INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO BIDDERS/OFFERORS
THIS SECTION IS FOR SOLICITATION PURPOSES ONLY. THIS SECTION WILL BE PHYSICALLY REMOVED FROM ANY RESULTANT AWARD BUT WILL BE DEEMED INCORPORATED BY REFERENCE.
(a) GENERAL INSTRUCTIONS:
1. All proposal preparation cost will be the sole responsibility of the Offeror. The Government will not reimburse any firm for their proposal preparation cost.
2. The price, technical response and past performance of each Offeror will be evaluated independently. Offerors shall separately document each of the three factors. Each of the three offer sections shall therefore be labeled with the Offerors name, business address, and VA Solicitation Number.
3. To assure timely and equitable evaluation of proposals, Offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including these instructions, the terms and conditions, representations and certifications, and technical and past performance requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
The offer shall consist of three (3) separate parts:
• Part I – TECHNICAL CAPABILITY
• Part II – PAST PERFORMANCE
• Part III – PRICE
(b) SPECIFIC INSTRUCTIONS
1. PART I - TECHNICAL PROPOSAL – Submit electronic PDF document(s) by email.
Management Approach, Corporate Experience, Key Personnel Qualifications, Construction Schedule, Phasing Plan, Infection Control Plan, Quality Control Plan, Safety Plan and the Contractors plan to minimize interference.

a. Past Performance Questionnaires: The contractor shall send out the enclosed past performance questionnaires to each of their references listed under the Experience technical factor or any other references that the Offeror deems necessary. The Government will evaluate the quality and extent of Offeror’s performance deemed relevant and recent to the requirements of this solicitation.
b. The references must return the survey directly to the Government Contracting Officer listed on the survey, via email.
3. PART III - PRICE PROPOSAL - Submit electronic PDF document(s) by email.

TAB B: Offer, Guarantee, and Certifications
a. Complete the SF 1442, Blocks 14, 15, 17, 19, 20A, 20B, 20C and 30. An official having the authority to bind your company contractually must sign and date the SF 1442 Block 30b. One copy of the SF 1442 must bear an original, authorized signature. In doing so, the Offeror accedes to the contract terms and conditions as written in the RFP. All sections of the RFP, excluding Instructions to Offerors and Evaluation Procedures Sections, constitute the model contract.
b. Complete the PROPOSAL ITEM LIST on Attachment A. As part of their proposal, each Offeror shall be required to complete the attached Base Proposal Item Price Breakdown Form for the Base Proposal Item only. (Solicitation Attachment A)
c. Complete 52.204-8 Annual Representations and Certifications, paragraph (d) applies to this contract. 52.209-5--complete sections (a) (1) and (2), and 52.209-7 paragraph (b). Print out these provisions in their entirety and submit it with the proposal. Offerors shall submit only those pages that require fill-in.
d. Acknowledgement of Amendments
e. Copy of VetBiz Registry printout showing your firm as a verified SDVOSB.
f. Not completing 52.209-7(b), Information Regarding Responsibility Matters (OCT 2018), will render the proposal technically unsatisfactory and will eliminate the proposal from any further consideration for award.
(c) GENERAL INFORMATION
1. Format for proposal Part I & II shall be as follows:
a. A page is defined as one face of an 8 ½” x 11” sheet of paper containing information.
b. Typing shall not be less than font size 12.
2. Offers shall be submitted via email to the Contracting Specialist, Keith Norman, at Keith.Norman2@va.gov prior to the due date and time identified in the solicitation. Multiple emails will likely be required to submit all attachments. Server capacity is 10MB for file attachments, per email.
3. Each response shall address each factor in the sequence listed and clearly identify which factor is being addressed.
4. There shall be no mention of costs in the Technical Proposal.
5. FEDERAL HOLIDAYS: The following Federal Legal Holidays are observed by the Contracting Office and the Hampton VA Medical Center:
• New Year’s Day – 1 January
• Martin Luther King’s Birthday – Third Monday in January
• Presidents Day – Third Monday in February
• Memorial Day – Last Monday in May
• Independence Day – 4 July
• Labor Day – First Monday in September
• Columbus Day – Second Monday in October

