GENERAL REQUIREMENTS

SECTION 01 00 00
GENERAL REQUIREMENTS
1.1 SAFETY REQUIREMENTS
Refer to SAFETY REQUIREMENTS for safety and infection control requirements.
1.2 GENERAL INTENTION
A. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for the replacement of mechanical and electrical systems as required by drawings and specifications.
B. Visits to the site by Bidders may be made only by appointment with the
Contracting Officer’s Representative (COR).
C. Before placement and installation of work subject to tests by testing laboratory retained by Department of Veterans Affairs, the Contractor shall notify the COR in sufficient time to enable testing laboratory personnel to be present at the site in time for proper taking and testing of specimens and field inspection. Such prior notice shall be not less than three work days unless otherwise designated by the COR.
D. All employees of general contractor and subcontractors shall comply with VA security management program. The general contractor and subcontractors will be required to attend or review hospital specific contractor training prior to starting work. Contractor badges issued at the completion of this training are required to be worn at all times.
The general contractor will be responsible for the return of ALL BADGES at the completion of the project. The general contractor and subcontractors are prohibited from accessing areas of the hospital not required to perform the work of this project.
1.3 STATEMENT OF BID ITEM(S)
A. BID ITEM I, BASIC BID: Replace Mechanical, Electrical and Plumbing: Work includes general construction and mechanical and electrical alterations, necessary removal of existing structures and construction and certain other items as shown on the plans and specifications. Base bid includes all drawings and specifications. The period of performance is 458 days.
B. BID ITEM 2, BID DEDUCT NO.1: Eliminate steam line work shown on drawing sheets 01-M01, 01-M02, 01-M03, 01-M04, 01-M05, 01-M06, 01-M07, 01-M08 & 01-M09. The period of performance is 458 days.
C. BID ITEM 3, BID DEDUCT NO. 2: Eliminate ACA Electrical work shown on drawing sheets 01-M01, 01-M02, 01-M03, 01-M04, 01-M05, 01-M06, 01-M07, 01-M08, 01-M09, 32- E10, 32-E11, 32-E12, 32-E20, 32-E30, 32-E40, 32-E50, 32-E60, 33-E01, 33-E02, 33- E11, 33-E12, 33E21, 33-E22, 33-E31, 34-E01. The period of performance is 408 days.

Note: The chiller plant work that creates interruption of services shall be scheduled between November and February or other times approved by the COR.
1.4 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR
A. Drawings and contract documents may be obtained from the website where the solicitation is posted.
1.5 CONSTRUCTION SECURITY REQUIREMENTS
A. Security Plan:
1. The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.
2. The General Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations.
B. Security Procedures:
1. General Contractor’s employees shall not enter the project site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site.
2. Before starting work the General Contractor shall give one week’s notice to the COR so that security training can be provided for the employees. This notice is separate from any notices required for utility shutdown described later in this section.
3. No photography of VA premises is allowed without written permission of the Contracting Officer.
4. VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of the Contracting Officer.
C. Key Control:
1. The General Contractor shall provide duplicate keys and lock combinations to the COR for the purpose of security inspections of every area of project including tool boxes and parked machines and take any emergency action.
2. The General Contractor shall turn over all permanent lock cylinders to the VA locksmith for permanent installation.
D. Document Control:
1. Before starting any work, the General Contractor/Sub Contractors shall submit an electronic security memorandum describing the
01 00 00 -3 approach to following goals and maintaining confidentiality of
“sensitive information”.
2. The General Contractor is responsible for safekeeping of all drawings, project manual and other project information. This information shall be shared only with those with a specific need to accomplish the project.
3. Certain documents, sketches, videos or photographs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”.
Secure such information in separate containers and limit the access to only those who will need it for the project. Return the information to the Contracting Officer upon request.
4. These security documents shall not be removed or transmitted from the project site without the written approval of Contracting
Officer.
5. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.
6. Notify Contracting Officer and Site Security Officer immediately when there is a loss or compromise of “sensitive information”.
7. All electronic information shall be stored in specified location following VA standards and procedures using an Engineering Document
Management Software (EDMS).
a. Security, access and maintenance of all project drawings, both scanned and electronic shall be performed and tracked through the
EDMS system.
b. “Sensitive information” including drawings and other documents may be attached to e-mail provided all VA encryption procedures are followed.
