A.1 Statement of Work
Duct Cleaning Building 31
1. Expectations This contract is for the work listed below. All work will be performed by the Contractor and all materials will be provided by the Contractor unless noted that the VA “will provide.” The specific job site location is Building 31 which is our research facility, VA Medical Center, 2215 Fuller Rd., Ann Arbor, MI.
2. Background To supply all necessary equipment, materials, and labor to clean all supply and exhaust ductwork and reheat coils serving the Basement, 1st, and 2nd floors of Bld. 31. 3: Air Handling Units (AHU) and all exhaust fans located in the Mechanical Penthouse will be cleaned as well.
3. Scope of work Required elements for Duct Cleaning:
A) Specific Tasks
1) Work is required to take place after hours and/or weekends.
2) Complete pre-job safety checklist.
3) Perform Lockout/Tagout procedure for AHU equipment. Coordinate with VA staff.
4) Contractor to cut in openings in existing ductwork to perform tasks inside ductwork.
5) Contractor will encapsulate VAV boxes lined with fiberglass using IAQ Fiberlock 8500..
6) Will clean and sanitize all supply duct, return duct, 3: AHUs, fans, reheat coils and all associated equipment related to the complete Air Handling Equipment to include diffusers for the entirety of Bld. 31 Air Handling Equipment.
7) In room filters in registers will be changed by contractor. Filters will be provided by VA Staff.
8) Contractor will install new access panels and/or covers for ductwork that was opened to perform work.
9) Contractors responsible for any modification to existing ductwork.
10) Start, test, and check equipment for proper operation.
11) Verify all ductwork is secured and seams have been sealed.
12) Clean up the work area when complete and haul away all debris.
13) Work to be completed in approximately 1 month.
4. Responsibilities Contractor will be responsible for providing all required equipment which includes but not limited to:
1) Cleaning and sanitizing materials.
2) Will clean work spaces once completed. No special cleanings are required.
3) Carts or dollies required to transport equipment, materials, and tools.
4) All necessary equipment & tools.
5) All necessary replacement equipment & parts.
6) All necessary PPE.
7) Personal Lockout/Tagout locks.
5. VA staff requirements VA staff will provide:
1) Lockout/Tagout of effected equipment.
2) Notify all necessary staff of interruptions to their workstations.
3) VA Staff will be present to provide access to all areas where work is needed.
4) VA Staff will take proper precautions to cover/protect all vital equipment/ongoing research projects.
5) VA Staff will relocate animals from rooms ahead of scheduled cleaning of equipment in the affected spaces.
6) Coordinate with Contractor on timeframes of work to take place and which areas will be affected with a 2-week notice to Bld. 31 staff.
7) Guidance of location of eyewash/safety showers, alarm pull stations, building exits, emergency procedures.
8) Guidance on vehicle and equipment staging.
9) Verify work has been completed performed in accordance with the National Air Duct Cleaners Association standard # ACR-2013.
6. Conformance standards All work is required to be performed in accordance with the National Air Duct Cleaners Association standard # ACR-2013. All labor practices conform to OSHA rules: proper PPE and Safety Training are required.
7. Parts All materials and installation work will have a standard warranty period of one year from the date of installation.
8. Reporting requirements When necessary for the Contractor, (or Sub-Contractor(s)), to be onsite at the Ann Arbor Campus, they shall be required to report to the HVAC Shop Supervisor in Building 32 to log in during normal working hours. Before or after normal working hours report to the Utility Systems Repairer Operator on duty. This check in is mandatory. Normal working hours are Mon. – Fri. 7am.-3:30pm. Excluding Federally Observed Holidays.
9. Safety Requirements If on the Ann Arbor Campus during the performance of this contract, the Contractor shall take such safety precautions as OSHA may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Vendor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work and hold the Contractor in default.
Questions with Answers:
1. Are we completing AHUs 1 ,2, and 3, or just 1 and 2? 1,2,&3
2. Are we cleaning the section of Outside Air connected between AHUs 1 and 2? Yes
3. Is the exhaust ductwork included as well, especially in the basement animal testing center? Yes
4. What is the recommended/suggested cleaning method for the valves attached to nearly each return grill? None
5. On the first floor, in the labs, many of the return grills had filters on them. Are we supposed to change out those filters post cleaning, or simply leave them off? Change post cleaning, VA will provide
6. The basement had an extra step with the animal testing center. What will be the day process for ensuring the animals are removed, prior to cleaning? How many rooms at 1 time will get during this period and how many rooms/day will be given? VA staff will coordinate room availability and provide as many rooms as possible at a time/day. Can’t determine # at this time.
