Provide all work as described by attached plans, specifications, and Wage Determinations for Veterans Administration project 1st Floor Carpet Replacement VAMC as described in the attached statement of work. Price includes all costs for construction. Cost shall include but not be limited to direct and indirect costs including profit, overhead, general and administration, insurance, labor, bonds, taxes, necessary for 100% completion of the project as indicated in the attached SOW. The construction performance period is 30 calendar days from receipt of notice to proceed.

A.4 Statement of Work

1ST FLOOR CARPET REPLACEMENT

1. PURPOSE: The Western Colorado Health Care System (WCHCS), located at 2121 North Ave, Grand Junction CO, 81501, has a requirement for replacement flooring in its 1st floor waiting areas and entry way. Business hours 7am to 4:30 pm. Work will take place during non-business hours and preferably weekends.

2. BACKGROUND: WCHCS has a requirement for the removal of existing carpet, and replacement with carpet tiles, in the locations shown on the attachments. Remove of borders in areas shown on plans, patch walls, paint install new carpet tiles.

3. Scope: The selected contractor shall furnish labor, manufacturer-approved parts, equipment, and technical support to provide preventative and corrective maintenance. Services and parts shall meet manufacturer’s technical specifications and maintain the original equipment warranty. Work will take place on the Grand Junction Station of the VA Western Colorado Health Care System (WCHCS). Remove existing carpet and replace with new carpet tiles in Building 1, first floor in the locations shown on the attachments. Remove wall borders in areas shown on plans, patch walls, paint (as needed). install new carpeting tiles.

4. Mandatory Tasks and Deliverables: The Contractor shall perform all tasks listed below in accordance with this statement of work (SOW).

Task 1: 1: Primary Care waiting (approx. 2268 SF): Remove existing carpet and base; reserve base for reinstallation and dispose of carpeting. Paint walls. Prepare floor for installation of new like carpet (i.e. see photograph). Contractor shall meet specifications as per document titled “carpet specification.docx” Obtain and install new carpeting tiles. Reinstall reserved base; install new base as required; install flooring transitions as required.

Task 2: Pharmacy Pickup waiting (approx. 436 SF): Remove existing carpet and base; reserve base for reinstallation and dispose of carpeting. Prepare floor for installation of new like carpet (i.e. see photograph). Contractor shall meet specifications as per document titled “carpet specification.docx” Obtain and install new carpeting tiles. Reinstall reserved base; install new base as required; install flooring transitions as required.

Task 3: Pharmacy drop off waiting area (approx. 230 SF): Replace carpet in waiting area, Furniture can be moved to side and restored at the end of the night’s work. Remove existing carpet and base; reserve base for reinstallation and dispose of carpeting. Prepare floor for installation of new like carpet (i.e. see photograph). Contractor shall meet specifications as per document titled “carpet specification.docx” Obtain and install new carpeting Tiles. Reinstall reserved base; install new base as required; install flooring transitions as required.

Task 4: Replace carpet in front Vestibule area (approx. 240 SF), Furniture can be moved to side and restored at the end of the work. Remove existing carpet and base; reserve base for reinstallation and dispose of carpeting. Prepare floor for installation of new carpeting (i.e. see photograph). Contractor shall meet specifications as per document titled “carpet specification.docx” Obtain and install new carpet tiles. Reinstall reserved base; install new base as required; install flooring transitions as required.

5. Project Management: Based on the above list of work items the general contractor selected must complete, maintain, and submit the following documentation and managerial requirements:

· Construction schedule including sequencing and durations.

· The schedule will be provided to the Contracting Officer’s Representative (COR) ten (10) working days prior to the start of work.

· Site-Specific Safety Plan (SSSP) with an emphasis on tasks to be performed during the work. The SSSP must:

· Include a detailed description of how the work will be performed and be submitted with sketches, supporting documents, submittals etc.

· Include a Job-Safety-Analysis (JSA) for all tasks associated with Statement of Work (SOW)

· Be reviewed and commented by VA-Safety and COR prior to commencement of work.

· Maintain site supervision by a competent individual(s) with OSHA-30 certificates. OSHA-30 certified technician shall be onsite anytime work is being performed.

· Obtain VA identification PIV badges for all employees. Contractor to consider lead-time for badge procurement.

· Manage job-site access and security.

· Implement and maintain required ICRA and ILSM measures.

6. Submittals: Contractor shall submit the following prior to beginning any work on site.

· Project schedule with sequencing, durations, phasing, etc.

