Vendor Questions and Response:

· Is the intent for the patient lift to be under the canopy? No

· Is the patient lift to be facing the building or away from the building? Facing the building

· What will be the orientation of the Mobile Unit on the pad? Parallel to the building

· Can the facility support the (480 Volt A.C., 3 Phase, 125 KVA, fused at 150 Amps) system requirements? See amendment to RFQ which added the Utility infra-structure

· Has the facility completed all siting requirements to accommodate the parking of the trailer? Yes

· SPACE, TWIST, BLADE is Siemens software. Assuming VA desires GE CUBE, TRICKS, and PROPELLER. Please advise. Equivalent

· If providing 16 channel Flex Coils can we check “Comply” on the 4 channel shoulder, 8 channel foot/ankle/knee coil, and 8 channel wrist? yes

· Please clarify “response within 1 hour” for 24/7 telephone technical support. Does this mean a returned phone call within the hour? yes

· Can contractor provide alternative equipment for 6 months until specified equipment is available? No... prefer finished Mobile

· Please clarify the reasoning for including “52.232-31 Invitation to Propose Financing Terms” as this is for a lease and not a purchase. Clause is not relevant…please disregard 52.232-31.

· Does the site have a pad and/or power? Yes

· What is the expected date for awarding the contract? (allow approx. 4 weeks for pad construction and power) The expected date of award is January 18, 2019 with delivery and set-up for February 1, 2019. However, the dates are subject to changed once an award is made.

· 3D HEART is used for Coronary Artery imaging. Is 3D HEART required (pg.15)? No

· Would the Siemens Aera, or Philips Ingenia be considered as substitutes for Voyager? Yes…as long as the unit meet the salient characteristics.

· How many days of applications training are needed (pg. 18)? Week on site and week at the training center with optional follow up as needed.

· Does the cboc currently have imaging services? What unit is in use? Phillips Xrays. Soon to be active Toshiba CT Scanner

· 1 year warranty is mentioned (pg.18), will a service contract be needed for the remaining term? No…the Vendor is responsible for the maintenance of the unit, including preventative maintenance, parts and labor.

· MAGiC suite (pg.15) can include (brain, and/or prostate) which is required? Brain

· BREASE (Breast Spectroscopy)/ViosWorks (pg. 15,16) are unusual requirements for a clinical setting, are these applications required? No

· TMJ coils (pg.16) are not compatible with the voyager system. Ok…noted.

· CVWorks is part of our standard GE Voyager configuration and includes basic cardiac sequences. Are additional CMR options required (pg.15)? (Cardio package, Cardio plus, MRI echo) No

· Our standard configuration for a GE Voyager includes 3 (S/M/L) high end 16ch flex coils for MSK imaging. These 16ch flex coils are used for clinical imaging of the shoulder, elbow, wrist, hip, knee, ankle and foot. Would the flex coils with a higher channel count be considered as replacements for the shoulder/knee/ankle/and foot coils? Yes

· We offer system service from (8am – 9pm) is the unit only to be serviced after normal duty hours (after 4:30pm)? (pg.15) Flexible

· Our normal system coil configuration is; 32ch Head and neck unit, 32ch Embedded Posterior Spine Array, 16ch Anterior Array, MSK Flex Coil Suite (small, medium, large) would any additional coils be necessary? Must meet the minimum stated in the RFQ.

· Is the location of the Place of Performance a different location than the primary CBOC, 600 N Main, Mt Vernon, MO 65712? I know the address in the SF1449, sect. 15 is different but you ref by name the CBOC and I cannot find a facility located at 1800 W. Republic St. all databases point to 600 N Main. The clinic is still named Gene Taylor Community Based Outpatient Clinic 1850 W Republic Rd., Springfield, MO 65810

· In Section 2.1 of the work statement, it says that the contractor is responsible for site construction (to include utility hook-ups).

· Does this site have an existing pad and power in place to accommodate the mobile MR unit? Yes

· If so, has a mobile MR of any mobile imaging system been operational at this location previously? No

· Have you previously used a Mobile MRI at the proposed location? No

· You show you pad is 10’ X43’. Our mobile trailer is 48’ long. It looks like it will still fit. Can you verify that a 48’ Calumet trailer will fit? Verified…the trailer will fit

· Will you require an awning to be built from the MRI system to the facility entrance? If so, how many feet long, wide, and tall should the awning be? Will you build the awning or is this the responsibility of the contractor? Please see amend to RFQ…contractor required to provide an awning long enough to cover entry

· Is there any additional construction required on-site and if so who is responsible for the construction? No

· Do you have dedicated power requirement of 480 volt 3 Phase Y 200 Amp with a Russellstoll Female Connector Model RUSSELL STOLL # DF2504FRAB (wall mounted) OR DF2504FP (cable end). We will also require data cables run to the mobile MRI so we connect to your PACS system and VOIP or phone lines if there is no VOIP? See amendment 0001 to RFQ…utility infra-structure added to PWS.

· How many days of applications training do you require? See amendment 0001 to RFQ…one week onsite, one week offsite and as needed.

· How many MRI technologists will be trained to operate the system? See amendment 0001 to RFQ…three Government personnel

· What MRI systems are your current MRI Technologists trained to operate? Phillips 1.5T close magnet and Hitachi 1.2T open magnet

· Does the Gene Taylor Community Based Outpatient clinic currently served by an MRI Onsite? And if so, what kind of system is currently deployed? No

· Will the government accept an interim MRI unit of similar capabilities until a new unit can be deployed? An “interim” unit will be evaluated. However the requirement calls for a fully functioning and fully operable unit.

· Are proposed technicians currently trained on a unit and what kind of scanner are they currently familiar with? Technicians with the Fayetteville VAMC are currently trained and scanning on a 1.5T close Phillip and 12T open Hitachi MRI Scanner.

· Do technicians currently have a reading solution workstation in their location or will they require additional workstations? Not at the CBOC site. Technician will read the MRIs remotely from the Fayetteville VAMC on the Carestream PACS.

· Is the Gene Taylor Community Based Outpatient Clinic already set up on the VA side with power and plug to accept a mobile unit? Yes.

· Will the VA provide at least two IT data ports? Yes

· Are they willing to allow a temporary interim unit for a few months until a new unit can be delivered approx.. 3-4 months from the award date? The VA prefer the final unit.

· Are there pictures of the proposed parking location available to prospective contractors? Yes. The RFQ includes site plans and a site visit is scheduled for 12/13/2018 @ 10am.

· Would the facility like to have any logo graphics installed during the lease period to identify the facility or modality? No.