E. Motor Vehicle Restrictions
1. Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitted 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.
2. Two (2) permits shall be issued for General Contractor and its employees for parking in designated areas only.
1.6 OPERATIONS AND STORAGE AREAS
A. The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by
01 00 00 -4 the Contracting Officer. The Contractor shall hold and save the
Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.
B. The Contractor shall, under regulations prescribed by the Contracting
Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting
Officer. When materials are transported in prosecuting the work, vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer of the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Contractor shall protect them from damage. The
Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.
C. Working space and space available for storing materials shall be as shown on the drawings.
D. Workmen are subject to rules of the Medical Center applicable to their conduct. Execute work in such a manner as to interfere as little as possible with work being done by others. Keep roads clear of construction materials, debris, standing construction equipment and vehicles at all times.
E. Execute work so as to interfere as little as possible with normal functioning of the Medical Center as a whole, including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel, EXCEPT AS
PERMITTED BY COR where required by limited working space.
1. Do not store materials and equipment in other than assigned areas.
2. Schedule delivery of materials and equipment to immediate construction working areas within buildings in use by Department of
Veterans Affairs in quantities sufficient for not more than two work days. Provide unobstructed access to Medical Center areas required to remain in operation.
3. Where access by Medical Center personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and safety requirements.

F. Utilities Services: Where necessary to cut exist ing pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems (excep t telephone), they shall be cut and capped at suitable places where sh own; or, in absence of such indication, where directed by COR. All such actions shall be coordinated with the COR or Utility Company involve d.
G. Phasing:
1. The Medical Center must maintain its operation 2 4 hours a day, 7 days a week. Therefore, any interruption in servic e must be scheduled and coordinated with the COR to ensure th at no lapses in operation occur. It is the CONTRACTOR'S responsibi lity to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, t he interim life safety measure to be used during the work, and a sc hedule defining the duration of the work with milestone subtasks. The work to be outlined shall include, but not be limited to:

To insure such executions, Contractor shall furn ish the COR with a schedule of approximate phasing, with dates, on whi ch the Contractor intends to accomplish work in each specific area of site, building or portion thereof. In addition, Contractor shall n otify the COR two weeks in advance of the proposed date of starti ng work in each specific area of site, building or portion thereof. Arrange such phasing to insure accomplishment of this work in su ccessive phases agreeable to the COR.
3. Contractor shall take all measures and provide a ll material necessary for protecting existing equipment and pro perty in affected areas of construction against dust and debris, so t hat equipment and affected areas to be used in the Medical Centers op erations will not be hindered. Contractor shall permit access to Depa rtment of
Veterans Affairs personnel and patients through oth er construction areas which serve as routes of access to such affec ted areas and equipment. These routes whether access or egress s hall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc to facilitate patient and st aff access.
Coordinate alteration work in areas occupied by Dep artment of
Veterans Affairs so that Medical Center operations will continue during the construction period.

H. Construction Fence: Before construction operatio ns begin, Contractor shall provide a chain link construction fence, 2.1m (seven feet) minimum height, around the construction area indica ted on the drawings.
Provide gates as required for access with necessary hardware, including hasps and padlocks. Fasten fence fabric to terminal posts with tension bands and to line posts and top and bottom rails wi th tie wires spaced at maximum 375mm (15 inches). Bottom of fences shal l extend to 25mm
(one inch) above grade. Remove the fence when direc ted by the COR.
I. Utilities Services: Maintain existing utility se rvices for Medical
Center at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uni nterrupted services.
Where necessary to cut existing water, steam, gases , sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (incl uding telephone), they shall be cut and capped at suitable places whe re shown; or, in absence of such indication, where directed by COR.
1. No utility service such as water, gas, steam, se wers or electricity, or fire protection systems and communications syste ms may be interrupted without prior approval of the COR. Elec trical work shall be accomplished with all affected circuits or equip ment de-energized. When an electrical outage cannot be acco mplished, work on any energized circuits or equipment shall not comme nce without a detailed work plan, the Medical Center Director’s p rior knowledge and written approval. Refer to specification Sectio ns 26 05 11, REQUIREMENTS FOR ELECTRICAL INSTALLATIONS for addit ional requirements.
