STATEMENT OF WORK AND CRITERIA FOR

I. TITLE:

Apply Herbicide to the Campus Grounds, Asphalt and Sidewalks.

II. BACKGROUND INFORMATION:

The hospital campus pavements, grounds, sidewalks need herbicide applied quarterly to control the weeds emerging and making the grounds unsightly.

III. OBJECTIVES:

The purpose of this contract is to have a Contractor to furnish all labor, materials, supplies, tools, and equipment to apply herbicide to approximately 18 acres, asphalt parking spaces and sidewalks. Services performed shall conform to industry standards and this statement of work (SOW). Additional scope as per the below:

• Contractor to mechanically knock down tall weeds prior to applying herbicide.

• Contractor to post signs where herbicide is being applied.

• Contractor to retain record of areas where herbicide was applied and quantity of herbicide applied to submit to the Maintenance and Operations Chief on a quarterly basis.

• Contractor to provide Safety Date Sheet (SDS)for review and keep a copy onsite for the applicator.

• Contractor to ensure secondary containment for herbicide brought onsite.

• Contractor to clean-up spills immediately and prevent runoff of herbicide.

• Contractor to obtain permission to use suggested herbicide which should be on the Department of

Defense (DOD) approved herbicide list.

• Contractor to avoid applying herbicide on days where there is excessive wind.

PERIOD OF PERFORMANCE:

One (1) Year (Base Year) and Four (4) Option Years

IV. PLACE OF PERFORMANCE:

VA Loma Linda 11201 Benton St Loma Linda, CA 92357

V. LICENSING REQUIREMENTS:

a. Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card.

b. Offeror should have a State of California Herbicide license.

VI. SPECIFIC REQUIREMENTS:

a. Schedule with Contracting Officer Representative (COR) or Designee five (5) business days in advance prior to start of work.

b. Prior to start of job, all personnel shall have a valid Loma Linda PIV badge

c. Prior to working, ALL energy sources (electrical, water, etc.) shall be locked and tagged out, and upon completion of work, will be re-energized and verified.

d. Prior to work each day, workers or a designated lead or supervisor are/is to check in with the Graphics Control in the plant.

e. Contractor shall conform to the following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here
