About this file

This is a minimum insurance requirements document for a PFOA and PFOS Analytical Testing Results project administered by Broward County's Water and Wastewater Services Agency in Florida. The project requires vendors to furnish and deliver test bottles and results of PFOA/PFOS testing from Effluent, District 1A and District 2A Points of Entry (POE), and Biosolids samples. The document establishes comprehensive insurance coverage requirements that all contractors must maintain throughout the contract period and for three years following completion of work on claims-made policies.

The insurance requirements include General Liability coverage with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate; Auto Liability of $500,000 combined bodily injury and property damage (waivable if no driving is required); Professional Liability (Errors & Omissions) with $1,000,000 per claim and $2,000,000 aggregate with a maximum deductible of $100,000; Workers' Compensation at statutory limits; and Employers Liability of $100,000 per accident. Broward County must be named as an additional insured with primary coverage, and a waiver of subrogation applies in the County's favor. The contractor's insurance must provide 30 days' written notice of cancellation and 10 days' notice for non-payment cancellation to Broward County's Risk Management Division.
