Subject: LEAD-BASED PAINT (LBP) SURVEY Dorn VA Medical Center Building 100, 4th Floor South

6439 Garners Ferry Road Columbia, South Carolina

TES Project #2259-17

Dear Mr. Baston:

Trident Environmental Services, Inc. (TES) appreciates the opportunity to present this Lead-Based Paint (LBP) Survey that was completed on the 4th Floor South of Building 100 at the WJB Dorn VA Medical Center located in Columbia, South Carolina. The purpose of the LBP survey was to determine the general presence LBP on representative interior finishes in advanced of proposed renovation activities. A description of the survey procedures, the results of the field activities, and our conclusions and recommendations are presented in this report.

INTRODUCTION AND SITE ORIENTATION

TES was contracted to perform LBP testing on representative interior building components located on the 4th Floor South of Building 100 (i.e., primary hospital services building). TES personnel were escorted to the project area by Mr. Chris Rainey. The majority of the project area is not in use and includes patient rooms, nurses station, clean and soiled utility rooms, waiting room, staff offices, restrooms, break room, lockers and the pain clinic area (exam rooms/offices partially in use at pain clinic). Several rooms located within the project area remain in use for administrative purposes (i.e., offices and storage) and were not tested as part of this survey. It is noted the painted finishes in the administrative offices appeared consistent with other rooms in the project area.

In order to establish a consistent orientation during the survey procedures, side of the building located parallel to Garners Ferry Road was identified as SIDE A. The remaining three sides of the building were identified as SIDE B, SIDE C, and SIDE D, respectively, proceeding clockwise around the building. This identification system was also applied to the individual rooms and hallways in which testing was completed (i.e., wall located parallel to Garners Ferry Road is Wall A). Representative photographs of the project area are provided in Appendix A (see Photographs 1 to 9).

FIELD SURVEY ACTIVITIES

The LBP testing was performed by Mr. James Pease (EPA Certified Inspector/Risk Assessor #SC-R- 7570-4) on February 22, 2017. Certifications for Mr. Pease are provided in Appendix B. The LBP testing was performed using a Niton XLp (Model 300A and S/N 24485) to measure the lead content of surface coatings on wood, concrete, plaster, metal and tile that were observed to be painted, stained and/or shellacked. Please note tile surfaces (i.e., primarily restroom floors and lower walls) were tested when encountered due to the potential presence of lead in the surface glazing applied to older tile finishes.

One hundred seventy-one (171) XRF readings were recorded during the LBP survey procedures, to include instrument standardization and calibration checks that were performed at the beginning and end of the testing activities. All calibration readings were within acceptable limits. The XRF readings were evaluated against the following standards:

South Carolina Department of Health and Environmental Control (SCDHEC) regulations specify a positive determination of lead paint when the lead content is equal to or greater than 0.7 milligrams of lead per square centimeter (mg/cm2) of painted surface when measured by XRF.

United States Department of Housing and Urban Development (HUD) Guidelines which specify a positive determination of lead paint exists with an XRF reading greater than or equal to 1.0 mg/cm2.

Please note the Occupational Safety and Health Administration (OSHA) regulations do not recognize a percentage of lead by weight for definition purposes, only the presence or absence of lead content. Thus, OSHA considers any detectable amount of lead in paint to be subject to enforcement and requires training, engineering, and administrative controls for workers in accordance with 29 CFR 1926.62. Destructive actions to lead-based paint finishes (i.e., sanding, manual demolition, torch cutting, blasting, etc.) may create a lead exposure hazard and require compliance with the OSHA regulations, including employee exposure monitoring and proper disposal. The waste generated by renovation and/or demolition activities is also regulated. Components coated with lead-based paint may be disposed of in a Municipal Solid Waste (MSW) or Subtitle D landfill. Accumulations of lead paint (chips, blasting debris, etc.) must be analyzed by TCLP to determine if the debris is classified as “hazardous waste” (greater than or equal to 5 milligrams per kilogram (mg/kg).

