About this file
This is a vendor disclosure and notification form required by the Colorado Department of Corrections for any contract performance occurring outside the United States or the State of Colorado, as mandated by Colorado Revised Statutes 24-102-206. The document establishes a mandatory reporting requirement for vendors and their subcontractors who anticipate performing any services under the awarded contract outside U.S. or Colorado borders, with the completing signatory being the business president, board chairperson, or the contract's authorized representative.
The form requires vendors to disclose whether services will be performed outside the United States or Colorado, and if so, to specify the type of services, the geographic location(s) where services will be performed, and the justification for performing work outside these boundaries. The contractor must provide written notification within 20 days of contracting or deciding to contract outside the U.S. or Colorado. The form includes signature blocks for contractor authorization and designated space for procurement office use to document the solicitation number, contract type, services performed outside the U.S. or Colorado, and confirmation of timely vendor notice submission.
