Late quotes or documentation will not be accepted. All questions, comments, or inquiries are to be submitted in writing to the Point of Contact, referenced above, prior to the closing of the solicitation.
I. Basis of Award
The Government intends to award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation meets or exceeds the requirement at the lowest price, Lowest Price Technically Acceptable (LPTA). The quotes will be evaluated based on the evaluation factors listed below. The technical acceptability will be evaluated on an "acceptable" or "unacceptable" basis. The lowest price contractor will first be evaluated. If determined to be technically “unacceptable,” the next lowest price response will be evaluated, and so on, until an offer is determined to be “acceptable.” The following areas will be evaluated:

EVALUATION FACTOR CRITERIA:
1. TECHNICAL-Contractor Shall address each technical evaluation criteria sub category, a – i for their respective quote to be technically acceptable. Those offerors who fail to address each sub category will not be eligible to receive the award.
a. Ceiling lifts must have a continuous charge rail that charges the lift anywhere in the room and must have been successfully proven to work with existing supports which are Guldmann GH2 ceiling mounts and wall mounts; otherwise, new supports must be included in the quote. Ceiling lifts that are to be placed on existing Guldmann GH2 ceiling lift rail must be proven to charge and be safe and functional on those rails;
otherwise, new rail must be included in the quote. Ceiling lift rail designs must avoid blocking the function of privacy curtains.
b. Ceiling lifts must have a double positioning lock feature where specified, and on those specified lifts, the motion of the traverse or lift on the traverse must be able to be stopped independently.
c. Ceiling lifts must comply with ISO 10535 and UL standards and be suitable for heavy, repeated use as found in an intensive care, therapy or radiology environment, including repeated repositioning operations such as one 20-second-long heavy lift per minute for 4 minutes.
d. Ceiling lifts must have weight capacities and scales as specified in the Statement of Work pages 4-28, with compact 2-point hanger bars that are quickly removable without tools. Rubber flaps are not acceptable. . Scales must display weight in pounds or kilograms on the hand control and must not reduce lifting height, and the hand control must stay connected to the lift.
e. Ceiling lifts must be compatible with and warranted for use with other manufacturers’ loop slings that are suitable for 2-point hanger bars, including the full range of Guldmann lifting, therapy, and repositioning slings, Alpha Modalities repositioning and lifting slings, Barton car slings, Medco ambulation slings, Horcher loop-based lifting and repositioning slings, Invacare lifting slings, HoverTech HoverSlings, and EZ-Way lifting and ambulation slings.
f. Ceiling lifts must be able to lower patients easily in an emergency without the use of a ladder or tools, with a sustained pull on a red cord.
g. Ceiling lifts must allow sleep in inpatient rooms, including no warning beeps or flashing lights during normal operation or when there are battery problems.
h. Quote must have a minimum of a 3 year warranties on tracks, installation, motors, and all newly installed parts and must cover labor, shipping, and travel. This warranty service is expected to be generally completed onsite and must not require facility staff to ship a lift back to the vendor.
i. Vendor must describe how they will ensure minimal downtime when support and parts are needed during the warranty period and how they will provide education for users and biomedical engineering staff.
2. PAST EXPERIENCE:
Contractors shall provide a minimum of three (3) references indicating same/ similar services/projects for similar installations, including room-coverage systems mounted both by wall and by ceiling, curtain crossing, and connections between rooms., Contractor shall also list number of years’ experience with installation onto Guldmann continuous-charge rails and supports.
3. PRICE - Contractor shall complete B.4 Price/Cost Schedule. The schedule may be altered, or an alternate schedule used where appropriate in order to express quote details.
The Government intends to award the contract resulting from this solicitation without discussions. However, the Government reserves the right to conduct discussions if it determines discussions to be in its best interest.
Statement of Work: Ceiling lift Replacements in Dallas VAMC and Bonham VA
A. General Information
1. Title of Project: Ceiling lift Replacements in Dallas VAMC and Sam Rayburn Memorial Veterans Center
2. Scope of Work: The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. 550-pound, 770-pound, or 825-pound advanced ceiling lifts as specified (with fast motion, continuous-charge room-covering rail systems, and tool-free emergency lowering, plus scales where specified) are to be delivered, installed, and tested, in up to 309 places as listed in the location list. In each case, functioning lifts will be traded in (and there will be 6 extra traded-in motors because the GH2 Twin lifts had two lifts each). Any discount associated with trade-in must be noted on the bid. Deinstallation and disposal of old lifts and unneeded rail and hardware is the contractor’s responsibility. All work must be coordinated with COR and Supply Chain Management Services in order to coordinate paperwork, and all new and decommissioned lifts must be taken to PPM in the Warehouse (Bldg 44 in Dallas; bldg.