2. Contractor shall submit a request to interrupt a ny such services to
COR, in writing, 7 days in advance of proposed inte rruption. Request shall state reason, date, exact time of, and approx imate duration of such interruption.
3. Contractor will be advised (in writing) of appro val of request, or of which other date and/or time such interruption w ill cause least inconvenience to operations of the Medical Center. Interruption time approved by Medical Center may occur at other than Contractor's normal working hours.

4. Major interruptions of any system must be reques ted, in writing, at least 15 calendar days prior to the desired time an d shall be performed as directed by the COR.
5. In case of a contract construction emergency, se rvice will be interrupted on approval of COR. Such approval will be confirmed in writing as soon as practical.
6. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the c onstruction project, for such items as water, sewer, electricit y, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.
J. Abandoned Lines: All service lines such as wires , cables, conduits, ducts, pipes and the like, and their hangers or sup ports, which are to be abandoned but are not required to be entirely re moved, shall be sealed, capped or plugged at the main, branch or pa nel they originate from. The lines shall not be capped in finished are as, but shall be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or par titions; so that they are completely behind the finished surfaces.
K. To minimize interference of construction activit ies with flow of
Medical Center traffic, comply with the following:
1. Keep roads, walks and entrances to grounds, to p arking and to occupied areas of buildings clear of construction m aterials, debris and standing construction equipment and vehicles. W herever excavation for new utility lines cross existing roa ds, at least one lane must be open to traffic at all times with appr oval.
2. Method and scheduling of required cutting, alter ing and removal of existing roads, walks and entrances must be approve d by the COR.
L. Coordinate the work for this contract with other construction operations as directed by COR. This includes the sc heduling of traffic and the use of roadways, as specified in Article, U SE OF ROADWAYS.
1.7 ALTERATIONS
A. Survey: Before any work is started, the Contract or shall make a thorough survey with the COR in areas of buildings in which alterations occur and areas which are anticipated routes of acc ess, and furnish a report, signed by both to the Contracting Officer. This report shall list by rooms and spaces:

1. Existing condition and types of resilient floori ng, doors, windows, walls and other surfaces not required to be altered throughout affected areas of building.
2. Existence and conditions of items such as plumbi ng fixtures and accessories, electrical fixtures, equipment, veneti an blinds, shades, etc., required by drawings to be either reu sed or relocated, or both.
3. Shall note any discrepancies between drawings an d existing conditions at site.
4. Shall designate areas for working space, materia ls storage and routes of access to areas within buildings where al terations occur and which have been agreed upon by Contractor and C OR.
B. Any items required by drawings to be either reus ed or relocated or both, found during this survey to be nonexistent, o r in opinion of COR, to be in such condition that their use is impossibl e or impractical, shall be furnished and/or replaced by Contractor wi th new items in accordance with specifications which will be furnis hed by Government.
Provided the contract work is changed by reason of this subparagraph B, the contract will be modified accordingly, under pr ovisions of clause entitled "DIFFERING SITE CONDITIONS" (FAR 52.236-2) and "CHANGES" (FAR
52.243-4 and VAAR 852.236-88).
C. Re-Survey: Thirty days before expected partial o r final inspection date, the Contractor and COR together shall make a thorough re-survey of the areas of buildings involved. They shall furn ish a report on conditions then existing, of resilient flooring, do ors, windows, walls and other surfaces as compared with conditions of s ame as noted in first condition survey report:
1. Re-survey report shall also list any damage caus ed by Contractor to such flooring and other surfaces, despite protectio n measures; and, will form basis for determining extent of repair wo rk required of
Contractor to restore damage caused by Contractor's workmen in executing work of this contract.
D. Protection: Provide the following protective mea sures:
1. Wherever existing roof surfaces are disturbed th ey shall be protected against water infiltration. In case of le aks, they shall be repaired immediately upon discovery.

2. Temporary protection against damage for portions of existing structures and grounds where work is to be done, ma terials handled and equipment moved and/or relocated.
3. Protection of interior of existing structures at all times, from damage, dust and weather inclemency. Wherever work is performed, floor surfaces that are to remain in place shall be adequately protected prior to starting work, and this protecti on shall be maintained intact until all work in the area is com pleted.
