About this file

This document is a Business Proposal Attachment E for a state government procurement process, specifically for the Indiana state government. The document provides a comprehensive template for respondents to complete detailed information about their company, including sections for company structure, financial information, references, business registration, and diversity subcontractor agreements. The proposal requires respondents to provide extensive details about their organization, including legal form, financial stability, corporate integrity, reference contacts, and business experience serving state and similar governmental entities.

The attachment emphasizes several key requirements, including demonstrating financial stability through documents like Dunn & Bradstreet reports or audited financial statements, providing references from at least three clients with similar service offerings, and detailing engagement with Minority & Women's Business Enterprises (MBE/WBE) and Indiana Veteran Owned Small Business (IVOSB) subcontractors. Additional requirements include being registered to do business with the Indiana Secretary of State, providing authorizing documentation for contract commitments, and outlining the company's business continuity and data security processes. The document also seeks information about the respondent's experience serving state governments and their willingness to extend pricing to other governmental bodies.
