantable, weathertight condition. Contractor to provide an 18-month warranty.
Addendum No. 2
26. Will the areas underneath the attic where holes are cut be evacuated?
Response: Yes. Area below attic will be evacuated to allow contractor access. No inmates will be present on lowest level during construction. Inmates will be present on upper floors.
27. Is there an elevator the contractor could use to bring construction materials to the attic?
28. Can you provide a detail of support for the duct in photo 2 on page M012 once the bottom half of the duct is removed?
Response: Contractor to provide all temporary/permanent supports as required for duct. Refer to similar support examples in the structural drawings.
29. Is spray applied fireproofing required for structural steel?
Response: Yes. Cementitious fireproofing spray, 2 hour rated, shall be provided on all new structural steel supporting attic slab/reinforcement
30. Please provide a list of glazing contractors that have worked and are acceptable to this facility?

EQUIPMENT USED
The following equipment was used on this project:
• Concrete GPR Antenna: This GPR Antenna is handheld and rolls over the surface. The device displays scan data on a screen, and the operator marks detected objects on the surface in real-time. The antenna needs a reasonably smooth, unobstructed surface for scanning and cannot scan within 2”-4” of obstructions such as walls and metal tracks. Ideally, the client removes obstacles such as these before our work begins. The total effective scan depth can be as much as 18” or more with this antenna but can vary depending on the concrete conditions, composition, and other factors such as the spacing of the reinforcing. Depth accuracy depends on obtaining a precise depth calibration for the concrete. This device does not emit harmful radiation and can be safely operated while people are in close proximity. For more information, please visit: Link https://www.gp-radar.com/equipment/1600-2000-2300-2700-mhz-gpr-antenna
