THOSE ASKED ON THE SITE VISIT. THIS AMENDMENT

WILL ALSO EXTEND THE SOLICITATION TO ACCOMADATE THE CHANGES TO THE STATEMENT OF WORK, SECOND SITE VISIT AND TIME FOR THE

VENDORS TO REVIEW THE ANSWERS TO THOSE QUESTIONS SUBMITTED.

ALL OTHER TERMS AND CONDITIONS REMAIN THE SAME.

Due to the number of questions raised by the Vendors that attended the site visit, the VA is revamping the Statement of Work to alleviate some of the concerns that were raised and correct some language to make the statement of work more concise. The CO will post the updated Statement of Work by Tuesday 09/05/2023 at 10:00 am. A second site visit will be held at the HINES VAMC to ensure that the VA addressed all questions and concerns, this site visit will be on 09/08/23 at 09:00 AM central time. MEET COR JASON SARENY IN BUILDING 2 BASEMENT ROOM 011A. All vendors must check-in with the police desk. Questions after the site visit on 09/08/23 shall be submitted no later than 09/11/2023 at 07:00 am.

Responses to the following RFQ questions submitted by vendors are in RED.

Question # 01:

· Will selection criteria include firms which demonstrate an understanding of the scope of work gained during the walk through, by providing sufficient detail and quantities in proposal submittal to minimize the potential for modifications?

The Selection criteria is listed in section E of the solicitation. Offerors that attend the site visit and show a better understanding of the scope of work will receive better consideration (this will need to be shown through their submitted package). The Government will award based on “best value”.

Question # 02:

· Will the VA be providing an electronic copy of the site visit sign in sheet?

Question #03:

· Is there a required completion priority between CLIN 0001, 0002, and 0003? For instance, do the active leaks in CLIN 0001 need to be repaired first? Contractor discretion

Question #04:

· Please clarify that all work is to be performed in the stone space / sub-basement. That is, NO work shall be performed in the kitchen, lab or outside of the mechanical space. Plumbing systems will connect to existing penetrations in the deck system. True. No work needs to be accomplished above ceiling line of sub-basement.

Question #05:

· Please confirm the work hours for this project. Repairs to the various leaks will require a shutdown and are likely to occur on weekends or outside normal business hours to limit curtailment of hospital operations. The sanitary sewer systems must remain active from approximately 0400-2000 each day to allow for normal function of the kitchen and hospital operations. Therefore, repairs to the kitchen and lab will need to be performed at night, and each day the new piping will need to be tied into the existing system. To minimize impact on hospital operations and patient care, it appears all scope of work/pricing should be based on working after hours (overtime rates). Some work might have to be done off hours. See Section 7 of the STATEMENT OF WORK.

· Please provide a more thorough breakdown of the required scope of work for the below item in Attachment 1. Seeking clarity so all bidders uniformly approach this solicitation. None. This will be taken out of future SOW. Updated SOW will be posted ASAP.

· The following active leaks were identified during the site visit. Please advise whether these items should be incorporated in CLIN 0001. All steam repairs are encapsulated in CLIN 1. This list was given by Pipe Shop Supervisor and not in order of CLINs.

Question #09:

· Please provide a more thorough breakdown of the required repairs associated with the five sump pump items identified in Attachment 1 under CLIN 0001. Specific information on required replacement is necessary to provide an accurate bid package (i.e., pump sizes, pump voltage, electrical service connections / point of connection, and linear footage of piping). The following table provides quantity and sizing requirements as identified by AZIMUTH during the site visit. Please advise on accuracy. Please see updated Attachment 1. This will be posted with updated SOW.

Question #10:

· Please confirm all replacement sanitary piping shall be cast iron per VA standards. True, all new pipes shall be up to code.

Question #11:

· How many cleanouts are required to be installed? We recommend a minimum of 5 cleanouts be installed to improve access for maintenance and prevent future environmental hazards. Cleanouts should be brought up to Cook County Code.

Question #12:

· The following items were identified as abandoned in the vicinity of the work during the site visit. How should the contractor handle removal of these larger equipment items identified in the below table? Should we assume disconnecting the electrical services as well as salvaging and disposing of any Freon is INCLUDED in the CLIN 0003 line item? None of these. Only water booster located under Kitchen.
