Question #01:

· The following active leaks were identified during the site visit which are not listed in the scope of work. Please advise whether these items should be incorporated into the project scope. Yes

Question #02:

· Attachment 1 under CLIN 1 identified the active leaks below at structural column D-8. However, during the site visit no active leaks were observed. Is this system currently active? Will the system be active in heating season in the next month and require a steam shutdown to replace? Please advise whether these items should be incorporated into the project scope of work and if so, provide detail on quantity, size, fittings which are desired to be replaced. Work still needed.

Question #03:

· Addendum 0002 Attachment 1 defines the required quantities for the demolition and replacement scope of work under the kitchen and lab. Addendum 0002 Section B2.6.b.iv. narrowed the scope of work as all piping between columns marked S-10 to J-10 through S-4 to J-4 in the building 200 sub-basement per referenced blueprints. Please advise which section of the SOW governs, as these statements are in conflict. Please see Attachment 3 and SOW

· Portions of the existing sanitary systems extend to the S-3 to J-3 columns and the total quantity of plumbing to replace is greater than the quantities listed in the SOW. Should bidders provide pricing on the quantity of plumbing listed in the SOW, or should they bid on the quantity of plumbing they believe represents the entirety of work to replace the system within the bounds of the project area and list that quantity in their schedule of services? Please see updated SOW. Contractors should include quantities and linear feet on their bids.

· Please clarify the work area and quantities related to the plumbing desired to be replaced. Please see Attachment 3 and SOW

Question #04:

· Please advise what chemicals are utilized within the facility that would be impacting the new acid waste pipes serving the labs so that the proper schedule pipe material can be identified or identify the schedule of pipe to be utilized for the waste pipes serving the lab. Orion or equivalent for chemical piping.

Question # 05:

· Addendum 0001 Question #4 confirmed no scope of work located above the ceiling line of the sub-basement. With the inclusion of the kitchen floor drains in Addendum 0002, please confirm work within the kitchen area is now required. Who will be the facility’s point of contact to provide after-hours/overnight access and coordination of facility terminal cleaning services overnight so that the kitchen is operational and not impacted by 0500? Please see updated SOW. 8 Kitchen drains can be accessed and worked on during regular hours when scheduled with Hines staff.

Question # 06:

· Is it acceptable to perform repairs to the kitchen and lab at night, and each day tie the new piping to be into the existing system to avoid an impact to hospital kitchen operations? Are lab services and plumbing active overnight? Please see updated SOW

· Addendum 0002 notes kitchen work must take place between 9:00pm Friday – 4:00am Monday. If a weekend shutdown associated with the kitchen/lab plumbing must be completed, how many weekend shutdowns are allowable? Kitchen work can be accomplished during regular work hours. Tie-ins from old to new should be scheduled with Hines Engineering/Kitchen staff when impacting patient food services. The less shutdowns of weekends the better.

Question # 07:

· With the inclusion of the kitchen floor drains in Addendum 0002, please define what level of containment is required. Is a temporary Visqueen containment system for dust control sufficient? Can the containment system remain in place during the daytime operations, or must it be removed at the end of each workday? Containment left during Kitchen hours should be hard containment. Contractor shall leave work area in same condition as start, food sanitary clean.

Question # 08:

· The September 9th site visit appears to have identified more than 13 floor drains. Please confirm the appropriate number of floor drains incorporated in the scope of work. Please see updated SOW and Attachment 4

· Please advise if a commercially available floor drain and cover is acceptable. The current floor drain covers were custom fabricated by the VA Hines Machine Shop. Selections must be okayed through Hines Engineering.

Question # 09:

· Please confirm the appropriate number of pumps for the Column Q-2 sump pump system. Does the facility want one 2-inch grinder pump or a dual stainless steel ½ horsepower system? Please see updated Attachment 1. Selections must be approved by Hines Engineering

Question #10:

· Please provide a more thorough breakdown of the required removals associated with Section 6.c. xiii. Addendum #0001 Question #12 confirmed the exclusion of the several pieces of abandoned equipment in Quadrant D. Please advise. Please see updated SOW and Attachment 3

Question #11:

· Addendum 0002 notes the Contractor shall remove 120’ of abandoned conveyor system. Please confirm the remaining portions of the conveyor system are to remain in place. Contractor shall remove abandoned conveyor machinery from door to door of hallway serving better access from Quadrant D-A

· What are the hot work permit requirements associated with working in the sub-basement to remove the conveyor? None

· Are all ventilation systems operating properly within the subbasement to achieve adequate air exchange to facilitate construction work? Yes

Question #12:

· Addendum 0001 Questions #13 & #14 indicated C017 as the access point to the sub-basement. Quadrant D can be accessed from the loading dock drive. Please confirm if the driveway into the loading dock can be utilized as a work area for demolition material and access, so long as the loading dock remains operational. Noting majority of this work will be occurring off hours and overnight when use of the loading dock is minimal. Deliveries and schedules for dumpster placement must be approved by Hines Engineering

Question #13:

Please provide quantities of ACM insulation to be abated as part of this contract. Minimal (8) small joints at most throughout different regions of basement.