1.8 DISPOSAL AND RETENTION
A. Materials and equipment accruing from work remov ed and from demolition of buildings or structures, or parts thereof, shall be disposed of as follows:
1. Reserved items which are to remain property of t he Government are identified by attached tags or noted on drawings or in specifications as items to be stored. Items that re main property of the Government shall be removed or dislodged from p resent locations in such a manner as to prevent damage which would b e detrimental to re-installation and reuse. Store such items where d irected by COR.
2. Items not reserved shall become property of the Contractor and be removed by Contractor from the Medical Center.
3. Items of portable equipment and furnishings loca ted in rooms and spaces in which work is to be done under this contr act shall remain the property of the Government. When rooms and spac es are vacated by the Department of Veterans Affairs during the alter ation period, such items which are NOT required by drawings and s pecifications to be either relocated or reused will be removed by th e Government in advance of work to avoid interfering with Contracto r's operation.
1.9 PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND
IMPROVEMENTS
A. The Contractor shall preserve and protect all st ructures, equipment, and vegetation (such as trees, shrubs, and grass) o n or adjacent to the work site, which are not to be removed and which do not unreasonably interfere with the work required under this contrac t.
B. The Contractor shall protect from damage all exi sting improvements and utilities at or near the work site and on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any dam age to those facilities, including those that are the property o f a third party, 01 00 00 -10 resulting from failure to comply with the requireme nts of this contract or failure to exercise reasonable care in performin g the work. If the
Contractor fails or refuses to repair the damage pr omptly, the
Contracting Officer may have the necessary work per formed and charge the cost to the Contractor.
1.10 RESTORATION
A. Remove, cut, alter, replace, patch and repair ex isting work as necessary to install new work. Except as otherwise shown or specified, do not cut, alter or remove any structural work, an d do not disturb any ducts, plumbing, steam, gas, or electric work witho ut approval of the
COR. Existing work to be altered or extended and th at is found to be defective in any way, shall be reported to the COR before it is disturbed. Materials and workmanship used in restor ing work, shall conform in type and quality to that of original exi sting construction, except as otherwise shown or specified.
B. Upon completion of contract, deliver work comple te and undamaged.
Existing work (walls, ceilings, partitions, floors, mechanical and electrical work, lawns, paving, roads, walks, etc.) disturbed or removed as a result of performing required new work , shall be patched, repaired, reinstalled, or replaced with new work, a nd refinished and left in as good condition as existed before commenc ing work.
C. At Contractor's own expense, Contractor shall im mediately restore to service and repair any damage caused by Contractor' s workmen to existing piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications sy stems (including telephone) which are not scheduled for discontinuan ce or abandonment.
D. Expense of repairs to such utilities and systems not shown on drawings or locations of which are unknown will be covered b y adjustment to contract time and price in accordance with clause e ntitled "CHANGES"
(FAR 52.243-4 and VAAR 852.236-88) and "DIFFERING S ITE CONDITIONS" (FAR
52.236-2).
1.11 LAYOUT OF WORK
A. The Contractor shall lay out the work from Gover nment established base lines and bench marks, indicated on the drawings, a nd shall be responsible for all measurements in connection with the layout. The
Contractor shall furnish, at Contractor's own expen se, all stakes, templates, platforms, equipment, tools, materials, and labor required
01 00 00 -11 to lay out any part of the work. The Contractor sha ll be responsible for executing the work to the lines and grades that may be established or indicated by the Contracting Officer. The Contra ctor shall also be responsible for maintaining and preserving all stak es and other marks established by the Contracting Officer until author ized to remove them.
If such marks are destroyed by the Contractor or th rough Contractor's negligence before their removal is authorized, the Contracting Officer may replace them and deduct the expense of the repl acement from any amounts due or to become due to the Contractor.
1.12 AS-BUILT DRAWINGS
A. The contractor shall maintain two full size sets of as-built drawings which will be kept current during construction of t he project, to include all contract changes, modifications and cla rifications.
B. All variations shall be shown in the same genera l detail as used in the contract drawings. To insure compliance, as-built d rawings shall be made available for the COR review, as often as requ ested.
C. Contractor shall deliver two approved completed sets of as-built drawings in the electronic version (scanned PDF) to the COR within 15 calendar days after each completed phase and after the acceptance of the project by the COR.
