1. 8-15-19 8-16-19 Page GI001 Note 3--Says to coordinate all space requirements for this project. On sheet GI201 site plan it shows location for dumpster and contractor parking. Yet on the Pharmacy plans (GC101) which both of these projects will be going at same time it shows a laydown area where our dumpster location is and shows the contractor parking to be in the same location as the kitchen job. Please determine which is correct.

The layout area is marked and indicated on clean North dock. This focus is on the renovation of the kitchen project. Contractor parking is limited and will be established prior to mobilization. A material laydown area, if needed, will be available near the parking deck. The dumpster will have space near the parking deck on the east dock area.

Another location can be near the North dock area.

The exact location will be determined during coordination. Refer only to only this project.

2. 8-15-19 8-16-19 Sheet GI201 there is a General Note A that says that a temporary location for utilities, box truck and refrigerated trailer is shown. Nowhere other than this note does it show a location. We either need an exact location where these will be located as to figure into our pricing what has to be run to the truck or an allowance of some kind given for this.

The reefer trailer will be a 53 ft trailer, at a minimum, with a diesel motor/generator to maintain temperature control. This trailer will be located at the north loading dock area. The typical refer trailer will come with dividers. The box truck will be furnished by the VA. The refrigerated trailer should not require utilities. The reefer trailer will store perishables and be operated by kitchen staff. The reefer trailer will be on a submittal for approval.

3. 8-15-19 8-16-19 Since the VA knows what goes into the refrigerated truck we need a list so that the corrected size truck and trailer can be used.

The contents of the trailer are not required for planning concerns. The items in the trailer will be managed by VA staff.

4. 8-15-19 8-16-19 Page GI001 Note 5--Says that if an unforeseen condition or problem is found after starting the work and the plans, specs and the work area disagree with each other then who decides what work is reasonably anticipated to have been found during walkthrough. This walkthrough started promptly at 10:00am however there were about 20 people who wanted to attend but were held up because the walkthrough on one of the other projects started and ended late. Should offer another walkthrough to whoever needs to go. Lots of things are found to be different after the work starts.

There will not be another walk through. The walk through started at 10:00 AM and restarted at about 10:20 AM when others joined late. There was plenty of time for questions/answers. All questions were answered on site.

5. 8-15-19 8-16-19 Page GI301 Note 2--States that temporary smoke detectors will be hooked up to the fire alarm system and a certification must be gotten that they work with the existing alarm system. There is no mention of heat detectors in the notes that might also need to be used as mentioned in the walkthrough.

See EF10G-2. Temporary Heat Detection Plan.

6. 8-15-19 8-16-19 Page GI401 Note F.--F.3 states that the temporary construction door must be fitted with a card reader. What purpose does that serve. No one other than VA employees could get into job. On all other previous jobs at JLM we have used a cypher lock so that subcontractors will have code to get in to do their work.

Contractor may use cypher lock but the lock must have a place for 7 pin Best core. The VA will provide the core.

7. 8-15-19 8-16-19 In several location on the drawings there are notes for floor protection that states put down heavy duty 6 mil fire retardant visqueen then on top of that put down 3/8" plywood fastened and taped together. We have been told through safety that wood products are not allowed on the job because of a fire hazard. That is why the temporary dust walls went from plywood to sheetrock. I know these are temporary coverings, but the walls were temporary also and could not be used. Please advise.

Answer concerning first half of this question could not be provided as vendor did not cite page numbers concerning drawing notes.

Temporary walls will be double sided sheet rock walls.

8. 8-15-19 8-16-19 Can you provide info on the climate-controlled space that you call for to store equipment in. Can you give maybe the temperature variances required, etc.

Temperature range for equipment storage is 65-72 degrees F. Humidity range is 20-40 per cent.

9. 8-15-19 8-16-19 In the specifications 01 00 00 - 5 Paragraph F subparagraphs 1 & 2 it calls for some type of vehicle authorization request for every vehicle entering the jobsite and this must be done 24 hours in advance. It also states that the GC will be issued from 2 to 5 permits to be used in designated parking areas. We still don't know where those area are located. There is a need for at least one job shack and probably up to three connexs that need to be located. Please advise.

Contractor will have limited parking available designated by the project COR. Contractor will be provided a laydown location behind the east dock and by the parking deck. Contractor will have to manage the space allocated. Contractor must coordinate space requests with project COR.

10. 8-16-19 8-26-19 Page ASB101 General Notes--Last note states that the asbestos abatement contractor will be responsible for the daily air monitoring. Thought that the new VA rule from safety was that they would provide a third-party monitoring service and it could not have the abatement contractor to do both. Please advise.

Air monitoring will be the responsibility of the general contractor. This may not be the responsibility of the company performing asbestos removal.

11. 8-16-19 8-26-19 Page GI402 shows on information required to go on temporary wall entrance that a framed picture of the COR and their contact info must be put on wall. Is the VA furnishing the framed picture?

No.

