About this file
This is a Responsible Contractor/Subcontractor Certification Form Packet issued by Montgomery County, Pennsylvania, for use in the 2026 Park Paving Program, which involves asphalt paving at five park, trail, and historic sites locations throughout the county. The project scope includes subbase preparation, binder course, wearing course, asphalt seal coat, pavement markings, topsoil backup, and permanent restoration at parking lots, access roadways, and pathways. Contractors and subcontractors must complete the certification cover sheet and checklist prior to contract award, providing documentation of compliance within five calendar days upon request. The form requires contractors to certify their responsible contractor status by indicating whether they meet all, none, or certain responsible contractor requirements, with detailed explanations required for partial compliance.
The certification checklist establishes comprehensive requirements applicable to all contractors and subcontractors, including valid licenses and credentials, bonding and insurance compliance, verification of no debarment or suspension within three years, no project defaults within three years, no license revocations or suspensions within three years, no criminal convictions relating to contracting business within ten years, compliance with applicable wage and fringe benefit rates, completion of OSHA 10-hour training for all craft employees, and participation in bona fide apprenticeship programs registered with the U.S. Department of Labor. For prime contractors and construction managers, the requirements include ensuring that at least seventy percent of craft labor workers are journeyperson workers or registered apprentices in the same trade, and that at least one project person completes OSHA 30-hour training. Subcontracting firms must submit subcontractor lists within seven days of notice of intent to award, provide subcontractor responsibility certifications for all listed entities, and comply with restrictions on sole proprietorship entities and unauthorized subcontractors.
