It is not new for this SOW.
Attachment J.1 - SOWTask 3: Determining Compliance Status for Hospice, IRF, LTCH, SNF and HHA Quality Reporting Programs i) Targeted outreach to PAC providers not meeting the assessments and/or CDC National Healthcare Safety Network (NHSN) requirements10Regarding i): What is the current method/ process for targeted outreach to PAC providers not meeting the assessments and/or CDC NHSN requirements'?Currently, the incumbent maintains a database of providers that was originally created using the contact information in the QIES-ASAP system. Providers have the opportunity to contact them directly to add additional contacts to their listserv database. Outreach is performed via email approximately 30 days prior to each quarterly submission deadline to those providers who have not yet submitted their required data.
Attachment J.1 - SOWTask 3: Determining Compliance Status for Hospice, IRF, LTCH, SNF and HHA Quality Reporting Programs j) Provide a list to CMS of facilities (including HHA when applicable) that meet or do not meet each APU criteria; the list should be provided in a tool that will allow CMS to view determinations by the APU requirements.10Regarding j): Does this tool currently exist and if so what technology is used in the tool?CMS is not dictating the tool to be used for this task. The list would be generated from the analytics performed using CAADS and can be provided in any format, Excel, Word, etc. that is mutally agreed on by the contractor and CMS.
Attachment J.1 - SOWTask 3: Determining Compliance Status for Hospice, IRF, LTCH, SNF and HHA Quality Reporting Programs k) Provide all communications with providers, including supporting documentation for reconsideration requests, to the reconsideration contractor in a format agreed upon by both contractors.11Regarding k): How much provider communication and how many reconsideration requests were provided to the reconsideration contractor in 2018 and so far in 2019?For FY 2018 and FY 2019 APU, there has been a non-compliance rate of 5-10% for IRF and LTCH PAC providers; Hospice and HHA, approx 10-15% non-compliance rate; SNFs have had a non-compliance rate of <5% but were granted some leniencies due to newness of the QRP and other data submission system-related issues. Not all providers who receive initial notices of non-compliance submit reconsideration requests. For FY2019 APU - the number of reconsideration requests submitted by PAC was as follows: HHA - 122 recon requests; Hospice - 85; IRF - 32; LTCH - 20; and SNF 142 recon requests.
Attachment J.1 - SOWTask 3: Determining Compliance Status for Hospice, IRF, LTCH, SNF and HHA Quality Reporting Programs m) Prepare/assist in the development of briefing documents/materials related to APU (including HHA when applicable)11Regarding m): how many briefing documents/ materials were developed in 2018 and so far in 2019?Each program requires a minimum of 2 briefing documents - one for the pre-reconsideration briefing and one for the post-reconsideration briefing. Ad hoc briefing documents could be required for other circumstances (e.g. natural disasters, issues with provider ability to submit data via the data sytems ie, NHSN or QIES or iQIES)
Attachment J.1 - SOWTask 3: Determining Compliance Status for Hospice, IRF, LTCH, SNF and HHA Quality Reporting Programs n) Generate the initial and final notification letters of non-compliance that are sent via the CMS data submission system notifying providers they are subject to the 2% reduction in their annual payment update.11Regarding n) how many initial and final non-compliance notification letters were generated in 2018 and so far in 2019? How many were sent via USPS?See response to question 14. FY2020 APU files still being finalized but appears to be following same estimates. Initial notifications are sent to all providers determined to be non-compliant. Final notification letters are only sent to those who submitted a reconsideration request to inform the provider whether CMS upheld or overturned their decision. In regards to use of USPS, CMS preference is to use the MACs and the QIES/iQIES reports folders. USPS will be used if we are unable to meet the timeline for sending via the other routes. However, the post-reconsideration contractor does mail final notification letters via USPS to those providers who submitted a reconsideration request.
Attachment J.1 - SOWTask 4: Help Desk Support11Can CMS provide the average monthly helpdesk ticket volume for each of the PAC programs included in the SOW? How many inquiries were received into each program specific mailbox in 2019 and so far in 2019?The largest volume of reconsideration helpdesk tickets occurs during the peak APU months: July (last year approx. 350) and August (last year approx 550) for FY APU programs and October (last year approx 100) and November (approx. 50) for CY APU programs. For the other months, volume has typically been 50-100 emails.
Attachment J.1 - SOWTask 4: Help Desk Support12Can CMS clarify whether external users will submit questions solely to the awardees program-specific mailboxes stated in the SOW? Or will external users also have access to other centralized products such as JIRA for web-based ticket submission? Can CMS clarify how or whether external user questions submitted to program-specific mailboxes will interact with the ServiceNow application?Reconsideration requests can only be submitted to each PAC program's specific reconsideration mailbox already established. This is the only mailbox used for questions related to compliance, APU, or thresholds. Currently Service Now or JIRA is not required for the operation of these mailboxes but if needed in the future, contractor would be required to obtain access so that they can support the reconsideration helpdesk mailboxes.
