QUESTIONS:

1. Concerning the trips in the outlying areas (i.e., Tallahassee, Valdosta, Lecanto, Jacksonville, etc.) – How many trips in the outlying areas are local trips (10 miles or less)? How many trips originating from outlying area and going to Lake City or Gainesville VA (per year or per month)? Trips vary from month to month; however, the VA station drivers will transport some Veterans

1. There is no mention of patients on O2. Does this mean that there will be no patients on O2? No If there are some that are being transported with O2, do we provide the O2 or does the VA? If we do provide are we able to charge for the O2? The VA will supply the oxygen if needed.

1. The Price Cost states that there are 9,500 local passenger wheelchair trips and 200 local stretchers. What are the number of wheelchair and stretcher trips that are more than the local (10 miles or less)? There is no standard number, the patients vary from month to month.

1. Section 8(b) mentions the VA sending an “authorized attendant (RN, LPN, CNA, or Physician). What is the purpose of sending licensed staff? WE cannot provide any medical treatments in non-emergent medical transport. Some patients require special needs but doesn’t necessarily require an ambulance. A CNA may be needed.

1. Would like clarification on the term scooter. Is this term to include the motorized wheelchairs or simply the “Jazzy” or 3 wheeled scooter type devices? 3 wheeled scooter I understand we are not to transport the patient on the “scooter,” but would we be required or authorized to transport the scooter for the patients use at the VA? Transporting the patient mode of transportation is necessary for the patient to be mobilized for appointment etc.

1. How many mental health transfers? No standard number the patients vary from month to month. Are the patients typically in crisis or routine appointments? We have several mental health transfers. They are typically routine. Would the vehicles need to be secure (meaning having a cage installed)? The vehicles do not need to have a cage but it will be a clever idea to have locked doors/safety child lock.

1. What about transports scheduled on holidays or after hours? Typically, there is an additional charge for any transports on a holiday or after 6pm. Nothing extra on holiday or afterhours

1. Please clarify the wait time aspect. Will we receive notice when the transport is scheduled that wait time is approved? Also, if we transport a patient in our wheelchair, will the VA have a w/c to transfer the patient to while at the VA if we are not staying with the patient for the duration? And stretcher patients would we be approved to wait with patients who are on our stretchers or would we be transferring those patients to a VA stretcher/bed? Information in Performance Work

1. Can you provide the volume at each listed location? No standard number the patient vary from month to month.

1. How many patients are Dialysis patients? And are they transported locally? Estimated at 25; however, trip vary from month to month.

1. Contractor is required to have personnel performing services under this contract to have a background check conducted by the Gainesville VAMC Human Resource Department and will include having their fingerprints captured. This will involve a significant cost burden to the contractor to send personnel. Can this cost be included in the Pricing/Cost? The background check conduct will not be done by Gainesville VAMC.

13. The DoL advises that contractors get a wage assignment in writing from the contracting agency before bidding on a contract. What is the employee class assigned to drivers under this contract? Will it be Shuttle Bus Driver or Taxi Driver? 31290 - Shuttle Bus Driver

14. The contract indicates that only loaded miles shall be billed for. Is this the case even in circumstances where the pick-up and drop off locations are in remote areas? E.g. when a pick up is 75 miles away from any VA medical facility and the drop off is just 2 miles from the pick-up location, would the VA only pay a base rate for that trip and not consider the dead-head mileage the contractor incurred? Would the VA consider paying for unloaded miles on these types of trips to protect the contractor from a steep increase of dead-head/unloaded mileage? The choice program and recent legislation may result in dramatic increases over the life of the contract which could drastically skew the pricing submitted by vendors.

The VA will only pay a base rate for the trip.

15. Who is the incumbent contractor and what was their contract amount per year? New requirement

16. For the 45-minute response time, does this mean that all pickups must be completed within 45 minutes or that a vehicle must be dispatched and in route to a pick up within 45 minutes? It means that all pickups must be completed within 45 minutes, unless instructed otherwise.

17. In the case of dry runs cancelled on-site where the pick-up is at the furthest point in the service area in which the veteran needs to go to appointment 2 miles from his/her home, would the contractor only be entitled to the base rate for the trip or would the base rate + mileage driven to the pickup point be compensable? It would be the base rate only.
