STATEMENT OF WORK

General Information:

1. Title of Project: Skytron Freedom Booms Stevens Moon

2. Scope of Work: The Ann Arbor Veterans Affairs Healthcare System has a requirement for a total of 3 booms containing surgical lights, an equipment carrier and a Camera System to be installed in the EP Lab. The camera system will support the Cardiology Service in the Medicine Service. The Vendor shall provide all resources necessary to accomplish the deliverables described in this Statement of Work except as may otherwise be specified. The delivery order will be awarded as Firm-Fixed-Price.

3. Background: The EP lab will be undergoing renovations and this equipment is part of that renovation. This equipment is needed to perform ablations, implants and extractions in the procedure lab. The equipment carrier boom will improve safety by integrating the electrical/data cabling into the boom and running it into the ceiling. Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C250-22-AP-3086

1. Contracting Activity: Department of Veterans Affairs, VISN 10, Network Contracting Office 10 on behalf of the LTC Charles S. Kettles Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105. 2237: 506-22-3-118-0187.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Surgical lighting and boom system for use in the Cardiology Catherization/Ablation Lab.

[Redacted]

Total Estimated Value: [Redacted]; No Options Included.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: BRAND NAME ONLY: There are many booms and surgical lights available on the market, but none that have vertical bars on the sides offering protection to our ablation system and other delicate equipment used in this procedures lab. These vertical bars will prevent the equipment from accidently being hit by our fluoroscopic monitors or by accidently being knocked into or off of the equipment carrier. These vertical bars still allow access to equipment stored on the carrier from 360 degrees, which is a huge advantage over solid sided storage solutions. This system positively impacts and enhances patient care due to these unique characteristics. The order needs to be placed as soon as possible due to the construction schedule is slated to start in July of 2022. The current lead time of the booms is 90 to 120 days ARO. Any delay in construction will delay surgeries and urgent medical procedures, causing an unacceptable negative impact on patient safety.

4. Description of market research conducted and results or statement why it was not conducted: Market Research was conducted. Reviewed numerous product catalogues from the SAC, NAC, SEWP, Open Market and was not able to find anything that offered the protection to the currently owned ablation equipment while giving accessibility from all angles. VIP was queried for NAICS 334510 and 198 concerns were found. VIP was queried for the above NAICS, and keyword “lighting” and 11 concerns were found. Skytron LLC is a small business and manufactures the requirement. They have one SDVOSB distributor, Alliant Enterprises, who can also meet the requirement. The VA rule of two cannot be met. I recommend a total small business set aside using FAR Part 13.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