RFI Questions for VAMC Hampton
1. Is it the intent of the specification to remove all of the gravel surfacing of the existing roof embedded in the asphalt or coal tar pitch?
Answer: The intent is to remove all loose gravel
2. What is the composition of the existing roof on each section (material in place and type of roof deck)? Answer: Various throughout buildings
3. Are the active leaks in any of the areas where new roofs are being installed?
Answer: Yes, most buildings
4. Has an infrared or other moisture survey being performed? Is a survey required prior to roofing over the existing system? Are the roofs saturated with water?
Answer: No moisture survey, No survey required, some areas have some water puddling
5. Are traditional methods of PVC attachment/installation allowed?
Answer: No all attachment methods must follow plans or be approved by COR if manufacturer instructions conflict.
6. Do the walk pads embedded in asphalt or coal tar pitch need to be removed?
Answer: No
7. Who is responsible for any electrical or mechanical work on this project?
Answer: Prime Contractor
8. Are all the drains open and functioning properly?
Answer: As far the VA knows the drains function properly
9. Alterative Roofing Solutions and ERC share the same mailing address. How can we be assured that bidders are going to receive he same warranty and material pricing?
Answer: See Part 2.C.3 allowing for other manufacturers
10. The roofing specification appears to be proprietary with only one manufacturer listed. This is a very limited specific roofing specification. The windspeed and 30-year warranty are not typical for other VA jobs. Most VA roofing projects remove all existing roofing and build roofs to specific codes.

· Is the Project Tax Exempt. If yes, will a tax exempt certificate be issued?
Answer: No it is not
· What is the on-site personnel requirement for the project? Can the SSHO, QCM and Superintendent be the same individual, provided has the qualification to perform the duties for all posts.
· Answer: Superintendent may share the SSHO or QCM responsibilities. SSHO and QCM can be the same individual. There must be 2 personnel onsite. Must have the proper qualifications.
· Please provide Asbestos survey report for all the buildings included in the project?
Answer: Providing survey.
· Will a third party air monitoring still be required, for the abatement activity being in the roof and in an open environment? If required whet is the extend of the oversight?
Answer: No, Third party air monitoring is only required for patient areas
· Please, provide/confirm availability for location of GC trailer, parking for crews and crane laydown area. where can the GC plan on being provided a staging area? will there be staging / laydown areas at all buildings? Provide sizes and locations.
Answer: Trailer and crew parking will be located in the Medical contractor Laydown area approximately 200 yards west of building 15. Staging areas for each building are noted on T-1 of drawings.
· Define locations of all ICRA barriers for all buildings and their respective levels of coverage. Including the route to access the roofs of each building.
Answer: See notes on drawing T-2
· Confirm electrical availability to project trailer. Is temp power available?
Answer: Cannot guarantee any power availability
· Are cranes to have safety fencing surrounds?
Answer: Must comply with all OSHA requirements. Also see notes from drawing T-2
· If needed, Are crane area capable for proposed to loads to be imposed?
· Can crane work be performed on any day of the week? Are there specific lift hours and requirements?
Answer: Lifts can be done during normal business hours if they don’t interfere with routine daily medical center activities (entrances/exits). Lift plans must be approved and coordinated through COR Via VA safety. If the lift plan affects Medical Center activities contractor will be required to complete after normal business hours.
· Will it be required to have fencing around the full perimeter of building site? Answer: See Answer: Drawing T-2
· Please provide required rating, basis of design for scaffolding and guard rails. Provide overall building heights from grade for scaffolding Answer: This is a Prime Contractor responsibility
· Are there any notable abandoned equipment in any of the roofs to be removed.
Answer: See drawings
· Are there any notable equipment in any of the roofs to be temporarily removed for roofing work to be reinstalled later?
Answer: See drawings
· Is new lightning protection required to be installed in any of the buildings mentioned in the project? If yes, Provide specification and details Answer: Re-use existing lightning protections as per drawings
· What are the acceptable manufacturers for the required roof membrane?
Answer: See Specifications for requirements
· The bid as of now is due on January 19th, 2021. Depending on when we will have answers on these RFIs, should we expect on an extended bid due date, if there is less than five week days between the bid date and RFI answers?
Answer: Proposal due date has been moved to 1/26/21