D. Paragraphs A, B, & C shall also apply to all sho p drawings.
1.13 USE OF ROADWAYS
A. For hauling, use only established public roads a nd roads on the Medical
Center property and, when authorized by the COR, su ch temporary roads which are necessary in the performance of contract work. Temporary roads shall be constructed and restoration performe d by the Contractor at Contractor's expense. When necessary to cross cu rbing, sidewalks, or similar construction, they must be protected by wel l-constructed bridges.
1.14 TEMPORARY USE OF MECHANICAL AND ELECTRICAL EQUIPMENT
A. Use of new installed mechanical and electrical e quipment to provide heat, ventilation, plumbing, light and power will b e permitted subject to written approval and compliance with the followi ng provisions:
1. Permission to use each unit or system must be gi ven by COR in writing. If the equipment is not installed and main tained in accordance with the written agreement and following provisions, the
COR will withdraw permission for use of the equipme nt.

2. Electrical installations used by the equipment s hall be completed in accordance with the drawings and specifications to prevent damage to the equipment and the electrical systems, i.e. tran sformers, relays, circuit breakers, fuses, conductors, motor controll ers and their overload elements shall be properly sized, coordina ted and adjusted.
Installation of temporary electrical equipment or d evices shall be in accordance with NFPA 70, National Electrical Cod e, (2014
Edition), Article 590, Temporary Installations. Voltage supplied to each item of equipment shall be verified to be corr ect and it shall be determined that motors are not overloaded. The e lectrical equipment shall be thoroughly cleaned before using it and again immediately before final inspection including vacuu m cleaning and wiping clean interior and exterior surfaces.
3. Units shall be properly lubricated, balanced, an d aligned.
Vibrations must be eliminated.
4. Automatic temperature control systems for prehea t coils shall function properly and all safety controls shall fun ction to prevent coil freeze-up damage.
5. The air filtering system utilized shall be that which is designed for the system when complete, and all filter elemen ts shall be replaced at completion of construction and prior to testing and balancing of system.
B. Prior to final inspection, the equipment or part s used which show wear and tear beyond normal, shall be replaced with iden tical replacements, at no additional cost to the Government.
C. This paragraph shall not reduce the requirements of the mechanical and electrical specifications sections.
D. Any damage to the equipment or excessive wear du e to prolonged use will be repaired replaced by the contractor at the contr actor’s expense.
1.15 TEMPORARY USE OF EXISTING SERVICE/FREIGHT ELEVATORS
A. Contractor will be allowed the use of existing s ervice/freight elevators with COR approval.
B. Use of existing elevator elevators for handling building materials and
Contractor's personnel will be permitted subject to following provisions:
1. Contractor makes all arrangements with the COR f or use of elevators.
The COR will ascertain that elevators are in proper condition.

Contractor may use service/freight elevators in the Main Building with COR approval. Contractor personnel shall yield the elevator to patient transport at all times.
2. Contractor covers and provides maximum protectio n of following elevator components:
a. Entrance jambs, heads soffits and threshold plat es.
b. Entrance columns, canopy, return panels and insi de surfaces of car enclosure walls.
c. Finish flooring.
3. Place elevator in condition equal, less normal w ear, to that existing at time it was placed in service of Contra ctor as approved by Contracting Officer.
1.16 AVAILABILITY AND USE OF UTILITY SERVICES
A. The Government shall make all reasonably require d amounts of utilities available to the Contractor from existing outlets a nd supplies, as specified in the contract. The Contractor shall car efully conserve any utilities furnished without charge.
B. Electricity (for Construction and Testing): Furn ish all temporary electric services.
1. Obtain electricity by connecting to the Medical Center electrical distribution system. Electricity for all other uses is available at no cost to the Contractor.
C. Water (for Construction and Testing): Furnish te mporary water service.
1. Obtain water by connecting to the Medical Center water distribution system. Provide reduced pressure backflow preventer at each connection as per code. Water is available at no co st to the
Contractor.
2. Maintain connections, pipe, fittings and fixture s and conserve water-use so none is wasted. Failure to stop leakag e or other wastes will be cause for revocation (at COR discretion) of use of water from Medical Center's system.