10 in Bonham) for inventory record/sticker purposes. In large rooms to be specified below, new continuous-charge rail will be required and will need to attach to existing structural supports. Some of those rooms, as specified, will require a redesign to accommodate curtains and door widths (likely to mean the traveling rail is the longer rail). Locations are in Dallas VAMC and Sam Rayburn Memorial Veterans Center (Bonham). Installation/replacement will need to be timed according to availability of rooms. Details are provided in sections C and D below, Specific Mandatory Tasks and
Associated Deliverables. If proposed cost exceeds budget, VA will prioritize and specify final lift replacement locations.
3. Background: Ceiling lifts have been installed in many places throughout the VA North Texas Health Care System. The Arjo Maxi Sky lifts in SCI Outpatient Clinic are currently the oldest in the building, from 2006, and are breaking down often enough to warrant replacement. The GH2 lifts installed in 2008 and 2009 will be unsupported by the manufacturer as of 2020, so we are starting the process of replacing them. This first set of upgrades will include only systems that do not have continuous charge, so that we solve a problem of usability caused by the older charging stations. These systems integrate into an existing system in which mostly Guldmann ceiling lifts (with ceiling lifts from 3 other manufacturers and floor lifts from 7 manufacturers scattered throughout the facility) are used to lift, transfer, and reposition inpatients and outpatients. Patients may arrive at any of these locations on a sling or be sent on to another location on a sling applied at these locations. The systems being installed here are being installed in areas that already contain similar equipment. They may also receive patients wearing assorted brands of slings from home or may need to use slings from Barton/Humancare, Guldmann, and/or Alpha Modalities to lift patients and send them to other departments.
Staff from other parts of the facility participate in transfers in Radiology, so compatibility and similar operation are important.
4. Performance Period/Schedule: The contractor shall start the work required under this SOW by about October 2019 and coordinate installation with room availability unless otherwise directed by the Contracting Officer (CO). It is anticipated that this quantity of lift replacements may require up to a year to complete. If the contractor proposes an earlier completion date, and the Government accepts the contractor’s proposal, the contractor’s proposed completion date shall prevail. Installation dates and designs must be coordinated with Contracting Officer Representative (COR), and dates for installation in remodeling projects may be adjusted by the Government as required if engineering changes are required and delayed.
5. Type of Contract: Firm-Fixed-Price B. Contract Award Meeting
1. The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived.
C. General Requirements
1. All installation must comply with requirements of Patient Safety Alert AL14-07, including the Checklist for Installation and Relocation. The installation checklist must be filled out for each installed lift, in addition to any checklist required by the manufacturer, and supplied to the COR.

2. Contractor will provide evaluation of existing structure where ceiling lifts are needed and submission of plans to VA North Texas Healthcare System Engineering service, to assure that workable systems are installed and comply with VA design requirements and relevant standards.
3. Contractor will coordinate with Engineering service to verify access to engineering mechanical, HVAC, and fire systems components within the mounting area of the lift units.
4. Contractor will coordinate with Contracting Officer Representative (COR) to schedule installations.
5. Contractor will provide information on minimum clearances for operation of each lift, to be presented with the bid.
6. Contractor will provide a list of all construction or modifications required to install these lifts, to be presented with the bid. The goal is to install around any obstructions without requiring VA to make construction modifications.
7. Contractor will provide all qualified personnel, materials, equipment and services necessary to install ceiling lifts for the rooms described below under Location, including lifts, tracks, spreader bars, attachment hardware, and all other equipment necessary for safe operation. Detailed requirements are described in the Standards and Special Requirements sections. Lift systems may be configured in any way that covers the rooms best and meets the requirements.
8. Contractor will provide installation and testing of all equipment for ceiling lifts, according to VA checklist requirements, resulting in a turnkey system.
9. Contractor will provide training on equipment use for all new types of equipment included in this contract, one session per shift for up to 2 shifts of personnel in each affected area and for onsite trainers (up to 10 sessions scheduled after installation and before use), in coordination with COR.
10. Contractor will provide evidence of testing of all lift systems provided and schedule testing when it can be overseen by COR.
11. Contractor will mitigate any new hazards created by installation, such as ramps over interstitial support beams that create trip hazards.
12. Contractor will meet construction safety requirements, including construction orientation, safety training requirements, dust control requirements from the Infection Control Risk Assessment, TB testing, electrical safety requirements, and Above Ceiling Permit requirements including re-sealing any penetrations of fire barriers.
D. NARA Records Management Language Clause
1. Citations to pertinent laws, codes and regulations such as 44 U.S.C Chapter 21 , 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.
2. Contractor shall treat all deliverables under the contract as the property of the U.S.
Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.
3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government ‘IT’ equipment and/or Government records.
4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.
5. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract.
6. The Government Agency owns the rights to all data/records produced as part of this contract.
7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.
8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.].
9. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.
10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under or relating to this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