· All material submittals – Submit to COR for approval. (i.e., this includes but is not limited to the carpeting, paint, and any other materials involved in the scope of this project).

· Site superintendent OSHA-30, and tradesman/laborer OSHA-10 certificates.

7. Period of Performance: Work shall be completed within 30 days of Notice to Proceed.

Under current definitions, five holidays are set by date:

The Contractor shall be responsible for managing and overseeing the activities of all Contractor personnel, as well as subcontractor efforts used in performance of this effort. The Contractor’s management responsibilities shall include all activities necessary to ensure the Accomplishment of timely and effective support, performed in accordance with the Requirements contained in the statement of work.

8. Changes to the SOW:The Contracting Officer, from information furnished by the COR and Contractor, may direct changes to the drawings and/or specifications that are within the general scope of the contract. If such change is necessary, it shall be accomplished via a written contract modification between VA and the Contractor.

9. Government and Contractor Responsibilities. The selected Contractor shall furnish all management, supervision, labor, material, equipment, tools, supplies, parts, and related services, in accordance with the VA plans and specifications, as necessary for the completion of work. Contractor shall be responsible for verifying all quantities and dimensions.

10. Hours of Operation: Work by the Contractor shall be completed during non-regular business hours, between 5pm and 11pm M-F, or Between 7am and 5pm Sat and Sun. Contractor’s work shall be conducted in a manner and at times which would present the least amount of disruption to the VA healthcare facility.

11. Safety: Safety shall be the Contractor’s highest priority. The Contractor shall be solely responsible for compliance with Federal/State/Local (F/S/L) OSHA regulations. The Contractor shall furnish and have in place Personal Protective Equipment (PPE), safety signage, cones, barricades etc., to protect the safety of VA patients, employees, visitors, volunteers, tenants, and others. Contractor employees, and Contractor suppliers shall not be permitted to work and shall be asked to leave if the proper safety items are missing. Contractor shall furnish required safety items.

12. ICRA: The Contractor shall comply with an Infection Control Risk Assessment (ICRA) which will be developed with the COR and the Infection Control Practitioner. Multiple ICRA’s may be necessary to address specific risks at various stages of the project and must be approved prior to proceeding on each phase. VA anticipates an overall ICRA level 3 for this project.

13. Acceptance Standards: The VA reserves the right to dispute completion of contract scope if contract work is found not in accordance with manufacturer recommendations, is not satisfactorily executed, or if the system is not properly tested and inspected to achieve certification. All work completed by the Contractor must be reviewed and approved by the VA COR.

14. Meetings: The contractor is required to attend the pre-construction meeting after award, this meeting will be held at a date and time specified by the Contracting Officer to review project scope, site, and requirements, as well as provide answers to any questions.

End of Statement of Work

INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO BIDDERS/OFFERORS

PART I - INSTRUCTIONS, CONDITIONS AND NOTICES TO OFFERORS

PART I/SECTION A – REQUEST FOR QUOTE

This is a Request for Quote in accordance with FAR Part 13. Potential offerors must follow all instructions to be compliant with the solicitation.

It is the responsibility of the offeror to verify that their offer is complete. The offeror is responsible for submitting all required forms and documents with the offer.

The Government will award a single contract resulting from this solicitation to a responsible SDVOSB offeror whose offer conforming to the solicitation will be most advantageous to the Government based on Past Performance and Price.

I.1A JOINT VENTURES

If submitting an offer as a joint venture as defined by P.L. 109-461 AND Department of Veterans Affairs I.L. 049-06-4. Certified SDVOSB/VOSB listed in the SBA certification database at https://veterans.certify.sba.gov/ (see 13 CFR 128). A joint venture may be considered eligible if it meets the requirements in 13 CFR part 128, and the managing joint venture partner makes the representations under VAAR 819.7003 paragraph (b). An offeror must provide a copy of the joint venture agreement specific to the project that is consistent with the percentage of required work and net proceed. Additionally, the offeror must provide a breakdown or labor of which is to be performed by the SDVOSB firm and that which is performed by the small business joint venture firm and that which is sub-contracted. All Joint Ventures will be required to be registered and verified as an entity in VETCERT.

