About this file

This document is Addendum Number 1.0 to Contract Number C4698B between the State of Wisconsin Department of Corrections and Wisconsin Lock and Load, a company based in Green Bay, Wisconsin. The addendum modifies an existing contract for extradition transport and security services that was originally executed for the period of July 1, 2015 through June 30, 2017, with three possible one-year renewal options by mutual agreement.

The contract covers extradition transport and security services for transporting Department of Corrections offenders. The addendum specifically adds a new requirement mandating that Wisconsin Lock and Load must notify the Contract Administrator and Contract Manager within two weeks of any traffic incidents, crashes, or wrecks involving their vehicles while transporting DOC offenders. The contractor must provide the names of all Wisconsin DOC offenders on the transport vehicle during the incident and include a brief synopsis of the event. The addendum was executed in October 2015 and was digitally signed by Brian Westphal (Director of Operations for Wisconsin Lock & Load), Sheri Hicks (Records Director), and Edward F. Wall (Secretary of the Wisconsin Department of Corrections).