Question #14:

Please provide a cleaning specification for the kitchen area around the drains Please see updated SOW. After work, contractor shall leave area in same condition as found, sanitary.

Question #15:

Do the sump pumps need to be on a duplex system with generator backups? No Question #16:

Please issue specifications for the materials to be used in this project. Please see updated SOW

B.2 STATEMENT OF WORK/PWS

*UPDATED* STATEMENT OF WORK

Building 200 Sub-Basement Utility Repair

1. Location of Service The Contractor shall provide all labor, material, tools, travel, management, and equipment necessary for the repair of items described in this document at the Edward Hines Jr. VA Hospital (Hines VAH), 5000 South 5th Avenue, Hines, Illinois 60141.

2. Period of Performance The period of performance shall not exceed one hundred sixty (160) days after contract award. This period reflects the entire contract period to include preconstruction periods, administrative time, and contract close out. Actual onsite construction time is not expected to exceed one hundred (100) days from initial mobilization. A detailed schedule must be provided to the Contracting Officer Representative (COR) within 20 days after receiving the contract Notice to Proceed (NTP) and prior to any onsite work.

3. Background/History/Current Conditions Hines VA is an active medical campus. The contractor must not interfere with patient care or VA staff unless given permission in writing from the COR/ Contracting Officer (CO). Building 200 sub-basement had a catastrophic failure in sewage pipes that left raw sewage draining into existing gravel surface. A remediation company already on-site, mobilized to contain, remove damaged stone, and disinfect affected area.

4. Objective The services and personnel shall be provided by the contractor that has specifically trained field support or technical support representatives for all plumbing, pipefitting, and labor needed to replace the plumbing and repair the steam.

5. Scope of Work This contract includes the repair of various steam pipes and all drains and vents between columns marked S-10 to J-10 through S-3 to J-3 in sub-basement.

6. Deliverables and Tasks

a. CLIN 1 – Repair Steam Pipes

i. Contractor shall repair all steam lines at marked locations. (Attachment 1)

ii. Contractor shall remove asbestos (AB) insulation at affected joints and replace with appropriately sized fiberglass pipe insulation. AB removal is considered “glove-bag” due to the quantity.

iii. Any steam shutdowns will be coordinated through Douglas Crombie, Hines VAH Pipe Shop Supervisor.

b. CLIN 2 – Replace Sanitary Pipes

iv. Contractor shall replace all sanitary pipes and joined vent pipes, between columns marked S-10 to J-10 through S-3 to J-3 in building 200 sub-basement with “no-hub” cast iron. Please note: S column represents exterior concrete wall on the west and the J column represents the concrete hallway wall separating Quadrant A from Quadrant D. (Attachment 3) This contract does not include underground work below the stone grade of sub-basement. Any “new” copper/cast iron drains/vents shall not need to be replaced. Pipes that travel past designated work area shall be cut at boundary line then attached and run throughout.

v. All abandoned pipes in work area and pipes in need of replacement, shall be disposed of properly and removed from campus by contractor.

vi. All vertical piping through concrete deck above, shall be cut (reasonable distance 3”-6” from deck above) and joined to new piping systems. Exception: (8) Kitchen Floor Drains (Attachment 4)

vii. All horizontal piping through walls shall be sealed accordingly. Where necessary for contractor to create new horizontal penetrations, contractor shall repair the walls of old penetrations.

viii. Contractor shall work with Hines Engineering and Kitchen staff for work requirements under Kitchen and in area. Kitchen working hours are 5:00am – 8:30pm every day. Any sanitary shutdown in the kitchen, must be scheduled two (2) weeks in advance.

ix. Contractor shall replace eight (8) floor drains on the Kitchen floor with appropriately sized heavy gauge strainers able to withstand rolling carts, connecting to new sanitary below. Replacement of floor drains shall be scheduled with Hines Engineering/Kitchen staff in advance. Containment and work must not interfere with Kitchen. Contractor shall clean area to sanitary conditions after work completion.