I.4A SELECTION CRITERIA/QUOTE PREPARATION INSTRUCTIONS

Quotes will be evaluated in accordance with FAR 13.106 Simplified Acquisition Procedures Award will be made based on the following factors:

Factor 1 - Past Performance Factor 2 - Price Offerors are encouraged to provide their best offers with the past performance information requested for evaluation. The Government, based on the offered material presented, will award to the offeror that best demonstrates to be most advantageous for the Government.

Factor 1 - Past Performance: Offerors shall provide past performance information on two (2) federal, state, local government or private contract of similar type, scope, size, and complexity. For purposes of this evaluation, a relevant project is further defined as completing carpet replacement, painting, and drywall repair in a medical facility environment. Project shall have a minimum amount of $25,000.00 or more and completed within the past five years from the date of issuance of this RFQ. If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted with the quote. The Government may also evaluate recent and relevant past performance, based upon sources available to the contracting officer that may include Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS) and Electronic Subcontract Reporting System (eSRS). IF THE OFFEROR CANNOT PROVIDE A CPARS FOR PROJECTS LISTED IN FACTOR 1, then the offeror shall submit Past Performance Questionnaires (PPQ).

Completed PPQs should be submitted directly from your client via email to Timothy Verburgt at Timothy.Verburgt@va.gov. If the offeror is unable to contact a client for a project(s) before closing date, the offeror should complete and submit with the quote the first page of the PPQ, which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires.

Basis of Evaluation: Past Performance will be evaluated in accordance with FAR 13.106-2 and award will be made to the offeror presenting the best value to the Government.

The Government also reserves the right to award to the lower priced offeror with lower past performance rating if the Contracting Officer (CO) determines the price difference does not merit awarding to the higher priced offeror. In that event, the CO shall make an integrated assessment best value award decision.

Factor 2 - Price: Offerors shall provide pricing as requested in the Schedule of Service identified within the SF 1442 Basis of Evaluation: The Government will evaluate price based on the total price. Total price consists of the basic requirements. Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:

•Comparison of quoted prices received in response to the RFQ
•Comparison of quoted prices with the IGCE.
•Comparison of quoted prices with available historical information.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

Special Standard of Responsibility Solicitation Submittal Requirements:

The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture and for the entity itself. EMR shall not be submitted for subcontractors.)

1. OSHA or EPA Violation(s):

Offerors shall provide Summary of Work-Related Injuries and Illnesses, affirming that the Offeror has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years.

2. Experience Modification Rate (EMR):

Offerors shall provide documentation to show that the offeror has an Experience Modification Rate (EMR) of equal to or less than 1.0. If an offeror’s EMR is above 1.0, the offeror must submit a written explanation of the EMR from its insurance carrier on the insurance carrier’s letterhead, describing the reasons for the EMR and the anticipated date the EMR may be reduced to 1.0 or below.

Offerors who fail to meet any special standard of responsibility may not be eligible to receive contract award.

PART I/SECTION B - INSTRUCTIONS, CONDITIONS AND NOTICES TO OFFERORS

I.1B COST RANGE: Between $25,000 and $100,000 I.2B GENDER: Wherever the masculine gender is used in this solicitation and contract documents, it shall be considered to include both masculine and feminine.

I.3B METRIC PRODUCTS:

Products manufactured to metric dimensions will be considered on an equal basis with those manufactured using inch-pound units, providing they fall within the tolerance specified using conversion tables contained in the latest revision of Federal Standard No. 376, and all other requirements of this document are met.

If a product is manufactured to metric dimensions and those dimensions exceed the tolerance specified in the inch-pound units, a request should be made to the Contracting Officer to determine if the product is acceptable. The Contracting Officer, in concert with the Contracting Officer’s Representative (COR), will accept or reject the product.

I.4B REQUESTS FOR INFORMATION:

All requests for information must be submitted in writing to the Contracting Officer for action no later than the date shown in Block 10 of the SF 1442, Solicitation, Offer and Award.

I.5B QUOTE REQUIREMENTS

General – Both past performance information and price proposal shall be submitted in response to this solicitation. Quotes shall be submitted via email to Timothy.Verburgt@va.gov. Emails are limited to 15 MB. The proposal, in its entirety, shall be one email (one attachment for price proposal, one attachment for LOS certification, and one attachment for past performance information) not to exceed 15MB. Quote can be submitted in one email if 15MB or less. Failure to submit in the format required and clearly address those factors may result in rejection of the offer. Electronic copies of the quotes will be time/date stamped by Microsoft Outlook upon receipt of email. Further details on electronic submission are outlined below.