7. Are the ERV units to be included or not? If included, should it be within, or as an option? Included
8. On the list of inclusions, it states the exhausts in the unit rooms are part of the scope, but is all of the other exhaust systems servicing these areas to be included? Yes
9. Are these areas going to be occupied while the work is being done? No
10. Is all of the ductwork for unit#3 to be included, or just listed as an option? Included
11. Will Carl, Mark, or other maintenance personnel from the VA Medical Center staff be on site if needed during the work? VA staff will be present
12. Requesting the HVAC plans Provided
13. Do you have to be a part of the National Air Duct Cleaners Association ? Yes
14. Floor plan layout or mechanical drawings for this location that you could send me? –Include ceiling grid / which rooms / locations have the different types of ceilings and what type of ceilings are in those locations (hard ceilings, animal rooms with vents in the middle, etc.)? Provided

INSTRUCTIONS TO OFFERORS QUOTATION SUBMISSION INFORMATION AND EVALUATION CRITERIA
Offerors are required to complete Blocks 17, and 30a-c of the SF1449, the Schedule of Supplies/Services (see section B), FAR 52.212-3 Offeror Representations and Certifications – Commercial Items, VAAR 852.219-75, FAR 52.204-24 and acknowledgement of all solicitation Amendments issued, and the information identified below.
1) GENERAL INFORMATION
Selection of an offeror for contract award will be made on the basis of an assessment of each offeror’s response to the Request for Quotes (RFQ). Award will be based on the overall Best Value to the Government for quotes meeting the evaluation criteria.
Award shall be based upon best overall value to the Government:
· Price
· Past Performance
· Certification
· Provide documentation of required training and certification
NOTE: Failure to comply with the RFQ requirements may be grounds to eliminate the quote from further consideration for contract award.
2) THE OFFER
The submission of the documentation specified below will constitute the Offeror’s acceptance of the terms and conditions of the RFQ. These instructions prescribe the format for the quote and describe the approach for the development and presentation of quotation data. These instructions are designed to ensure the submission of necessary information to provide for the understanding and comprehensive evaluation of quote.
3) NUMBER OF COPIES TO SUBMIT
Offerors shall submit a SINGLE PACKAGE via email to Morgan.Stein@va.gov. The package shall include:
· ORIGINAL - RESPONSE TO RFQ –36C25024Q0728, with applicable response to 0002
· Provide documentation certifying training and certification
· Contractor shall complete FAR 52.204-24 and return with quotation (Section E)
· Contractor shall complete VAAR 852.219-75 and return with quotation (Section B)
4) Quote Submission - Offerors shall submit one (1) complete copy of their quote either via email to Morgan.Stein@va.gov no later than the date and time specified on the 1449 Block 8.
a) Standard Form (SF) 1449 Submit the SF 1449 issued under this solicitation, with Block 17 filled-out by the Offeror. Include acknowledgment of any and all Amendments that may have been issued.
NOTE: Failure to acknowledge any and all Amendments issued pursuant to this solicitation may be cause for rejection of your offer.
b) Price Quote – Shall contain one (1) original copy of the signed quotation. The cost/price portion of each quote shall be submitted in the space provided in the Schedule (section B).
c) Past Performance This factor shall be used to evaluate the offeror’s most recent and relevant experience accomplishing the type of work required by this solicitation. This factor shall also be used to evaluate how well an offeror has performed on previous projects of similar scope, magnitude and complexity, and is an indicator of the likelihood of completion of new projects in terms of time, cost, and quality of work. Past Performance shall be evaluated through the use of the Contractor Past Performance Retrieval System (CPARS). An offeror that has no available relevant past performance history shall not be evaluated favorably or unfavorably.
d) Certification
· Provide documentation of required training and certification
5) Failure to submit the necessary information may render the offer unacceptable, in which case it will not be considered for award. Quote will be evaluated in accordance with the criteria set forth above. The Government may, at its discretion and in its best interest, make award without conducting discussions with offerors. Therefore, perspective offerors are encouraged to submit initial quote that represent the best terms and conditions of the offeror.
6) BASIS FOR CONTRACT AWARD:
Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13.106-2. Comparative evaluation is the side by side pairwise comparison of quotes based on factors (price, past performance, certification, speed of delivery) resulting in a Contracting Officer decision for the quote most favorable to the Government.
The award will be made to the response most advantageous to the Government. Responses should contain your best terms and conditions.
Evaluation of Quotes:
The Government will select the quote that represents the best value to the Government at a price that can be determined reasonable. The Government will evaluate the following: Technical Capability (certification), Past Performance, and Price.
Following receipt of quotes, the Government will perform a comparative evaluation of the products / services quoted. The Government will compare quotes to one another to select the product/service that represent the best value to the Government by fulfilling the requirement.
· The VA will compare quotes against each other in an impartial and simplified manner.
· The VA may ask questions to one or more submitters but is not obligated to do so.
· The VA may reject all quotes.
· The contracting officer and VA users may use any means available to examine past performance, such as contractor reliability and reputation and suitability of the product or service to meet the VA’s need.
· VA may discuss price or price discounts with one or more submitters before awarding an order or rejecting all quotes.
· The VA will evaluate quotes for award purposes by adding the total price for all options (if applicable) to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
The evaluation of VA requirements outlined in this request for quote will determine suitability.

a. The offeror must provide sufficient documentation to allow for validation of the requirements listed in the Specific Tasks and capabilities listed.
b. The offeror must also provide sufficient documentation for validation on how it will meet the required Specific Tasks listed in each line item.
2. Price
a. Total quote price shall be inclusive of all listed information.
