About this file
This is a Leased Space Unescorted Access Questionnaire (FAA Form 4400-88) issued by the Federal Aviation Administration for Lease Contract #697DCM-27-L-00001 in Savoy, Illinois, dated September 1, 2026.
The questionnaire is designed to gather information about unescorted access requirements for lessor employees and contractors who will need to access FAA-controlled leased space. The FAA uses the information collected to determine appropriate background investigation requirements and Personal Identity Verification (PIV) badging needs. Respondents must identify the estimated total number of employees, contractors, and subcontractor personnel requiring unescorted access and provide detailed information for each employee position category, including position titles, number of employees, and whether employees will require routine access (more than one day per week) to FAA facilities for more than 180 days. The questionnaire allows for multiple position categories and additional positions as needed. The form notes that the public reporting burden is estimated at approximately 30 minutes per response and includes an OMB Control Number (2120-0595). Completed questionnaires must be returned to the FAA Real Estate Contracting Officer.