Quotes shall be received by 10:00 AM MT on July 10, 2023. There will be no public opening. Submit quotes to: Timothy.Verburgt@va.gov.

Important Notice: Apparent successful offerors must apply for and receive verification from the Small Business Administration (SBA) through the SBA certification database via 13 CFR Part 128, VAAR 819.7011, and VAAR 819.7003 by submission of documentation of Veteran status, ownership and control enough to establish appropriate status. Offerors must be both VISIBLE and VERIFIED by the SBA certification database at the time of offer submission. Failure to be both VERIFIED by SBA and VISIBLE on the SBA certification database at the time of offer submission and contract award will result in the offeror’s proposal being deemed non-compliant. All offerors are urged to contact the SBA and submit the required documents to obtain verification of their SDVOSB status if they have not already done so.

I.6B REQUIREMENTS FOR ELECTRONIC SUBMISSION OF OFFER

Unless paper offers are specifically authorized in an individual solicitation, all responses to solicitations must be submitted electronically as described below. Responses submitted in a paper form are unacceptable. Failure to comply with this requirement may jeopardize the possibility of receiving an award for the contract due to noncompliance with the terms of the solicitation. You must submit your electronic offer, and any supplemental information (such as spreadsheets, backup data, technical information), using any of the electronic formats and media described below.

In addition, contractors are notified of the award via an electronic Notice of Award e-mail. The award document will be attached to the Notice of Award e-mail.

Acceptable Electronic Formats (Software) for Submission of Offers

1. Files readable using the current Microsoft© Office version Products: Word, Excel, PowerPoint, or Access. Print or scan images of spreadsheets are not acceptable. Please see security note below for caution regarding use of macros. When submitting construction drawings contractors are required to submit one set in AutoCAD and one set in Adobe PDF (purpose: contracting can open the PDF version and engineering can open AutoCAD files).

1. Files in Adobe© PDF (Portable Document Format) Files: When scanning documents scanner resolution should be set to 200 dots per inch, or greater.

1. Other electronic format. If you wish to submit an offer using another format other than described in these instructions, e-mail the Contracting Officer who issued the solicitation. Please submit your request at least ten (10) calendar days before the scheduled closing date of the solicitation. Request a decision as to the format acceptability and make sure you receive approval of the alternate format before using it to send your offer.

1. E-mail Submission Procedures: For simplicity in this guidance, all submissions in response to a solicitation will be referred to as offers.

a. Subject Line: Include the solicitation number, name of company, and closing date of solicitation. Use only one of the terms Quotation, Offer, or Bid depending on the solicitation type.

b. Size: Maximum size of the e-mail message shall not exceed 15 megabytes. Only one email is permitted unless otherwise stated in this solicitation or in writing by the Contract Officer submitting the solicitation.

c. Unless approved by the contracting officer: DO NOT submit a link to your PDF documents. Please submit your request in an email for other electronic format at least ten (10) calendar days before the scheduled closing date of the solicitation. Request a decision as to the format acceptability and ensure you receive approval of the alternate format before using it to send your offer.

Each electronic offer must also include a signed and scanned SF 1442, Solicitation, Offer, and Award (Construction, Alteration, or Repair)

The Microsoft Outlook © Email time/date stamp will be used to date and time stamp offers for the official record of receipt for the submission. The date and time stamp in recipients “inbox” is the official record of receipt.

Security Issues, Late Bids, Unreadable Offers

1. Late submission of offers is outlined at FAR 52.215-1(c)(3). Particular attention is warranted to the portion of the provision that relates to the timing of submission.

1. Please see FAR 15.207(c) for a description of the steps the Government shall take with regard to unreadable offers.

1. To avoid rejection of an offer, vendors must make every effort to ensure their electronic submission is virus-free. Submissions or portions thereof submitted and which the automatic system detects the presence of a virus, or which are otherwise unreadable will be treated as “unreadable” pursuant to FAR 15.207(c).

1. The virus scanning software used by our e-mail systems cannot always distinguish a macro from a virus. Therefore, sending a macro embedded in an e-mail message or an e-mail attachment may cause the e-mail offer to be quarantined. You may send both the spreadsheet and the spreadsheet saved in PDF format to ensure that your quote is readable.

1. Password protecting your offer is not permitted. The Contracting Officer will file the offer electronically which will allow access only by designated individuals.

