About this file
The document is Attachment A, a Small Business Subcontracting Plan form from the Virginia Department of Small Business and Supplier Diversity (DSBSD). This form is designed for potential bidders to detail their plans for utilizing subcontractors, specifically focusing on DSBSD-certified small businesses, women-owned businesses, minority-owned businesses, and service-disabled veteran-owned businesses. Bidders must complete this document as part of their bid response to be considered responsive, and they are required to indicate whether they plan to use subcontractors or complete all work independently.
The form provides space for bidders to list up to five potential subcontractors, including details such as company name, contact information, SBSD certification number, certification type, value of subcontract (percentage or dollar amount), and description of work. The document emphasizes that proposed subcontractor participation will be incorporated into the subsequent contract and become a requirement, with potential contract breach if the proposed participation levels are not met. Certification for small businesses is available through the DSBSD website, and the plan aims to promote opportunities for small and diverse businesses in the procurement process.