12. 8-16-19 8-26-19 Page GI401 shows the corridor (no number given) next to rooms GC132,143 & 150 to have the same keyed notes #'s 6,7 and 8 as the rooms have. This means that the ceiling tile and grid would come out, light fixtures removed, sprinkler heads turned up, etc. all in that one area. It shows absolutely no work to be done there on the demo sheet ACD10G or on the new reflective ceiling plan sheet AC10G. Please advise as is to what is required.

No work will be done in this corridor.

13. 8-19-19 8-26-19 Sheets I611, I612 & I613 equipment schedules, TAG Numbers 001,002,003,004,029,031,032,038,095 &118 are all duplicate numbers show twice on the plans. Please advise.

Item description are listed on the sheet. A complete inventory of equipment will be accomplished with the Nutritional Food Service Staff and the General Contractor to account for equipment to be stored.

14. 8-19-19 8-26-19 Sheets I611, I612 & I613 equipment schedules, TAG Numbers 027, 030A, 049, 050, 051, 052, 064, 065, 066, 067, 068, 069, 070, 071, 072, 073, 076, 078, 079, 091 & 094 are not called out on Sheet IQ10G. Please advise.

15. 8-19-19 8-26-19 Sheet I613 TAG Numbers 137, 138, 139, 140, 141, 142 &143 are not shown. Please advise on what sheets they can be found on.

16. 8-20-19 8-26-19 Does the panel in GC143 get demo out? If so where will we pick up the power for any circuits that feed elsewhere?

No.

17. 8-20-19 8-26-19 What are the dimensions of the basket tray called out on EJ10G?

Solid Metal Raceway, 6” Wide, per Specification 28-05-00.

18. 8-20-19 8-26-19 Who will supply the utility distribution systems? Shown in drawings 4 (-Q 1-6)

These systems will be provided by the contractor.

19. 8-20-19 8-26-19 Will all bending and cutting of conduit have to be done outside or can it be performed indoors?

This work may be either indoors or outside. If any sparks are produced a hot work permit must be obtained.

20. 8-20-19 8-26-19 Will extended work hours be allowed, nights and weekends?

Yes.

21. 8-20-19 8-26-19 How many and how large of a temporary refrigerated storage trailer is needed?

53 ft. long. One trailer is required.

22. 8-20-19 8-26-19 Who is responsible for paying for temporary utilities associated with the temporary refrigerated storage trailer?

There should be no utilities as the refrigerated trailer shall be refrigerated by a generator on the trailer.

23. 8-20-19 8-26-19 Can negative air be connected to the existing exhaust system serving the kitchen?

Duct to ceiling will be acceptable outside kitchen and approved by safety. Must identify which exhaust.

24. 8-20-19 8-26-19 Looks to me as far as commissioning goes that will be contracted separately by the VA for this job. On page 01 91 00 - 15 under paragraph 1.8 Commissioning team paragraph C. 1 it states members appointed by VA is the commissioning agent and again in 1.9 VA's Commissioning Responsibilities sentence A. state that the VA will appoint an individual, company or firm to act as Commissioning Agent. Please advise.

VA will provide commissioning agent if required.

Contractor must perform TAB.

25. 8-20-19 8-26-19 The box truck called for in the plans will that also be a VA supplied item. Truck, personnel, fuel, et. Please advise.

The cargo truck is responsibility of VA to supply, staff and operate.

26. 8-20-19 8-26-19 What elevator will contractor be permitted to use? What is the weight limit for that elevator? What are the dimensions of the elevator?

None. Job site is in basement. Contractor entrance will be on same floor.

27. 8-20-19 8-26-19 Will the VA assign toilet accommodations during the project? If so, where will the assigned toilet be located?

Yes. Will be near work site.

28. 8-20-19 8-26-19 Electrical: Sheets E504 and E601: In regards to "Utility Distribution System B".... The Panel schedule for Panel "GHK" shows a 150amp breaker for this, BUT Panel "GPL18 Section1" shows a 70amp breaker for this also.

Please advise.

Utility Distribution System B requires both 480/277V and 208/120V power. GPL18 provides 208/120V power and GHK provides 480/277V power. The breaker sizes are correct.

29. 8-20-19 8-26-19 Electrical: Sheet E504 shows that "UDS-B" should be fed from a 70amp breaker. Please verify that the 150amp breaker shown in Panel "GHK" is not required.

Both breakers are required, E504 shows 70A breaker for 208/120v and 150A breaker for 480/277V sections.

30. 8-20-19 8-26-19 Electrical: On Sheet EJ10G there is some "wireway" called out. I assume this is to be cable tray, but there is no size mentioned, nor does it state if it is to be wire basket, or ladder style. Please advise as to type, and size required.

6” Wide, Solid Metal Wireway should be purchased per Specification 28-05-00.

31. 8-20-19 8-26-19 Electrical: Sheet EP10G Keyed Note #8 states the existing conveyor to remain as-is, yet there is a 2"conduit drawn as a new feeder, is this new conduit/conductors required since the note states to remain as-is, or is this existing?

