About this file
This document is a Quality Assurance Surveillance Plan (QASP) for Reference Laboratory Services for VISN1 (Veterans Integrated Service Network 1). The QASP outlines performance standards and surveillance methods for a contract involving primary reference laboratory testing services, phlebotomy, and courier services across VISN1 facilities.
Key performance standards include: 100% reporting of consultative case reports, 98% accuracy of report details, maintaining licensing and accreditation, ensuring specimen safety and integrity, meeting delivery times, maintaining turnaround times for tests, and correctly identifying patients. The government will use periodic inspections, random sampling, and contractor reporting to monitor performance. The surveillance will use a color-coded rating system (Green/Yellow/Red) to evaluate the contractor's performance, with potential consequences including non-exercise of option periods for unsatisfactory performance. The Contracting Officer's Representative (COR) will assess contract performance periodically and maintain quality assurance worksheets to be submitted within 30 days of contract expiration.