DISCUSSION OF RESULTS

The results of this survey indicate lead concentrations in excess of 0.7 mg/cm2 and 1.0 mg/cm2 were consistently detected on several interior building components. For the purposes of this report, concentrations that exceeded the SCDHEC and/or HUD action levels are considered "Positive". A tabular summary of the XRF results is provided in Appendix C of this report. Building components located in the building that are identical in substrate and color to positive XRF testing combinations should also be presumed positive for lead throughout the project area. Please note the XRF results applicable for 4th Floor South only, and should not be used for other floors or buildings located at the WJB Born VA Medical Center Campus. Representative photographs of site conditions are provided in Appendix A. A general summary of the positive XRF results is provided below:

Positive XRF readings were detected on the lower tile walls (beige/grey color) located throughout the project area (Photographs 1, 5 and 10, Appendix A). These tile walls were observed in the project area hallway, clean and soiled utility rooms, staff restrooms, pain clinic restrooms and the bathrooms within patient rooms. The tile walls were observed in good/intact condition and should be considered positive throughout the project area. It is noted positive XRF readings were not detected on the blue floor tile that is prevalent throughout the project area restrooms and some wall coverings.

A positive XRF reading was detected on an accessible structural steel beam located on Wall B within the HVAC room (Photograph 11, Appendix A). The beam was painted white and the condition of the paint was observed intact. All white-painted structural beams should be presumed positive for lead. It is noted structural beams located above the ceiling were not accessible and observed covered with sprayed insulation material (i.e., painted surface, if present, was not exposed).

A positive XRF reading was detected on a beige tile wash station located within the pain clinic (Photograph 12, Appendix A). The beiger tile was observed in good/intact condition.

Positive XRF readings were consistently detected on white-painted metal stairwell components located within the Stair 2 stairwell and landing (Photograph 13, Appendix B). Components that tested positive include, handrail, newel post, riser and stringer. The paint was observed in good/intact condition. All similarly painted stairwell components in the project area should be presumed positive for lead.

Please note the summary above is not inclusive of all positive XRF readings and does not include those surfaces which exceeded the OSHA standard of any detectable amount. Appendix C includes a tabular summary of XRF data that is organized by room and includes specific testing locations with corresponding results.

CONCLUSIONS AND RECOMMENDATIONS

The presence or absence of lead-based paint applies only to tested or assessed surfaces on the date of the field testing with the understanding that conditions may change due to deterioration or maintenance. Please note some painted surfaces may contain levels of lead below the SCDHEC and/or HUD action levels which could create lead-dust hazards if the paint is turned into dust by abrasion, scraping, or sanding. Based on the consistent detection of lead throughout the project building, TES recommends implementation of lead safe work practices during renovation activities to comply with OSHA requirements.

Additionally, formal Clearance Testing (i.e., dust wipe testing) should be performed throughout the site structure after all future renovation work has been completed in order to ensure the facility is safe for re-occupancy by hospital personnel and patients. Clearance Testing is designed to ensure renovation activities did not create unacceptable exposure hazards associated with lead-contaminated dust and serves as a mechanism by which the quality and effectiveness of lead abatement safeguards/containment procedures can be evaluated.

In the event building components that tested positive for lead are disturbed during renovations, then contractors and workers should be informed as to the presence of LBP. These workers should also be informed about appropriate work practices and personal protective equipment used to prevent exposure to lead dust/fumes and/or to prevent conveying lead contamination from the work site. Building components containing lead should be handled and disposed of in accordance with federal and state regulations if removed from the building. In summary, our understanding is that components containing lead should be segregated from other building materials, wrapped in plastic, secured, and disposed of at a Municipal Solid Waste Landfill, also termed Subtitle D landfill. Please refer to State (SCDHEC) guidelines for additional information about the state-specific requirements regarding the disposal of materials containing lead paint including Toxicity Characteristic Leaching Procedure (TCLP) analysis.

CLOSING

We appreciate your selection of TES for this project and would value the opportunity to be of continued service when a future need arises. Please call Mr. Kevin Leedy [843-670-0897] with any questions.

Sincerely, TRIDENT ENVIRONMENTAL SERVICES, INC.

Mr. James Pease EPA Risk Assessor #SC-R-7570-4