The conveyor will be removed, stored and reinstalled. See schedules on drawing I 612 Item No. 103. The schedules identify if conduit and wiring is new or will be re-used.

32. 8-20-19 8-26-19 Electrical: Sheet EP10G in room#GC166 there is a Keyed Note#18, yet the keyed note legend on this page only goes up to 15. Please provide the required information for this Keyed note.

Keyed Note #18 should have been deleted.

33. 8-20-19 8-26-19 Electrical: Sheet EP10G Keyed Note#6 Calls for some "plug mold", please provide part#'s/specs on what is to be provided.

Contractor to purchase LEGRAND or equal Plug Mold; Steel 2000Series, Ivory Scuffcoat, 6 Ft long, Wire Configuration GB, with 12 IN. outlet spacing.

34. 8-20-19 8-26-19 Upon review of the TEM Asbestos Inspection Report in conjunction with the Laboratory Results and Sample Chain of Custody documents in Appendix A, numerous pages that provide the laboratory testing results (but are referenced in the Inspection Report and Chain of Custody documents) are missing from the Laboratory Results. Please provide all missing pages

Revised report re-issued by TEM. Reference ATTACHMENT 7 - Asbestos Report - Little Rock Kitchen - Revised 8-26-19 provided with Amendment 0002.

35. 8-20-19 8-26-19 Will you provide another site visit to verify asbestos quantities?

No. The location of asbestos is shown.

36. 8-20-19 8-26-19 Drawing GI201 indicates a dumpster in the same space as the laydown area referenced on Drawing CG101of Upgrading Compounding Pharmacy (Solicitation# 36E77619B0008). Will the laydown area be required to share the same space as the dumpster?

37. 8-20-19 8-26-19 Per Drawing 601, please provide hardware schedule for doors GC143, GC143A, CG145, and CG 144. These were not included in the door schedule.

38. 8-20-19 8-26-19 Drawing IQ10G – Furniture/Equipment General Notes, Notes F and G reference off-site storage of food inventory and kitchen supplies. Will the location of the off-site storage be provided by the VA or is the contractor to provide the location? If the contractor is to provide the location, what are the parameters for the location of such off-site storage? For example, must it be within a specific mileage radius? Will the VA need to access the off-site storage containers while the food inventory and kitchen supplies are in storage?

The contractor will provide the storage space for the kitchen equipment. The location must be temperature controlled, safe and secure. Storage and location will be a submittal for approval.

39. 8-20-19 8-26-19 The Brand Name Justification Letter identifies that the Elkay M# LZSDWSSK L/ REFRIGERATION water fountain is to be used for this renovation. Please clarify what the L/ REFRIGERATION means?

Less refrigeration.

40. 8-20-19 8-26-19 I have gone through the specs on the John L McClellan VA Kitchen project, and I cannot find the section 114000 Foodservice Specs. Section 018113 (Pg. 9) states that there are dishwashers, fryers, griddles, hot food holding cabinets, etc... however, I can’t find any information on what is wanted. Is there something that I am missing, or are there simply not any specs on this equipment?

There is no new cooking equipment specified all will be removed for demo then reinstalled at the completion of construction.

Existing hoods also to remain in place and protected during construction. The only item new are the utility distribution systems (which supports the equipment – electrical, plumbing) by Caddy, attached to the wall as shown on the drawings and one under hood.

41. 8-5-19 8-26-19 I am submitting the following Product Data Sheets for consideration as an equal product for section 09 67 23.

60, Resinous Urethane Mortar Flooring.

The M1 MVE is MIlamar’s water proofing primer The Ure Guard is our resinous Urethane mortar The 3300 veil coat is the epoxy mid coat PM 500 chemical resistant Urethane topcoat.

The product substitution information for Urethane Mortar Flooring has been reviewed and does not meet the requirements to be deemed an approved equal.

42. 8-8-19 8-26-19 Tennant Coatings, Inc. (the third largest manufacturer and supplier of epoxy/resinous floor coatings) through one of our preferred contractor/installers would like to provide a bid for the Seamless Resinous/Epoxy Flooring for the Renovate NFS Kitchen-John L. McClellan VA Medical Center using our line of products. I have prepared a Request for Substitution of an equivalent system using Tennant Company Products and have attached it along with product information for your review.

The product substitution information for Tennant Coatings has been reviewed and does not meet the requirements to be deemed an approved equal.

43. 8-22-19 8-27-19 Can you revise the period of performance to reflect 280 days, instead of the current 180 days?

Please be advised that the period of performance has been adjusted from 180 to 210 calendar days after date of receipt of Notice to Proceed. Please reference the attachment titled “Price Schedule - Little Rock Kitchen - Revised 8-27-19”, which has been posted with this Amendment 0002.

Bidders must use this document when providing their bids.

Also, the period of performance will be adjusted in Block 11 of the SF 1442 and in FAR clause

52.211-10 COMMENCEMENT,

PROSECUTION, AND COMPLETION OF

WORK (APR 1984). These adjustments will be made upon award to the lowest bidder.