1.17 TESTS
A. As per specification section 23 05 93 the contra ctor shall provide a written testing and commissioning plan complete wit h component level, equipment level, sub-system level and system level breakdowns. The plan will provide a schedule and a written sequence of what will be tested, how and what the expected outcome will be. This document will
01 00 00 -14 be submitted for approval prior to commencing work. The contractor shall document the results of the approved plan and submit for approval with the as built documentation.
B. Pre-test mechanical and electrical equipment and systems and make corrections required for proper operation of such s ystems before requesting final tests. Final test will not be cond ucted unless pre-tested.
C. Conduct final tests required in various sections of specifications in presence of an authorized representative of the Con tracting Officer.
Contractor shall furnish all labor, materials, equi pment, instruments, and forms, to conduct and record such tests.
D. Mechanical and electrical systems shall be balan ced, controlled and coordinated. A system is defined as the entire syst em which must be coordinated to work together during normal operatio n to produce results for which the system is designed. For example, air conditioning supply air is only one part of entire system which provide s comfort conditions for a building. Other related components are return air, exhaust air, steam, chilled water, refrigerant, hot water, contr ols and electricity, etc. Another example of a system which involves sev eral components of different disciplines is a boiler installation. Eff icient and acceptable boiler operation depends upon the coordi nation and proper operation of fuel, combustion air, controls, steam, feedwater, condensate and other related components.
E. All related components as defined above shall be functioning when any system component is tested. Tests shall be complete d within a reasonable period of time during which operating an d environmental conditions remain reasonably constant and are typic al of the design conditions.
F. Individual test result of any component, where r equired, will only be accepted when submitted with the test results of re lated components and of the entire system.
1.18 INSTRUCTIONS
A. Contractor shall furnish Maintenance and Operati ng manuals (hard copies and electronic) and verbal instructions when requir ed by the various sections of the specifications and as hereinafter s pecified.
B. Manuals: Maintenance and operating manuals and o ne compact disc (four hard copies and one electronic copy each) for each separate piece of
01 00 00 -15 equipment shall be delivered to the COR coincidenta l with the delivery of the equipment to the job site. Manuals shall be complete, detailed guides for the maintenance and operation of equipme nt. They shall include complete information necessary for starting , adjusting, maintaining in continuous operation for long period s of time and dismantling and reassembling of the complete units and sub-assembly components. Manuals shall include an index covering all component parts clearly cross-referenced to diagrams and illustrati ons. Illustrations shall include "exploded" views showing and identify ing each separate item. Emphasis shall be placed on the use of specia l tools and instruments. The function of each piece of equipmen t, component, accessory and control shall be clearly and thorough ly explained. All necessary precautions for the operation of the equi pment and the reason for each precaution shall be clearly set forth. Man uals must reference the exact model, style and size of the piece of equ ipment and system being furnished. Manuals referencing equipment simi lar to but of a different model, style, and size than that furnishe d will not be accepted.
C. Instructions: Contractor shall provide qualified , factory-trained manufacturers' representatives to give detailed tra ining to assigned
Department of Veterans Affairs personnel in the ope ration and complete maintenance for each piece of equipment. All such t raining will be at the job site. These requirements are more specifica lly detailed in the various technical sections. Instructions for differ ent items of equipment that are component parts of a complete sy stem, shall be given in an integrated, progressive manner. All instructo rs for every piece of component equipment in a system shall be availab le until instructions for all items included in the system h ave been completed.
This is to assure proper instruction in the operati on of inter-related systems. All instruction periods shall be at such t imes as scheduled by the COR and shall be considered concluded only when the COR is satisfied in regard to complete and thorough covera ge. The contractor shall submit a course outline with associated mater ial to the COR for review and approval prior to scheduling training to ensure the subject matter covers the expectations of the VA and the co ntractual requirements. The Department of Veterans Affairs r eserves the right to request the removal of, and substitution for, any i nstructor who, in
01 00 00 -16 the opinion of the COR, does not demonstrate suffic ient qualifications in accordance with requirements for instructors abo ve.
1.19 RELOCATED EQUIPMENT ITEMS
A. Contractor shall disconnect, dismantle as necess ary, remove and reinstall in new location, all existing equipment s hown to be relocated by the Contractor.
B. Perform relocation of such equipment or items at such times and in such a manner as directed by the COR.
C. Suitably cap existing service lines, such as ste am, condensate return, water, drain, gas, air, vacuum and/or electrical, a t the main whenever such lines are disconnected from equipment to be re located. Remove abandoned lines in finished areas and cap as specif ied herein before under paragraph "Abandoned Lines".