Project # 590-19-101 Replace Roofs Various Buildings, at Hampton VA Medical Center
RFI 1. Part 1 of Section 07 54 00, references a full tear off and overlay of existing roof system. Is the intent of the designer to remove the existing roof entirely or overlay existing roof?
Answer: Overlay Existing Roof RFI 2. The 150 MPH wind uplift warranty is not offered by any major manufacturer. The specified manufacturer, Environmental Roofing Components (ERC) offers a 150 MPH uplift warranty for a wind vented roof system is also shares same addresses as a licensed contractor, Alternative Roofing Solutions (ARS).
Answer: See Part 2.C.3 allowing for other manufacturers
RFI 3. Is there a specific core sample of the existing roof system?
Answer: No except for asbestos testing see report
RFI 4. Has an engineering report been provided for the overlay of the existing roofs?
Answer: See drawings for existing conditions
RFI 5. Are there As-builts available? If so, will a copy be provided?
Answer: No
RFI 6. Are alternative roofing systems being considered?
Answer: No
RFI 7. Can a copy of the specified 150 MPH and 30 year NDL warranty be provided?

1) 075400, PART 2, C, 2- Manufacturer listed as basis of design, Environmental Roof Component, and a roofing contractor from the sign in sheet, Alternative Roofing Solutions, share the same address in both their South Carolina and North Carolina offices. Is there a mutual ownership interest between the two companies? Is this a conflict of interest for the VA Hospital? How can bidding contractors, that do not share the same office as the basis of design manufacturer, be assured they will receive the same material fee and warranty pricing?
Contracting needs to address this question. We have no information on companies or subs. The project allows for equal or better (See 075400 part 2.C.3)
2) Specifications references both a monolithic deck in Part D, and a steel deck? What are the deck types?
Answer: Various throughout buildings
3) Plan Pages D1-D6: Gravel removal is noted on the demolition pages. To what extent should the gravel be removed out of the existing coal tar roof? Loose gravel? Top layer of roof?
Answer: Loose Gravel
4) 07600 Section requires edge metal to be subject to FM standards. Section 75400 has no mention of FM, is it the intent of the specifications for the roof system in 075400 to be subject to FM requirements as well?
Answer: YES
5) 075400, PART 2, A- For warranty purposes where is the 150 mph measured? Is this a roof top wind speed or 3 meters above ground?
Answer: Rooftop
6) Has a moisture scan been performed on the existing roof sections? If so will this report be made to the offerors?
No there hasn’t been any moisture scans
7) 075400, PART 2, A- The manufacturer listed in the specification does not directly manufacture the PVC membrane and is a private labeler of PVC by others, so would an equivalent third party warranty be acceptable by a submitting manufacturer? Are there any other qualifications of this warranty other than 30 year, 150 mph and NDL? Any other limiting factors? Ex/ amount of escrow in warranty fund supporting warranty claims , act of god exclusions, void of warranty due to neglect?

8) Can a sample warranty from the basis of design manufacturer be made available to the bidders in an effort to provide some transparency and to better understand what other manufacturers might comply with the intent of the specified requirements?

9) After review with other PVC and Vent System manufacturers, there appears to be no other PVC or Vent/Valve system manufacturer who will meet these specifications as they are written. Is there another manufacturer, other than the listed basis of design, that meet these specifications? Is the intent of this specification to limit the competition to only one manufacturer?
Answer: There is no limiting competition. There are other companies that can meet the specifications. (2001 company, Windsmart, etc.)
