About this file
This document is a Bidder's Certification form for a state contract opportunity, specifically for the State of Ohio. The form requires bidders to certify several key representations, including understanding the project specifications, having visited the site, being familiar with local conditions, and agreeing to comply with all contract requirements. Bidders must affirm that their bid was developed independently without collusion, that they will not discriminate, and that they will make a good faith effort to ensure employees do not use illegal drugs or alcohol while working on state property.
The certification requires bidders to provide comprehensive contact and company information, including company name, mailing address, telephone number, vendor identification number, and contact details for contract processing. Bidders must submit compliance verification documentation with their quote and agree to furnish any additional information requested to evaluate their responsibility. The document emphasizes that the entire proposal, including all attachments and amendments, will become part of the final contract, and any modifications must be made in writing.