Attachment J.1 - SOWTask 4: Help Desk Support12In section 4.G, CMS requires a two-day turnaround for answering public questions and submitting the answers to CMS for approval. However, at times additional information will be required from the submitter in order to better define the question or provide additional information/context. In these situations, would submission to CMS noting the additional information needed meet the two-day turnaround?yes - as long as the submitter has received confirmation of the receipt of their question.
Attachment J.1 - SOWTask 4: Help Desk Support c) Develop a tracking tool, for each provider type, for questions/ responses e) Develop timeline and format for reporting, to CMS or other contractors, questions received by the mailboxes g) Assist CMS by drafting answers for CMS approval in response to questions received in the mailboxes and generate a database to capture responses previously approved by CMS.12Regarding c), e), and g): Do the tracking tool, CMS reporting, and the database for capturing responses all currently exist or is this new functionality?CMS does not have an existing database or tracking tool. We defer to the contractor to develop the tool to be used for these purposes.
Attachment J.1 - SOWTask 5: Support for Data Collection, Public Reporting, and Rule Writing: a) Provide subject matter expertise on all procedures necessary for providers and vendors to submit data to CMS including: • Access to reports and other resources on the CMS designated system portal (currently the Quality Improvement Evaluation System (QIES)). It is expected that the Contractor will also be an active participant supporting the PAC programs as they transition to the Internet Quality Improvement Evaluation System (iQIES) so they will be able to provide support for providers and vendors following the conversions.13What is the timeline for PAC program transition to the iQIES? Would CMS clarify the transition support required from the contractor?The migration to iQIES is occurring in phases by program. Transition support required would be related to updating/revising existing business requirements for the provider QRP reports and BPMs; particpate in the review/development of the provider threshold report; and any support needed in regards to APU or other compliance activities to ensure that iQIES is capturing necessary requirements correctly.
Attachment J.1 - SOWTask 5: Support for Data Collection, Public Reporting, and Rule Writing: b) Applying lean principles, the Contractor shall assist with the development of business requirements for the provider reports,13 and 14Do business requirements and BPMs documenting the reports/data file specifications and exchanges required to support the annual public reporting releases currently exist for all 5 programs?Yes - but not at the same level of detail. All require updating and we are requesting the format be aligned among all programs.
Attachment J.1 - SOWTask 6: Additional Business Requirements and Business Process Modeling for the Quality Reporting Programs b) Create business requirements for the quality reporting programs provider reports, (e.g. confidential feedback reports (QM reports), review and correct, and provider threshold reports), and provide support for other reports, or other system changes, necessary to support quality reporting program policy changes.14Do business requirements for the quality reporting programs provider reports, (e.g. confidential feedback reports (QM reports), review and correct, and provider threshold reports), other reports, or system changes currently exist for all 5 programs?See above response. The requirements need to be updated when there are changes to the reports - ie, addition or removal of new quality measures, or other items as finalized in the rules.
Attachment J.1 - SOWTask 8: Data Analyses16In Task 8, CMS requires the creation of compliance report templates within the CAADS environment. Is the intent for these templates to be used by Task 3 analysts actually running the compliance analysis? Or is the intention of these templates for another purpose? Do the templates necessary to produce reports to identify each provider’s compliance with CMS’ quality reporting requirements currently exist in the CAADS tool?Task 3 refers to the tasks required in CAADS to merge the data needed to generate and compile the list of compliant providers on a quarterly and annual basis. Task 8 requires the contractor to utilize the templates in CAADS to generate the reports to identify provider compliance for each of the PAC programs needed to communicate. These reports are required for the MACs and as support for the APU briefings.
Attachment J.1 - SOWTask 9: Monitoring and Evaluation16Is the current M&E strategy and M&E implementation plan available at transition?No. CMS will work with the contractor to discuss steps for development of an M&E strategy as stated for Task 9 a (pg. 16). It is anticipated that these discussions would begin after the contractor has had some time to review the recent trends in compliance and data reporting so that they are able to actively participate in making knowledgeable recommendations regarding an M&E strategy.
Attachment J.1 - SOWQuality Assurance17Will documentation for the current 100% validation check of all data files submitted and the established business rules and data collection protocol be available at transition?Quality assurance is the responsibility of the contractor to produce and implement. CMS expects the contractor to embed a quality assurance process within each task to ensure high quality deliverables.
Attachment J.1 - SOWInformation Technology (IT) Requirements18Do any of the current pre-existing environments used in support of this task order exist outside of the HCQIS FISMA boundary?CAADS is the data environment being used to support this contract and exists within the HCQIS cloud.
