Notice: THIS RFP IS ISSUED PENDING AVAILABILITY OF FUNDS
TO ALL OFFERORS:
This document officially issues Amendment Number 1 to this Request for Proposal (RFP) No.
75FCMC19R0032, entitled “Post-acute Program Analytics, Outreach and Support”.
The purpose of this amendment is to release the following:
• Attachment J.7 - Question Submission Format to Transmit Industry Questions and
Government’s Responses
In addition, the following attachments have been modified to align with the responses to Industry questions:

Attachment J.1 – Statement of Work Attachment J.2 – Schedule of Deliverables
As a reminder:
1) Proposal Due Date: Please submit an electronic copy of your Technical, Business, and
Conflict of Interest Proposals on or before 11:00 AM EST on July 2, 2019 via e-mail to the Contract Specialist, Theresa Doherty at Theresa.Doherty@cms.hhs.gov and the Contracting Officer, Kelley Williams-Vollmer at Kelley.Williams-Vollmer@cms.hhs.gov.
No hard copies will be accepted.
Late proposals, as defined in HHSAR 352.215-70, may not be considered unless it is determined to be in the best interest of the Government.
Please be advised that the issuance of this RFP does not commit the Government to pay any cost for the preparation and submission of a proposal. In addition, the Contracting Officer is the only individual who can legally commit the Government to the expenditure of public funds in connection with this procurement.
