About this file

This is Addendum No. 1 to Invitation to Bid No. RFP-54-27 issued by the Commonwealth of Kentucky Department for Facilities and Support Services, Division of Engineering and Contract Administration for the Kentucky Abandoned Storage Tank and Orphan Well Reclamation Program (KASTOW) Logan 001 project. The addendum, dated September 4, 2026, revises the plugging activities requirements by replacing previously issued specifications with revised attachment K requirements. The plugging work must be performed using a service rig with a work string of steel tubing run downhole; slickline plugging is not acceptable. The project encompasses wells in Western Kentucky counties including Ballard, Butler, Caldwell, Calloway, Carlisle, Christian, Crittenden, Daviess, Fulton, Graves, Hancock, Henderson, Hickman, Hopkins, Livingston, Logan, Lyon, Marshall, McCracken, McLean, Muhlenberg, Ohio, Todd, Trigg, Union, and Webster.

The revised plugging plan establishes differentiated procedures based on well depth. For wells with total depth at 1,500 feet or shallower, activities include coordination with the Division Inspector, access road construction, pit excavation and lining with 6-mil liner, brush removal, service rig setup, wellhead entry or dismantling, extraction of tubing and rods if present, TD establishment, tubing run with minimum five barrels of water or 6% gel circulation, casing perforation from 250 to 50 feet in 50-foot increments if uncemented or unknown, and Class A, Class L, or Type 1L cement circulation to surface, with 50/50 POZ or latex-laden cement required if H2S gas is present. For wells deeper than 1,500 feet, procedures include rod and tubing removal, free casing extraction, bond log consideration if cement records are unavailable, multi-stage cement plug placement at total depth and across perforated zones, casing perforation if required, and final cement placement from 600 feet to surface to protect fresh water and coal seams. All wells require casing cutoff three feet below surface the next day, pit fluid removal and proper disposal, pit backfill, and complete wellsite reclamation and stabilization.

WESTERN KY (Ballard, Butler, Caldwell, Calloway, Carlisle, Christian, Crittenden, Daviess, Fulton, Graves, Hancock, Henderson, Hickman, Hopkins, Livingston, Logan, Lyon, Marshall, McCracken, McLean, Muhlenberg, Ohio, Todd, Trigg, Union, Webster)

Wells with TD at 1500’ or shallower

• Confer with Division Inspector, assure landowner access is approved

• Construct access road to well

• Dig pit and line with 6 mil liner

• Remove brush from location, rig-up service rig

• Establish wellhead entry if not open casing, or dismantle wellhead if fully equipped

• If tubing & rods remain in well, extract same from the hole and trip out

• Trip in well with rig tools to establish TD and ensure no wellbore obstructions

• Run tubing to TD and pump minimum five barrels of water or 6% gel

• If casing is not cemented or unknown; perforate casing from 250’ to 50’; (50’ increments)

• Circulate Class A, Class L, or Type 1L cement solid to surface (Cement calculation based on hole volume). 50/50 POZ or latex laden cement shall be used if H2S gas is present and/or detected.

• Monitor cement level and top off as needed

• Cut off all casing 3 ft. below surface the next day to ensure no fall back has occurred

• Remove fluids from pit, properly dispose of same, and backfill pit

• Properly grade, stabilize, and fully reclaim wellsite as outlined in Scope of Work

Wells deeper than 1500’

• Confer with Division Inspector, assure landowner access is approved

• Construct access road to well

• Dig pit and line with 6 mil liner

• Remove brush from location, rig-up service rig

• Pull Rods & tubing, if present

• Any free casing (annulus not cemented) shall be removed. If no records are available to determine Top of Cement (TOC), a bond log may be required

• Trip in well with rig tools to establish TD and ensure no wellbore obstructions

• Run tubing, spot 200’ Class A, Class L, or Type 1L cement plug at TD. 50/50 POZ or latex laden cement shall be used if H2S gas is present and/or detected.

• Pull up above TOC, wash out, Tag Bottom Plug

• Pump spacer water or 6% gel

• Spot Class A or Class L cement plug 50’ below deepest perforation to 50’ above shallowest perforation (or production zones if open hole), across any and all perforated (or production zones in open hole) not covered by bottom plug. 50/50 POZ or latex laden cement shall be used if H2S gas is present and/or detected.

• Pump spacer water or 6% gel

• If casing is not cemented or unknown; perforate casing from 250’ to 50’; (50’ increments)

• Spot Class A, Class L, or Type 1L cement from 600’ to surface (to isolate and protect fresh water and coal seams).