2.1 52.222-5 CONSTRUCTION WAGE RATE REQUIREMENTS—SECONDARY SITE OF THE WORK (MAY 2014) (a)(1) The offeror shall notify the Government if the offeror intends to perform work at any secondary site of the work, as defined in paragraph (a)(1)(ii) of the FAR clause at 52.222-6, Construction Wage Rate Requirements, of this solicitation.

(2) If the offeror is unsure if a planned work site satisfies the criteria for a secondary site of the work, the offeror shall request a determination from the Contracting Officer.

(b)(1) If the wage determination provided by the Government for work at the primary site of the work is not applicable to the secondary site of the work, the offeror shall request a wage determination from the Contracting Officer.

(2) The due date for receipt of offers will not be extended as a result of an offeror's request for a wage determination for a secondary site of the work.

2.2 52.225-10 NOTICE OF BUY AMERICAN REQUIREMENT—CONSTRUCTION MATERIALS (MAY 2014)

(a) Definitions. "Commercially available off-the-shelf (COTS) item," "construction material," "domestic construction material," and "foreign construction material," as used in this provision, are defined in the clause of this solicitation entitled "Buy American—Construction Materials" (Federal Acquisition Regulation (FAR) clause 52.225-9).

(b) Requests for determinations of inapplicability. An offeror requesting a determination regarding the inapplicability of the Buy American statute should submit the request to the Contracting Officer in time to allow a determination before submission of offers. The offeror shall include the information and applicable supporting data required by paragraphs (c) and (d) of the clause at FAR 52.225-9 in the request. If an offeror has not requested a determination regarding the inapplicability of the Buy American statute before submitting its offer, or has not received a response to a previous request, the offeror shall include the information and supporting data in the offer.

(c) Evaluation of offers.

(1) The Government will evaluate an offer requesting exception to the requirements of the Buy American statute, based on claimed unreasonable cost of domestic construction material, by adding to the offered price the appropriate percentage of the cost of such foreign construction material, as specified in paragraph (b)(3)(i) of the clause at FAR 52.225-9.

(2) If evaluation results in a tie between an offeror that requested the substitution of foreign construction material based on unreasonable cost and an offeror that did not request an exception, the Contracting Officer will award to the offeror that did not request an exception based on unreasonable cost.

(d) Alternate offers.

(1) When an offer includes foreign solicitation in paragraph (b)(2) of the clause at FAR 52.225-9, the offeror also may submit an alternate offer based on use of equivalent domestic construction material.

(2) If an alternate offer is submitted, the offeror shall submit a separate Standard Form 1442 for the alternate offer, and a separate price comparison table prepared in accordance with paragraphs (c) and (d) of the clause at FAR 52.225-9 for the offer that is based on the use of any foreign construction material for which the Government has not yet determined an exception applies.

(3) If the Government determines that a particular exception requested in accordance with paragraph (c) of the clause at FAR 52.225-9 does not apply, the Government will evaluate only those offers based on use of the equivalent domestic construction material, and the offeror shall be required to furnish such domestic construction material. An offer based on use of the foreign construction material for which an exception was requested—

(i) Will be rejected as nonresponsive if this acquisition is conducted by sealed bidding; or

(ii) May be accepted if revised during negotiations.

2.3 52.236-27 SITE VISIT (CONSTRUCTION) (FEB 1995) ALTERNATE I (FEB 1995)

(a) The clauses at 52.236-2, Differing Site Conditions, and 52.236-3, Site Investigations and Conditions Affecting the Work, will be included in any contract awarded as a result of this solicitation. Accordingly, offerors or quoters are urged and expected to inspect the site where the work will be performed.

2.4 52.228-13 ALTERNATIVE PAYMENT PROTECTIONS (JULY 2000)

(a) The Contractor shall submit one of the following payment protections:

1. Irrevocable letter of credit, or 2. payment bond

(b) The amount of the payment protection shall be 100 percent of the contract price.

(c) The submission of the payment protection is required within 10 days of contract award.

(d) The payment protection shall provide protection for the full contract performance period plus a one-year period.

(e) Except for escrow agreements and payment bonds, which provide their own protection procedures, the Contracting Officer is authorized to access funds under the payment protection when it has been alleged in writing by a supplier of labor or material that a nonpayment has occurred, and to withhold such funds pending resolution by administrative or judicial proceedings or mutual agreement of the parties.

(f) When a tripartite escrow agreement is used, the Contractor shall utilize only suppliers of labor and material that signed the escrow agreement.