D. Provide all mechanical and electrical service co nnections, fittings, fastenings and any other materials necessary for as sembly and installation of relocated equipment; and leave such equipment in proper operating condition.
F. All service lines such as noted above for reloca ted equipment shall be in place at point of relocation ready for use befor e any existing equipment is disconnected. Make relocated existing equipment ready for operation or use immediately after reinstallation.
1.20 CONSTRUCTION SIGN
A. Provide a Construction Sign where directed by th e COR. All wood members shall be of framing lumber. Cover sign frame with 0 .7 mm (24 gage) galvanized sheet steel nailed securely around edges and on all bearings. Provide three 100 by 100 mm (4 inch by 4 inch) posts (or equivalent round posts) set 1200 mm (four feet) int o ground. Set bottom of sign level at 900 mm (three feet) above ground a nd secure to posts with through bolts. Make posts full height of sign. Brace posts with 50 x 100 mm (two by four inch) material as directed.
B. Paint all surfaces of sign and posts two coats o f white gloss paint.
Border and letters shall be of black gloss paint, e xcept project title which shall be blue gloss paint.
C. Maintain sign and remove it when directed by the COR.

1.21 SAFETY SIGN
A. Provide a Safety Sign where directed by COR. Fac e of sign shall be 19 mm (3/4 inch) thick exterior grade plywood. Provide two 100 mm by 100 mm (four by four inch) posts extending full height of sign and 900 mm
(three feet) into ground. Set bottom of sign level at 1200 mm (four feet) above ground.
B. Paint all surfaces of Safety Sign and posts with one prime coat and two coats of white gloss paint. Letters and design shal l be painted with gloss paint of colors noted.
C. Maintain sign and remove it when directed by COR .
D. Standard Detail Drawing Number SD10000-02(Found on VA TIL) of safety sign showing required legend and other characterist ics of sign is ade a part of this specification. E. Post the number of a ccident free days on a daily basis.
1.22 VA SUBMITTAL EXCHANGE

MEDICAL CENTER REQUIREMENTS
1.0 GENERAL INTENTION: This document pertains to station policy for construction projects performed at the Harry S Truman Memorial Veterans' Hospital (HSTMVH) in Columbia, MO. Safety and health concerns are taken seriously at this facility. Both our staff and yours are expected to strictly adhere to the regulations and requirements. This is exceedingly important, since we must be primarily concerned for the safety of our patients. In this regard, OSHA Standards may protect worker safety and health, but they have minimal benefit for protecting the safety and health of our patients, due primarily to their differing medical conditions. Review this information as orientation with your personnel performing work on site. Where the requirements as outlined in this and Section 01 00 00 are differing, the more stringent shall apply.
2.0 REQUIREMENTS: A.
Security:
1. Secure all construction areas, especially mechanical and electrical rooms against entry of unauthorized individuals including patients.
2. Notify the CO’s Technical Representative (COTR) for permission to work after hours and weekends. Standard work hours for this project will be Monday through Friday from 7:30 am to 4:00 pm. B. Key Security:
1. Only a limited number of keys will be issued to the contractor.
2. See section 01 00 00 for procedures in case the Contractor loses a key.
3. Ensure all doors leading to and from construction are either monitored or locked to prevent access to the area from unauthorized persons.
C. General Safety:
1. Follow all federal, state and local safety and health regulations.
2. Maintain safety in the construction site/area in accordance with the provisions of the contract that includes the Occupational Safety and Health Administration (OSHA) Regulations; National Electrical Codes;
National Fire Protection Association (NFPA) 70, National Electric Code;
and NFPA 101, Life Safety Code. Work in a safe manner and take all proper precautions while performing your work. Extra precautions shall be taken when working around persons occupying the building during construction.
3. Provide Personal Protective Equipment (PPE) for your employees.
4. Post appropriate signs in specific hazardous areas.
5. Keep tools, ladders, etc., away from patients to prevent injuries.
D. Safety Inspections: The professional Occupational Safety and Health staff at this facility will perform safety inspections of all contract operations. Written reports of unsafe practices or conditions will be reported to the COTR and CO for immediate attention and resolution. E.