x. Contractor shall repair damaged floor around new drains. Epoxy, sealed concrete, or other COR approved method shall be used around the finished floor to the drain when less than 6 inches beyond the edge of finished drain. If the demolition of the drain results in a larger than 6-inch gap from existing tile to finished drain, then a replacement porcelain tile shall be used to patch in. Final tile selection shall be approved by COR prior to install.

xi. Contractor shall replace six (6) sump pumps located in Attachment 1.

xii. All sanitary shutdowns for other lines will be coordinated two (2) weeks in advance and can be shutdown for five (5) working days maximum, through Hines VAH Engineering, Pipe Shop Supervisor, and COR.

c. CLIN 3 – Remove Heavy Equipment (Attachment 3)

xiii. Remove abandoned Water Booster located under kitchen.

xiv. Contractor shall approximately remove 120’ of abandoned conveyer system with disconnection of power and hydraulics to allow access to South of work area.

b. CLIN 4 – As Built

i. Provide pdf showing all new plumbing lines and pipe installed during this project. Drawing can be schematical but cannot be hand drawn redlines.

7. Hours of Operation Contractor shall ensure that all work shall be performed to minimize impact on hospital operations and patient care. This could necessitate evening and weekend work to mitigate an impact on hospital operations. Pricing shall be based on the possibility of working after hours.

2. Work outside of the business hours of Monday-Friday 6:00am-5:00pm, or during federal holidays shall be confirmed and approved by COR.

2. Utility shutdowns that would prevent patient care require minimum 20 calendar day notice.

2. The contractor shall complete the delivery on the specified above. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue response in accordance with applicable regulations.

8. Reports and Documentation

6. Bids from contractors should include sizes, number of fittings, and linear feet of the different sized pipe for bid evaluation.

6. Schedule. The contractor is responsible for submitting a schedule of sufficient detail for approval by the COR/CO.

4. Equipment Lead time. The contractor shall make an informed bid submission by working with suppliers and considering all potential long-lead items involved with the project reasonably known or considered to impact the start, performance, and completion of the project. All deliveries, dumpster placements, and movement of materials shall be coordinated with Hines Engineering during scheduling period.

4. The schedule shall be submitted within 20 days of the signed Notice to Proceed (NTP) and must be approved prior to any onsite work beginning.

4. Work outside of the business hours of Monday-Friday 6:00am-5:00pm, or during federal holidays shall be confirmed and approved by COR.

4. The contractor shall complete the delivery on the specified above within 120 calendar days of the signed NTP. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue response in accordance with applicable regulations.

4. This contract shall contain 40 days for inspections and close-out after contractor completes CLINs.

6. Weekly Report: The contractor shall provide a weekly report of all work performed, services provided, and service inspection.

5. The contractor shall provide written report electronically to COR.

6. Site Survey. On completion of work, contractor shall supply updated blueprints of sanitary piping for sub-basement in bldg. 200.

9. Changes to the Contract

6. Any changes to this Statement of Work (SOW), or the executed contract shall be authorized only through a contract modification from the CO. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. If the contractor finds an issue that is not a part of the statement of work, the contractor shall notify the COR with a written description of the issue and provide a quote to remedy the problem. Contracting Officer shall issue a modification or new purchase order if it is determined to be needed by the COR.

10. General Requirements

6. Where a conflict exists between the SOW requirements and the requirements of other associated contract documents, the SOW requirements shall prevail.

6. The awarded contractor will adhere to all codes, standards, guidelines, and design requirements found in the VA’s TIL https://www.cfm.va.gov/til/index.asp.

6. Wood studs, plywood, OSB, particle board, pressboard, or other flammable materials are not allowed at Hines.

6. Contractor shall cover all sprinkler heads (daily), signs, data transmitters, and light fixtures. Contractor shall prevent the spread of dust and fumes to other areas outside the work zone.

6. The contractor shall post and maintain the final signed emergency contact information (when issued) at the entrance to the jobsite for the duration of the project. See Attachments for the details of these forms. The contractor shall update the postings when new documents are issued.

6. Contractor shall be responsible for providing all utility hookups and other items or work as required such that the final product performs to the requirements per OSHA and VA guidelines. Any clarifications needed that would affect contract cost must be confirmed via RFI prior to work. Contractor is responsible for the cost of all assumptions made.

6. Hines VA Local Permits: Unless otherwise stated the contractor shall submit for all required permits fourteen (14) calendar days prior to the event based on the approved project schedule with the COR.
