Changes are required to the following parts of the SOW attached to the Combined Synopsis/Solicitation
36C25022Q0261. Please make changes to the verbiage under the Change: sections of numbers 1. through
4. and change to the verbiage under the To read: sections of numbers 1. through 4.
1. Section 4. Scope, page 2 of “SOW Ceiling Lifts CT2”.
Change: 4. Scope.
a. Site Preparation/Installation of thirty-four (34) GH3+825 w/scales Ceiling Lift Systems
b. Site Preparation/Installation of three (3) GH3+1100 w/scales Ceiling Lift Systems
c. Site Preparation/Installation of twenty-eight (31) GH3+ 825 wo/scales Ceiling Lift Systems
e. Site Preparation/Installation of one (1) Guldmann GH3+ w/integrated scales Ceiling Lift
System (#600 minimum weight capacity) and integrates with gait training system

a. Installation of thirty-four (34) GH3+825 w/scales Ceiling Lift Systems

2. Section 5., Specific Tasks paragraph b(i), page 3 of “SOW Ceiling Lifts CT2”.
Change: b. Site Preparation and Installation for sixty-five (65) EA GH3+825 systems in rooms listed below:
i. Site Preparation shall comprise of all structural modifications to facilitate lift installation. Due to the need for installation of ceiling hatches in each room, contractor will collaborate with the COR for the ceiling lift project to facilitate coordination of hatch and ceiling lift installations. The contractor will also be responsible for removal of all material required for equipment installation. See attached drawings for specific locations.
b. Installation for sixty-five (65) EA GH3+825 systems in rooms listed below:
i. The space where the ceiling lifts will be installed is in the process of renovation.
Coordination with the site prep contractor is crucial as there are multiple steps involved with the ceiling lift installation. The ceiling lift contractor will be responsible for coordinating ceiling lift installation timing with the COR and VA’s construction contractor.
One access panel per patient bathroom will be installed in locations designated by the ceiling lift installer. The ceiling lift contractor will be responsible for removal of all material required for lift installation.
The stages of installation are:
1. VA construction contractor remove ceiling
2. Ceiling lift contractor install lift supports to just below new ceiling height
3. VA construction contractor install new ceilings
4. Ceiling lift contractor finishes the ceiling lift installation
3. Section 5., Specific Tasks paragraph f. Brand Name or Equal Purchase Descriptions, number 4, The requested sixty-nine ceiling lifts must have:, the following bullet points., page 8 of “SOW Ceiling
Lifts CT2.”
Change:
• To facilitate ease in servicing and maintaining the systems, plug and play technology must be incorporated into the system.
• Plug and play technology must be employed to facilitate ease and reduced costs in system repairs and maintenance, one person replacement on current rail system.
• To facilitate ease in servicing and maintaining the systems, plug and play technology
(software that allows monitoring of service status information and historical data) must be incorporated into the system.

Change:
Contractor will be installing units in an active healthcare environment and will be required to comply with
VAMC required above ceiling work permits, infection control procedures, and all OSHA requirements.