Fire Alarms:

1. The fire alarm system connects all buildings at this facility, and is activated by various heat, duct, manual pull stations and smoke sensors. Manual pull stations are provided at each entrance. Please survey the area in which you are working to locate the manual pull stations.
2. In the event of a fire alarm sounding, you are to remain in your area, unless medical center personnel (Safety, Nursing or Engineering) instruct otherwise, or unless a fire situation is in your area, in which case you should immediately evacuate.
3. Any work involving the fire protection systems will require written permission to proceed from the COTR. Do not tamper with or otherwise disturb any fire alarm system components without prior written permission. To do so without written permission will result in an adverse action. F. Hazardous Materials:
1. Many of the operations you are scheduled to perform may involve the use of hazardous materials. Prior to locating hazardous materials on site, all Material Safety Data Sheets (MSDS) will be submitted through the COTR for evaluation by the facility Safety Officer.
2. Storage of hazardous materials within buildings will be minimal with only enough on hand to perform daily work tasks. Flammable materials will either be removed from buildings at the end of the work shift or stored in approved flammable storage containers.
3. Care must be taken to ensure adequate ventilation to remove vapors of hazardous materials in use. Many of the patients being cared for in the facility are susceptible to environmental contaminants, even when odors seem minimal. You will isolate those areas where vapors are produced, and ventilate to the most extent possible to reduce the number of complaints.
G. Airborne Dust Control During Construction:
1. Generation of dust is of major concern within staff and especially in patient occupied buildings. Where operations involve the generation of dust, all efforts will be directed at reducing airborne generated dust to the lowest level feasible. This may be accomplished by a number of methods. These include misting the area with water, or use of tools attached to high efficiency particulate air (HEPA) filtering vacuums.
Where large amounts of materials may be disturbed, resulting in airborne dust, establishment of full ceiling-to-floor plastic barriers may be required. 2. Classification of Jobs: see ICRA (Attachment A)
H. Infection Control Procedures: see ICRA (Attachment A)
1. Implement the requirements of VAMC’s Infection Control Risk Assessment (ICRA) team. ICRA Group may monitor dust in the vicinity of the construction work and require the Contractor to take corrective action immediately if the safe levels are exceeded.
2. Establish and maintain a dust control program as part of the contractor’s infection preventive measures in accordance with the guidelines provided by ICRA Group. Prior to start of work, prepare a plan detailing project-specific dust protection measures, including periodic status reports, and submit to COTR for review for compliance
01 00 10-3 with contract requirements in accordance with Section 01340, SAMPLES AND SHOP DRAWINGS. All personnel involved in the construction or renovation activity shall be educated and trained in infection prevention measures established by the medical center.
3. Medical center Infection Control personnel shall monitor for airborne disease (e.g. aspergillosis) as appropriate during construction. A baseline of conditions may be established by the medical center prior to the start of work and periodically during the construction stage to determine impact of construction activities on indoor air quality. In addition:
a. The COTR and VAMC Infection Control personnel shall review pressure differential monitoring documentation to verify that pressure differentials in the construction zone and in the patient-care rooms are appropriate for their settings. The requirement for negative air pressure in the construction zone shall depend on the location and type of activity. Upon notification, the contractor shall implement corrective measures to restore proper pressure differentials as needed.
b. In case of any problem, the medical center, along with assistance from the contractor, shall conduct an environmental assessment to find and eliminate the source.
4. In general, following preventive measures shall be adopted during construction to keep down dust and prevent mold.
a. Dampen debris to keep down dust and provide temporary construction partitions in existing structures where directed by COTR.
b. Do not perform dust producing tasks within occupied areas without the approval of the COTR. For construction in any areas that will remain jointly occupied by the medical Center and Contractor’s workers, the Contractor shall:
(1) Provide dust proof construction barriers to completely separate construction from the operational areas of the hospital in order to contain dirt debris and dust. A fire retardant polystyrene, 6-mil thick or greater plastic barrier meeting local fire codes may be used where dust control is the only hazard, and an agreement is reached with the COTR. This will be required in the ICU, the Step-Down Unit, and possibly other areas.
(2) Adhesive Walk-off Mats, minimum 24” x 36”, shall be used at all interior transitions from the construction area to occupied medical center area.
