2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This is a request for a brand-name procurement using a firm fixed priced contract for new Nevin dental cabinetry and the relocation of existing Nevin dental furniture. Existing equipment and cabinetry will be moved from the current Tulsa Dental Clinic to the new Tulsa Health Care Center (THCC) on or around February 15, 2021. The new, additional cabinetry/equipment will be built in accordance with our new needs and existing cabinetry in mind. The cabinetry/equipment is used in the Dental Lab to create and modify dental appliances for patients. Specific Nevin Dental Lab cabinetry items such as sinks, suction, air, gas, outlets and special connections for different types of lab equipment are required. Estimated total value of $154,422.97.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: There is a need for cabinetry /equipment that is designed specifically for the new dental lab and will include some pieces of the current Nevin cabinetry/equipment along with additional new pieces of cabinetry. For this reason, new pieces of cabinetry must match the existing pieces of cabinetry that will be used in the new laboratory. The chosen vendor must be authorized to move specific pieces of the current Nevin lab to the new THCC and must be authorized to provide and install new Nevin cabinetry/equipment around the existing cabinetry. The vendor must be an authorized distributor who is able to provide the cabinetry and perform this work.
